De toevoeging van temozolomide aan conditionering voor autologe transplantatie bij gerecidiveerd en refractair CZS-lymfoom (DRBEAT)
Een fase 2a-onderzoek naar de toevoeging van temozolomide aan een standaard conditioneringsregime voor autologe stamceltransplantatie bij gerecidiveerd en refractair centraal zenuwstelsel (CZS) lymfoom
Het primaire doel van de studie is het testen van de dosering van temozolomide om de doeldosis te vinden die een persoon kan verdragen. Het andere deel van de studie zal bepalen hoe nuttig het kan zijn voor CZS-lymfoompatiënten door temozolomide toe te voegen aan het "conditioneringsregime" voorafgaand aan stamceltransplantatie.
Deze onderzoeksstudie is bedoeld om het experimentele gebruik van temozolomide te testen als onderdeel van een conditioneringsregime voorafgaand aan stamceltransplantatie. Dit medicijn is nog niet goedgekeurd door de Amerikaanse Food and Drug Administration (FDA) voor gebruik bij stamceltransplantatie bij lymfomen van de hersenen (centraal zenuwstelsel of CZS), maar het is eerder onderzocht en gebruikt bij transplantatie met redelijke resultaten.

Momenteel is er geen standaardzorg voor recidiverend of refractair primair centraal zenuwstelsel (CZS) lymfoom. Na een hoge dosis methotrexaat of bestraling is de beste aanpak van recidiverende ziekte niet gedefinieerd. De gangbare praktijk is het regime RBEAM als conditioneringsregime in deze patiëntenpopulatie voorafgaand aan transplantatie. Het RBEAM-regime omvat R (rituximab), B (BCNU), E (etoposide), A (Ara-C (cytarabine)) en M (melphalan). Bovendien is dexamethason opgenomen in het regime, hoewel niet vermeld in het RBEAM-ezelsbruggetje. Er wordt echter aangenomen dat de melfalan die in deze combinatie wordt gebruikt niet veel CZS-penetratie heeft. Daarom zal in deze studie temozolomide, een alkylerend middel waarvan bekend is dat het het centrale zenuwstelsel binnendringt en door de FDA is goedgekeurd voor hersentumoren, worden gebruikt en geëvalueerd in plaats van melfalan.
Het doel van deze studie is het bepalen van een effectieve en veilige dosis temozolomide die oraal wordt toegediend aan patiënten met gerecidiveerd primair CZS-lymfoom gedurende de 5 dagen voorafgaand aan autologe stamceltransplantatie. De hoop is dat het conditioneringsregime DRBEAT [D (dexamethason) (R (rituximab), B (BCNU), E (etoposide), A (Ara-C (cytarabine)) en T (temozolomide)] de overleving van patiënten met recidiverend CZS-lymfoom.

