상세 설명
퇴행성 요추 척추 협착증(DLSS) 또는 척추의 골관절염은 노인의 통증, 장애 및 독립성 상실의 주요 원인입니다. 이는 척추 신경의 압박 및 허혈(신경-허혈)로 이어질 수 있는 척추관의 연령 관련 퇴행성 협착(협착)으로 인해 발생하는 만성 질환입니다. DLSS의 임상 증후군은 신경성 파행으로 알려져 있습니다. 양측 또는 일측 엉덩이 및/또는 하지 통증, 무거움, 무감각, 따끔거림 또는 쇠약이 특징이며, 걷거나 서 있을 때 촉발되고 앉거나 앞으로 구부리면 완화됩니다. 제한된 보행 능력은 DLSS로 인한 지배적인 기능 장애입니다. 고통받는 사람들은 무릎 또는 고관절 골관절염 환자보다 보행 제한이 더 크고 울혈성 심부전, 만성 폐쇄성 폐질환 또는 전신성 홍반성 루푸스 환자보다 기능 제한이 더 큽니다. 포함 기준:
- 50세 이상
- 등 및/또는 방사성 하지 또는 엉덩이 통증의 임상 증상; 걷거나 서 있을 때 악화되고 앉으면 완화되는 하지의 피로 또는 감각 상실
- 진행성 신경학적 기능 장애가 없는 간헐적 또는 지속적인 통증
- 3개월 이상의 증상 및 징후 지속 기간
- MRI, CT 스캔, 척수 조영술 또는 초음파를 사용하여 영상으로 확인된 척추관 협착
- 영상으로 확인된 협착의 분절 수준에 해당하는 임상 징후 및 증상
- 퇴행성 척추전방전위증 환자가 포함됩니다.
- 수술 후보(향후 12개월 이내) 또는 수술을 원하지 않는 환자로 간주되지 않음
- 가벼운 운동을 할 수 있습니다.
- 보조 장치 없이 최소 20미터 이상 30분 미만 지속적으로 걸을 수 있음

- 난치성 통증과 진행성 신경학적 기능 장애를 동반한 심한 퇴행성 협착증
- 퇴행성으로 인한 것이 아닌 요추부 척추관협착증
- 지난 12개월 동안 진단된 요추 추간판 탈출증
- 요추 척추 협착증 또는 불안정성에 대한 이전 허리 수술
- 강직성 척추염, 신생물, 감염 또는 대사성 질환과 같은 근본적인 척추 장애
- 혈관 질환으로 인한 간헐적 파행
- 제한된 보행 능력을 유발하는 하지의 심한 골관절염 또는 관절염
- 당뇨병을 포함한 하지의 기능 장애를 유발하는 신경계 질환

이 하위 그룹으로 무작위 배정된 참가자는 면허가 있는 척추 지압사에 의해 L3에서 S1까지 척추주위 근육에 일회용 자가 접착식 전기 패드(Blue Sensor P, Ambu A/S, 덴마크)를 부착하게 됩니다.
전기 패드 전극은 TENS 기계(Verity Medical Ltd U.K.의 NeuroTrac TENS)에 연결되어 대상자가 허리 주머니 안에 감춰 착용하게 됩니다.
TENS는 100-200usec의 펄스 폭으로 3초 간격으로 변조된 65-100Hz의 주파수에 대해 프로그래밍되고 시작 2분 전과 SPWT 중에 켜집니다.
현재 강도는 파일럿 실험에서 약 3 밀리암페어로 환자가 편안함을 느낄 수 있는 수준으로, 근육 경련을 일으키는 수준 이하로 설정됩니다.
