이 3상 시험은 3기 또는 4기이거나 재발(재발)한 자궁내막암 환자를 치료할 때 파클리탁셀 및 카보플라틴 단독과 비교하여 펨브롤리주맙, 파클리탁셀 및 카보플라틴의 조합이 얼마나 잘 작용하는지 연구합니다.
펨브롤리주맙과 같은 단일클론항체를 사용한 면역요법은 신체의 면역체계가 암을 공격하는 데 도움이 될 수 있으며 종양 세포의 성장 및 확산 능력을 방해할 수 있습니다.
파클리탁셀과 카보플라틴은 이러한 유형의 암에 대한 일반적인 치료 접근법의 일부로 사용되는 화학요법 약물입니다.

기본 목표:
I. 진행성 병기(측정 가능한 III기 또는 IVA기), IVB기 및 재발성 자궁내막암 환자에서 파클리탁셀 및 카보플라틴과 병용한 펨브롤리주맙(MK-3475)의 효능을 평가합니다.
2차 목표:
I. 각 치료군에 대한 CTCAE(Common Terminology Criteria for Adverse Events)에 의해 평가된 독성의 특성, 빈도 및 정도를 결정하기 위해.
II. 치료군별 및 MMR(mismatch repair) 면역조직화학(IHC) 상태에 의해 고형 종양 반응 평가 기준(RECIST) 1.1에 의해 평가된 맹검 독립 중앙 검토(BICR) 평가 또는 조사관 평가 객관적 반응률(ORR)을 평가하기 위해 측정 가능한 질병으로 연구에 참여하십시오.
III. 측정 가능한 질병을 가지고 연구에 참여하는 환자에서 치료군 및 MMR IHC 상태에 의해 BICR 평가 또는 조사자가 평가한 반응 기간(DOR)을 평가하기 위함.
IV. 불일치 복구 단백질 숙련도(pMMR) 또는 불일치 복구 결핍(dMMR)이 있는 환자의 전체 생존(OS)에 대한 펨브롤리주맙(MK-3475)의 효과를 평가합니다.
V. 표준 병용 화학요법에 펨브롤리주맙(MK-3475)을 추가하는 것이 환자 보고 결과 측정 정보 시스템(PROMIS)으로 측정할 때 환자가 보고한 신체 기능 개선과 관련이 있는지 확인하기 위해-신체 기능 척도(약식), 품질 pMMR 환자에서 FACT(Functional Assessment of Cancer Therapy) - En TOI(Endometrial Trial Outcome Index)로 측정한 수명 및 PROMIS-피로 척도(약식)로 측정한 피로 악화.
VI. 기관 MMR 면역조직화학(IHC) 검사와 중앙집중식 MMR IHC 사이의 일치성을 확인합니다.
탐구 목표:
I. PROMIS-신체 기능 척도(약식)로 측정한 환자 보고 신체 기능과 FACT-En TOI로 측정한 삶의 질 사이의 상관관계를 탐색합니다.
II. 표준 병용 화학 요법에 펨브롤리주맙(MK-3475)을 추가하는 것이 FACT/Gynecologic Oncology Group Neurotoxicity(GOG-Ntx) 하위 척도(단기)로 측정한 자가 보고 신경 독성과 관련이 있는지 여부와 환자가 pMMR 환자에서 암 치료의 부작용으로 인한 자체 보고된 귀찮음.
III. PD-L1(Programmed Death Ligand 1) IHC(PD-L1) IHC(양성 대 [vs] 부정).
IV. PD-L1 IHC(양성 대 음성)와 불일치 복구 상태(pMMR 및 dMMR) 사이의 연관성을 평가하기 위해.
개요: 환자는 2개의 팔 중 1개로 무작위 배정됩니다.
ARM I:
병용 단계: 환자는 각 주기의 1일에 30분에 걸쳐 위약을 정맥 주사(IV)하고, 각 주기의 1일에 3시간에 걸쳐 파클리탁셀 IV를, 각 주기의 1일에 30-60분에 걸쳐 카보플라틴 IV를 투여합니다. 치료는 질병 진행 또는 허용할 수 없는 독성이 없는 경우 6주기 동안 3주마다 반복됩니다. 여전히 측정 가능한 질병이 있는 안정적인 질병(SD) 또는 부분 반응(PR) 환자는 질병 진행 또는 허용할 수 없는 독성이 없는 상태에서 최대 총 10주기 동안 치료를 계속할 수 있습니다(주치의가 필요하다고 간주하는 경우).
유지 관리 단계: 환자는 각 주기의 1일에 30분에 걸쳐 위약 IV를 받습니다. 치료는 질병 진행 또는 허용할 수 없는 독성이 없는 경우 최대 14주기 동안 6주마다 반복됩니다.