Beschrijving
Inclusiecriteria:
- Patiënten ≥ 18 jaar en ≤ 75 jaar
- Patiënten moeten betrokken zijn bij het centrale zenuwstelsel (CZS) met een volwassen B-cel non-Hodgkin-lymfoom (WHO-criteria)
Patiënten moeten aan een van de onderstaande criteria voldoen:
- Patiënten die een complete respons (CR) of partiële respons (PR) hebben bereikt na initiële therapie voor B-cellymfoom van het centraal zenuwstelsel (CZS), OF
- Patiënten met recidiverende of gevorderde ziekte na therapie voor CZS B-cellymfoom die een volgende CR of PR hebben bereikt na salvage-chemotherapie, OF
- Patiënten die in eerste instantie ongevoelig zijn voor therapie voor B-cellymfoom van het CZS maar die een CR of PR hebben bereikt na een salvage-chemotherapieregime, OF
- Patiënten die een CZS-terugval hebben ontwikkeld door systemisch B-cel non-Hodgkin-lymfoom en die bewijs hebben van chemotherapiegevoelig lymfoom.
- Patiënten geschikt voor autologe stamceltransplantatie
- Patiënten die een schriftelijk geïnformeerd toestemmingsdocument kunnen begrijpen en willen ondertekenen
Uitsluitingscriteria:
- Patiënten van wie de levensverwachting ernstig wordt beperkt door andere ziekten dan maligniteit
- Prestatiescore Karnofsky <60
- Patiënten die zwanger zijn of borstvoeding geven
- Patiënten die hiv-seropositief zijn
- Patiënten met een ongecontroleerde infectie (vermoedelijk of gedocumenteerd) met progressie na een geschikte therapie gedurende meer dan een maand
- Patiënten met symptomatische coronaire hartziekte, ongecontroleerd congestief hartfalen. Linkerventriculaire ejectiefractie hoeft niet te worden gemeten, maar als het wordt gemeten, wordt de patiënt uitgesloten als de ejectiefractie <30% is
- Patiënten die continu aanvullende zuurstof nodig hebben. DLCO hoeft niet te worden gemeten, maar als het wordt gemeten, wordt de patiënt uitgesloten als DLCO <35%.
Patiënten met klinische of laboratoriumaanwijzingen voor leverziekte zullen worden beoordeeld op de oorzaak van de leverziekte, de klinische ernst ervan in termen van leverfunctie en histologie, en op de mate van portale hypertensie. Patiënten met een van de volgende afwijkingen in de leverfunctie worden uitgesloten
- Fulminant leverfalen
- Cirrose met bewijs van portale hypertensie of overbruggende fibrose
- Alcoholische hepatitis
- Slokdarmvarices
- Een voorgeschiedenis van bloedende slokdarmvarices
- Hepatische encefalopathie
- Oncorrigeerbare synthetische hepatische disfunctie blijkt uit verlenging van de protrombinetijd
- Ascites gerelateerd aan portale hypertensie
- Chronische virale hepatitis met totaal serumbilirubine >3 mg/dL
- Symptomatische galziekte
- Patiënten met niet-B-cellymfomen of hersentumoren die geen lymfomen zijn, worden uitgesloten van het onderzoek. Niet-B-cellymfomen omvatten: elk T-cellymfoom, natural killer (NK)-cellymfomen en Hodgkin-lymfomen
- Patiënten bij wie een onvoldoende aantal stamcellen (<2 X 106/kg) is verzameld

Experimenteel: DRBEAT-regime
|
Het DRBEAT-regime zal vergelijkbaar zijn met RBEAM.
Rituximab en Carmustine krijgen Dag -6.
Etoposide en Cytarabine worden gegeven op Dag -5 tot -2.
Temozolomide zal worden gegeven via verdeelde doses over vijf dagen, beginnend op Dag -5 tot Dag -1.
Een dosisescalatieontwerp, bekend als EWOC (escalatie met overdoseringscontrole), zal worden gebruikt om de doeldosis temozolomide voor dit onderzoek te bepalen.
De startdosis die gedurende vijf dagen wordt gegeven, begint bij 250 mg/m2 (cumulatieve totale dosis van 1250 mg/m2), aangezien eerdere gegevens aangeven dat dit een veilige dosis is.
Op basis van de gerapporteerde dosisbeperkende toxiciteiten van de vorige patiënten, zal de EWOC statistische modellering worden uitgevoerd om het volgende dosisniveau te bepalen.

Veiligste dosis Temozolomide voor het DRBEAT-regime

De veiligheid zal worden beoordeeld met behulp van een dosisescalatieontwerp voor het gebruik van temozolomide om de doeldosis te bepalen en ook om alle acute behandelingsgerelateerde toxiciteiten te evalueren.
Tijdens de follow-up en therapie van de patiënt zullen de toxiciteiten worden geëvalueerd, vooral omdat de onderzoekers de beoogde dosis temozolomide zullen bepalen.
Een van de belangrijkste criteria voor dosisbeperkende toxiciteit voor het onderzoek zal elke graad 3 of 4 niet-hematologische toxiciteit zijn uit een lijst van algemeen verwachte toxiciteiten die verband houden met autologe transplantatie en temozolomide.
