상세 설명
현재 재발성 또는 불응성 원발성 중추신경계(CNS) 림프종에 대한 표준 치료는 없습니다. 고용량 메토트렉세이트 또는 방사선 요법 후 재발된 질병에 대한 최선의 접근 방식은 정의되지 않았습니다. 일반적인 관행은 이식 전에 이 환자 집단에서 컨디셔닝 요법으로서 요법 RBEAM입니다. RBEAM 요법에는 R(리툭시맙), B(BCNU), E(에토포사이드), A(Ara-C(시타라빈)) 및 M(멜팔란)이 포함됩니다. 또한, 덱사메타손은 RBEAM 니모닉에 언급되지 않았지만 요법에 포함됩니다. 그러나 이 조합에 사용되는 멜파란은 CNS 침투력이 그다지 높지 않은 것으로 생각됩니다. 따라서 본 연구에서는 멜팔란 대신 CNS에 침투하는 것으로 알려지고 FDA의 승인을 받은 알킬화제인 테모졸로마이드를 사용하고 평가할 예정입니다.

설명
포함 기준:
- ≥ 18세 및 ≤ 75세 환자
- 환자는 성숙한 B 세포 비호지킨 림프종과 함께 중추신경계(CNS) 침범이 있어야 합니다(WHO 기준).
환자는 아래 기준 중 하나를 충족해야 합니다.
- 중추신경계(CNS) B 세포 림프종에 대한 초기 치료 후 완전 반응(CR) 또는 부분 반응(PR)을 달성한 환자, 또는
- 구제 화학 요법 후 후속 CR 또는 PR을 달성한 CNS B-세포 림프종 치료 후 재발 또는 진행된 질병이 있는 환자, 또는
- 초기에 CNS B 세포 림프종 치료에 반응하지 않았지만 구제 화학요법 요법 후 CR 또는 PR을 달성한 환자, 또는
- 전신성 B세포 비호지킨 림프종에서 중추신경계 재발이 발생하고 화학요법 민감성 림프종의 증거가 있는 환자.
- 자가 줄기 세포 이식에 적합한 환자
- 서면 동의서를 이해하고 서명할 의사가 있는 환자
제외 기준:
- 악성 이외의 질환으로 기대여명이 심하게 제한되는 환자
- Karnofsky 성능 점수 <60
- 임신 중이거나 수유 중인 환자
- HIV 혈청 양성인 환자
- 적절한 치료 후 1개월 이상 진행된 제어되지 않는 감염(추정되거나 문서화됨)이 있는 환자
- 증상이 있는 관상 동맥 질환, 조절되지 않는 울혈성 심부전 환자. 좌심실 박출률은 측정하지 않아도 되지만 측정할 경우 박출률이 30% 미만인 경우 제외
- 보충적 연속 산소가 필요한 환자. DLCO는 반드시 측정할 필요는 없으나, 측정하더라도 DLCO가 35% 미만이면 대상에서 제외됩니다.
간 질환의 임상적 또는 실험실적 증거가 있는 환자는 간 질환의 원인, 간 기능 및 조직학 측면에서 임상적 중증도, 문맥 고혈압 정도에 대해 평가됩니다. 다음과 같은 간 기능 이상이 있는 환자는 제외됩니다.
- 급성 간부전
- 간문맥 고혈압 또는 가교 섬유증의 증거가 있는 간경변증
- 알코올성 간염
- 식도정맥류
- 식도정맥류 출혈 병력
- 간성뇌증
- 프로트롬빈 시간의 연장으로 입증되는 수정 불가능한 간 합성 기능 장애
- 문맥 고혈압과 관련된 복수
- 총 혈청 빌리루빈 >3mg/dL인 만성 바이러스성 간염
- 증상이 있는 담도 질환
- 비B세포 림프종 또는 림프종이 아닌 뇌종양이 있는 환자는 연구에서 제외됩니다. 비 B 세포 림프종에는 다음이 포함됩니다. 모든 T 세포 림프종, 자연 살해(NK) 세포 림프종 및 호지킨 림프종
- 줄기세포 채취량이 부족한 환자(<2 X 106/kg)

실험적: DRBEAT 요법
|
DRBEAT 요법은 RBEAM과 유사합니다.
Rituximab과 Carmustine은 -6일에 주어질 것입니다.
에토포사이드와 시타라빈은 -5일에서 -2일 사이에 제공됩니다.
Temozolomide는 Day -5에서 Day -1까지 5일에 걸쳐 분할 용량으로 제공됩니다.
EWOC(Escalation with overdose control)로 알려진 용량 증량 설계는 이 연구를 위한 테모졸로마이드의 목표 용량을 결정하는 데 사용될 것입니다.
5일에 걸쳐 제공되는 시작 용량은 250mg/m2(누적 총 용량 1250mg/m2)에서 시작하며, 이는 이전 데이터에서 안전한 용량임을 나타냅니다.
