이 연구의 목적은 매우 아파서 호흡을 돕는 기계가 필요한 환자에게서 발생할 수 있는 삼킴 문제에 대해 자세히 알아보는 것입니다.
환자들은 스스로 호흡하는 데 문제가 있어 인공호흡기라는 기계의 도움이 필요했기 때문에 이 연구에 참여하도록 요청받았습니다.
이 인공호흡기가 공기를 폐로 밀어 넣으려면 환자는 기관내관이라는 튜브를 목구멍에 삽입해야 합니다.
이 기관내관을 삽입하는 과정을 삽관이라고 합니다.
이제 튜브가 제거되었으며 이를 발관이라고 하는 과정입니다.
때때로 기관내관을 가지고 있는 사람들은 일정 기간 동안 음식과 액체를 삼키는 데 어려움을 겪을 수 있습니다.
이 질환을 발관 후 삼킴곤란(PED)이라고 합니다.
PED는 심각한 상태이며 음식이나 액체가 입에서 폐로 들어갈 수 있습니다.
이것은 더 많은 폐 문제를 일으킬 수 있습니다.
이러한 위험을 감안할 때 의사는 때때로 PED 환자에게 먹거나 마시는 것을 피하거나 영양관을 사용하도록 제안합니다.
현재 환자에게 PED가 얼마나 자주 발생하는지, 왜 발생하는지 또는 이를 감지하는 가장 좋은 방법은 아무도 모릅니다.
표준 치료에는 교육받은 추측을 하는 임상의가 포함됩니다.

제외 기준:
- 경장 영양 투여에 대한 금기.
- 삼킴곤란 또는 흡인의 기존 병력.
- 기존 또는 급성 원발성 중추 또는 말초 신경근 장애.
- 만성 기관절개술의 존재(ICU 입원 전 존재).
- 기존의 두경부암 또는 수술.
- 조절되지 않는 비강 또는 인두 출혈을 유발하는 응고병증.
- 혼란 평가 방법(CAM-ICU)에 의해 평가된 발관 후 72시간 이상 섬망.
- 72시간 이상 발관.
- 환자 또는 적절한 대리인으로부터 정보에 입각한 동의를 얻을 수 없음.
- 연령 < 18세.

모든 참가자는 발관 전 72시간 이내에 기관 초음파, 인구 통계 및 병원 임상 정보 수집, 24시간 이내에 삼킴 기능을 다루는 3가지 선별 테스트(연구 정의 알고리즘 테스트, 3온스 물 삼킴 테스트, TOR-BSST) 시행을 받습니다. 발관 후, 광섬유 내시경 연하 검사(FEES) 검사.

3회 검사 프로토콜은 음성 품질 평가 및 2온스 물 일관성 평가인 Yale Swallow Test를 포함한 연구에서 개발한 5개 항목 의사 결정 트리 알고리즘으로 구성된 수정된 병상 삼키기 검사(BSE)입니다. 삼키기 테스트(3-WST) 및 토론토 침대 옆에서 삼키기 선별 테스트(TOR-BSST).
다른 이름들:
후두 구조를 평가하기 위해 고안된 얇고 유연한 내시경을 코를 통해 구인두까지 통과시켜 후두 구조, 혀의 기저부 및 인두를 시각화합니다.
필요한 경우 4% 국소 리도카인 및/또는 옥시메타졸린(Afrin)을 투여합니다.
그런 다음 각각 5ml의 6개의 식품 볼루스로 직접 삼킴을 평가합니다.
모든 환자는 삼키라는 구두 명령 없이 자발적으로 삼킬 수 있습니다.
검사 비디오가 녹화되고 삼킴곤란의 존재가 3명의 다른 관찰자(폐 전문의 1명과 언어 병리학자(SLP) 2명)에 의해 독립적으로 지정됩니다.
이 절차는 5-10분 정도 소요됩니다.

주요 후두 부종은 FEES 검사 중에 시각화된 상부 기도 폐쇄에 이차적으로 기관 재삽관이 필요한 심각한 호흡 곤란으로 정의됩니다.
