Aspiratie bij overlevenden van acuut ademhalingsfalen
Het doel van deze studie is om meer te weten te komen over slikproblemen die zich kunnen voordoen bij patiënten die erg ziek zijn en een machine nodig hebben om te ademen.

Het doel van deze studie is om meer te weten te komen over slikproblemen die zich kunnen voordoen bij patiënten die erg ziek zijn en een machine nodig hebben om te ademen.
Patiënten wordt gevraagd deel te nemen aan dit onderzoek omdat ze problemen hadden met zelfstandig ademen en daarom de hulp nodig hadden van een machine die een beademingsapparaat wordt genoemd.
Om ervoor te zorgen dat dit beademingsapparaat lucht in de longen duwt, hebben patiënten een buis in de keel nodig, een endotracheale buis genaamd.
Het proces van het plaatsen van deze endotracheale tube werd intubatie genoemd.
De buis is nu verwijderd, een proces dat extubatie wordt genoemd.
Soms kunnen mensen die endotracheale tubes hebben gehad gedurende een bepaalde tijd moeite hebben met het doorslikken van voedsel en vloeistoffen.
Deze ziekte wordt post-extubatie dysfagie (PED) genoemd.
PED is een ernstige aandoening en kan ertoe leiden dat voedsel of vloeistof van de mond in de longen terechtkomt.
Dit kan verdere longproblemen veroorzaken.
Gezien dit risico stellen artsen soms voor dat patiënten met PED eten of drinken vermijden of een voedingssonde krijgen.
Momenteel weet niemand hoe vaak patiënten PED ontwikkelen, waarom ze het ontwikkelen of wat de beste methode is om het op te sporen.
Bij standaardzorg maken clinici weloverwogen schattingen.
Deze studie probeert vast te stellen of het kijken naar de patiënt die slikt, zowel met als zonder een kleine camera, een nauwkeurige methode is om PED te detecteren.

Deelname Criteria

Inclusiecriteria:
- Opname op een IC.
- Mechanische beademing met een endotracheale tube gedurende meer dan 48 uur.
Uitsluitingscriteria:
- Contra-indicatie voor toediening van enterale voeding.
- Reeds bestaande geschiedenis van dysfagie of aspiratie.
- Reeds bestaande of acute primaire centrale of perifere neuromusculaire aandoening.
- Aanwezigheid van een chronische tracheostoma (aanwezig voorafgaand aan IC-opname).
- Reeds bestaande hoofd-halskanker of operatie.
- Coagulopathie resulterend in ongecontroleerde nasale of faryngeale bloedingen.
- Delirium gedurende meer dan 72 uur na extubatie zoals beoordeeld met Confusion Assessment Method (CAM-ICU).
- Meer dan 72 uur geëxtubeerd.
- Onvermogen om geïnformeerde toestemming van de patiënt of een geschikte surrogaat te verkrijgen.
- Leeftijd < 18 jaar.

Ander: Aspiratie bij overlevenden van acuut ademhalingsfalen
Alle deelnemers krijgen binnen 72 uur voorafgaand aan extubatie een tracheale echografie, verzameling van demografische en ziekenhuisklinische informatie, toediening van 3 screeningtests (studiegedefinieerde algoritmetest, 3-ounce watersliktest, TOR-BSST) om de slikfunctie binnen 24 uur aan te pakken post-extubatie en een vezeloptisch endoscopisch onderzoek van het slikken (FEES) examen.

Het 3-Screenings Protocol is een aangepast slikonderzoek (BSE) aan het bed dat bestaat uit een in een studie ontwikkeld beslissingsboomalgoritme met vijf items, inclusief beoordeling van de stemkwaliteit en een beoordeling van de consistentie van 2-ounce water, de Yale Swallow Test, met een score van 3-ounce Water Sliktest (3-WST) en de Toronto Bedside Swallowing Screening Test (TOR-BSST).

Een dunne, flexibele endoscoop, ontworpen voor de beoordeling van larynxstructuren, wordt door de neus naar de orofarynx geleid, waarbij de larynxstructuren en de basis van de tong en de keelholte worden gevisualiseerd.
Indien nodig wordt 4% topische lidocaïne en/of oxymetazoline (Afrin) toegediend.
Het slikken wordt dan direct beoordeeld met zes voedselbolussen van elk 5 ml.
Alle patiënten mogen spontaan slikken zonder een mondeling commando om te slikken.
De video van de onderzoeken wordt opgenomen en de aanwezigheid van dysfagie wordt onafhankelijk vastgesteld door 3 verschillende waarnemers (een longarts en twee logopedisten).
Deze procedure duurt 5-10 minuten.
De camera wordt dan verwijderd.

We definiëren larynxoedeem als obstructie van de bovenste luchtwegen binnen 24 uur na extubatie.
Licht larynxoedeem wordt gedefinieerd als stridor geassocieerd met ademnood, gedefinieerd als een verlengde inademingsfase en de aanwezigheid van oedeem bij FEES-onderzoek.
Ernstig larynxoedeem wordt gedefinieerd als ernstige ademnood die reïntubatie van de trachea nodig heeft secundair aan obstructie van de bovenste luchtwegen die tijdens het FEES-onderzoek werd gevisualiseerd
