mSIM 연구는 건망증 경도 인지 장애(aMCI) 환자를 위한 모바일 기반 동시 운동 및 기억 기술 훈련 프로그램(mSIM)을 제공할 웹 기반 애플리케이션의 개발 및 실행 가능성 테스트를 포함합니다.
무작위 통제 시험(RCT)은 2개의 연구 부문(그룹 1 활동 모니터링 제어(Fitbit을 통해)(CON) 대 그룹 2 mSIM 개입과 Fitbit을 통한 활동 모니터링)을 사용하여 기억력 성능 및 일상 기능에 대한 mSIM의 효능을 평가합니다. mSIM 치료 반응은 신경심리학적 및 기능적 평가를 사용하여 평가됩니다. 말초 바이오마커 뇌 유도 신경영양 인자(BDNF) 및 노르에피네프린(NE)의 농도 수준도 평가됩니다.

상세 설명
이 연구의 목적은 모바일 장치에서 제공되고 고정식 자전거에서 사용할 수 있는 웹 기반 인지 보상 및 주로 기억력 훈련 과정을 만들고 기억력 및 기능적 능력에 대한 효능을 테스트하는 것입니다.
참가자는 (1) 프로비던스 세인트 존스 건강 센터와 제휴한 대형 의사 의료 그룹 내 대량 기억 관리 및 치매 외래 환자 클리닉인 산타모니카의 태평양 뇌 건강 센터에서 모집합니다. (2) 참가자 광고 (3) 관심 있는 개인이 임상시험 코디네이터에게 연락할 수 있는 웹사이트 랜딩 페이지 구축. 최대 5명의 참가자가 코스 베타 테스터가 됩니다. 그들은 과정의 잘린 버전을 받고 상세한 피드백을 제공할 것입니다. 30명의 참가자가 두 그룹 중 하나로 무작위 배정됩니다. 그룹 1(제어 팔)은 신체 활동 수준을 지속적으로 모니터링합니다(Fitbit을 통해). 참가자는 정보에 입각한 동의서(ICF)에 서명하기 전에 실험 연구 피험자의 권리 장전을 받게 되며, 보호 대상 건강 정보(PHI)의 사용 및 공개에 대한 승인, 피험자의 치료 의사에 대한 의료 기록 공개 승인을 받게 됩니다. 연구에 등록하기 전에 각 참가자. 모든 서명된 ICF의 사본은 참가자에게 제공되며 조사자는 원본을 보관합니다.
그룹 2(활성 팔)는 mSIM 개입과 활동 모니터링(Fitbit을 통해)을 받습니다. 그룹 2는 먼저 50% HRR(예비 심박수)에서 40분을 유지할 수 있을 때까지 2~6주 동안 운동 램프 업(주당 2회)을 수행합니다. mSIM 훈련의 강도는 40-60% HRR입니다. 램프업 단계 후 mSIM 참가자는 주당 2회의 60분 세션으로 구성된 12주 개입을 시작합니다. mSIM 치료 반응은 신경심리학적 및 기능적 평가를 사용하여 평가됩니다. 평가는 기준선, 시험 중간(3-4개월) 및 시험 후(5-6개월)에 수행됩니다. 기준선 및 시험 후(5-6개월)에서 주변 바이오마커 BDNF 및 NE의 농도 수준도 평가될 것입니다.

- 피험자는 정보에 입각한 동의 시점에 50~80세여야 합니다.
- 피험자는 본 연구에 등록한 후 6개월 이내에 임상 평가 기록을 제공하거나 스크리닝 시 연구 직원의 간단한 기억 평가를 통해 aMCI 진단을 확인해야 합니다.
- 본 연구의 중재는 현재 영어로만 제공되므로 피험자는 동의 및 연구 참여를 위해 영어 구어체 및 문어체에 능숙해야 합니다.
- 의학적 상태가 있는 피험자는 이러한 상태에 대해 안정적이어야 합니다. 약물에 대한 안정적인 제어가 허용됩니다.

제외 기준
- 피험자는 신경퇴행성 장애(예: 알츠하이머병, 레비 소체 치매, 전측두엽 치매)의 기존 진단이 없어야 합니다.
- 피험자는 심각한 심혈관 질환, 심각한 호흡기 질환, 질병 또는 부상, 약물 남용, 정신분열증, 양극성 장애 또는 기타 신경계 질환을 포함하여 인지 및 운동 능력에 영향을 미칠 수 있는 사전 진단이 없어야 합니다.
- 피험자는 MMSE(Mini Mental State Exam) 점수가 19점 미만이어서는 안 됩니다.
- 피험자는 이전 임상 치매 등급(CDR) 전체 점수가 2 이상이어서는 안 됩니다.
- 피험자는 이전에 또는 선별 평가 중에 몬트리올 인지 평가(MoCA)43 점수가 18 미만이 아니어야 합니다.
- 피험자는 연구 임상 신경 심리학자의 최선의 판단에 기초한 스크리닝에서 aMCI에서 치매로의 진행을 나타내지 않아야 합니다.
