この研究の目的は、非常に病気で呼吸を助ける機械が必要な患者に発生する可能性のある嚥下の問題についてさらに学ぶことです.
患者は自力呼吸に問題があり、人工呼吸器と呼ばれる機械の助けが必要だったため、この研究に参加するよう求められました。
この人工呼吸器が空気を肺に送り込むためには、気管内チューブと呼ばれるチューブを喉に挿入する必要があります。
この気管内チューブを留置するプロセスは、挿管と呼ばれていました。
これでチューブが取り外されました。これは、抜管と呼ばれるプロセスです。
気管内チューブを挿入した人は、一定期間、食べ物や液体を飲み込むのが困難になることがあります。
この疾患は、抜管後嚥下障害(PED)と呼ばれます。
PED は深刻な状態であり、口から肺に食べ物や液体が入る可能性があります。
これは、さらなる肺の問題を引き起こす可能性があります。
このリスクを考慮して、医師は、PED 患者に飲食を避けるか、栄養チューブを挿入するよう勧めることがあります。
現在、患者がどのくらいの頻度で PED を発症するのか、なぜ発症するのか、またはそれを検出する最善の方法は誰にもわかりません。
標準治療では、臨床医が知識に基づいた推測を行います。
この研究は、小型カメラの有無にかかわらず、患者の嚥下を観察することが PED を検出するための正確な方法であるかどうかを判断することを目的としています。

除外基準:

- 嚥下障害または誤嚥の既往歴。
- -既存または急性の原発性中枢性または末梢神経筋障害。
- -慢性気管切開の存在(ICU入院前に存在)。
- 既存の頭頸部がんまたは手術。
- 制御不能な鼻または咽頭出血をもたらす凝固障害。
- -混乱評価法(CAM-ICU)によって評価された、抜管後72時間以上のせん妄。
- 72 時間以上抜管されている。
- -患者または適切な代理人からインフォームドコンセントを得ることができない。
- 年齢 < 18 歳。

すべての参加者は、抜管前の72時間以内に気管超音波検査を受け、人口統計および病院の臨床情報の収集、24時間以内の嚥下機能に対処する3つのスクリーニング検査(研究定義のアルゴリズム検査、3オンスの水嚥下検査、TOR-BSST)の実施抜管後、および嚥下の光ファイバー内視鏡検査(FEES)検査。

3 スクリーニング プロトコルは、声質評価と 2 オンスの水の一貫性評価を含む研究開発された 5 項目の決定木アルゴリズムで構成されるベッドサイド嚥下試験 (BSE) の修正版であり、3 オンスの水を採点したイェール スワロー テストです。嚥下テスト (3-WST)、およびトロント ベッドサイド嚥下スクリーニング テスト (TOR-BSST)。
他の名前:
喉頭構造の評価用に設計された薄く柔軟な内視鏡を鼻から中咽頭に通し、喉頭構造、舌の付け根、咽頭を視覚化します。
必要に応じて、4% 局所リドカインおよび/またはオキシメタゾリン (アフリン) を投与します。
次に、嚥下は、それぞれ5mlの6回の食物ボーラスで直接評価されます。
すべての患者は、飲み込むように口頭で命令することなく、自発的に飲み込むことができます。
検査のビデオが記録され、嚥下障害の存在は、3 人の異なる観察者 (1 人の呼吸器内科医と 2 人の音声言語病理学者 (SLP)) によって個別に指定されます。

主要な喉頭浮腫は、FEES検査中に視覚化された上気道閉塞に続発する気管の再挿管を必要とする重度の呼吸困難として定義されます