- 측정 가능한 III기, 측정 가능한 IVA기, IVB기(측정 가능한 질병이 있거나 없는) 또는 재발성(측정 가능한 질병이 있거나 없는) 자궁내막암.
- 기관 MMR IHC 테스트 결과를 보여주는 병리학 보고서.
- 원발성 종양의 조직학적 확인이 필요합니다(병리 보고서 제출 필요). 조직학적 유형이 다음과 같은 환자가 적합합니다: 자궁내막양 선암종, 장액성 선암종, 탈분화/미분화 암종, 투명 세포 선암종, 혼합 상피 암종, 달리 명시되지 않은 선암종(N.O.S.).
- 중앙집중식 MMR IHC 검사를 위한 종양 표본 제출은 1단계 이후와 2단계 등록 전에 필요합니다.
- 측정 가능한 질병이 있는 환자에서 병변은 RECIST 버전(v) 1.1에 의해 정의되고 모니터링됩니다. 측정 가능한 질병은 적어도 하나의 차원(기록할 가장 긴 직경)에서 정확하게 측정할 수 있는 적어도 하나의 병변으로 정의됩니다. 컴퓨터 단층촬영(CT) 또는 자기공명영상(MRI)으로 측정했을 때 각 병변은 10mm 이상이어야 합니다. 림프절은 CT 또는 MRI로 측정할 때 단축이 15mm 이상이어야 합니다.

- 자궁내막암 치료를 위해 이전에 화학요법을 받은 적이 없거나
- 이전 보조 화학 요법(예: 파클리탁셀/카보플라틴 단독 또는 동시 화학 요법 및 방사선 요법[시스플라틴 포함 또는 비포함]의 구성 요소)은 보조 화학 요법이 STEP 2 등록 전 >= 12개월 전에 완료되었습니다.
- 환자는 자궁내막암 치료를 위해 이전에 방사선 요법을 받았을 수 있습니다. 이전의 방사선 요법에는 골반 방사선 요법, 확장 필드 골반/대동맥 주위 방사선 요법, 질내 근접 요법 및/또는 완화 방사선 요법이 포함될 수 있습니다. 모든 방사선 요법은 STEP 2 등록 최소 4주 전에 완료되어야 합니다.
- 환자는 자궁내막암 치료를 위해 이전에 호르몬 요법을 받았을 수 있습니다. 모든 호르몬 요법은 STEP 2 등록 최소 3주 전에 중단해야 합니다.
- 이 임상시험에서 치료를 시작한 후, 그리고 질병 진행이 문서화되기 전에 간격 또는 종양 축소 수술은 허용되지 않습니다.

- 정상 범위 내의 갑상선 자극 호르몬(TSH). 갑상선 기능 장애의 증상이 없는데도 TSH가 정상 범위에 있지 않다면 정상적인 Free T4 수치가 필요합니다.
- 2단계 등록 후 6개월 이내에 감지할 수 없는 바이러스 부하로 효과적인 항레트로바이러스 요법을 받고 있는 인간 면역결핍 바이러스(HIV) 감염 환자가 이 임상시험에 참가할 수 있습니다.
