배경:
맹장암, 결장암 또는 난소암과 같은 다른 암에서 복부 내벽으로 퍼진 종양을 복막 암종증이라고 합니다. 대부분의 경우 결과가 좋지 않습니다. 연구자들은 새로운 치료법을 테스트하고 싶어합니다.
목적:
경구용 닐로티닙과 IV를 통해 복부에 직접 투여하는 파클리탁셀의 병용이 사람들이 수술을 받을 수 있을 만큼 충분히 종양을 줄일 수 있는지 알아봅니다.
적임:
수술을 하기에는 너무 광범위하게 퍼진 복막 암종증이 있는 18세 이상의 성인.

복강경 검사. 전신 마취를 받게 됩니다. 복부에 작은 상처가 생길 것입니다. 조직 및 체액 샘플을 채취합니다.
건강에 관한 설문조사
몸통 CT 스캔
참가자는 최대 4개의 복강경 검사를 더 받게 됩니다. 첫 번째 절차 중에 복부 피부 아래에 포트(IP 포트)를 배치합니다. 복부에 삽입되는 카테터에 부착됩니다.
참가자는 3주 주기로 3~6주기 동안 치료를 받게 됩니다. 그들은 하루에 두 번 입으로 닐로티닙을 복용합니다. 그들은 IP 포트(주기당 한 번) 및 IV(주기당 두 번)에 의해 파클리탁셀을 얻을 것입니다. 3주기와 6주기 후에는 복강경 검사와 CT 스캔을 받게 됩니다. 그런 다음 최대 1년 동안 닐로티닙을 복용하고 파클리탁셀 IV를 투여할 수 있습니다.

설계:
- 2상 오픈 라벨, 비무작위 연구
- 적격성 확인 후 진단 복강경 검사 시 생검을 실시하고 후속 화학 요법 투여를 위해 복강 내 카테터를 배치합니다.
- 주기 3 및 6 이후에 복강경 검사 및 생검 재결정과 함께 최대 6 주기가 계획됩니다.

이 연구에 참여할 자격이 있으려면 개인은 다음 기준을 모두 충족해야 합니다.

- 참가자는 입증된 내성 또는 반응 부족과 함께 적어도 한 줄의 승인된 전신 화학 요법으로 치료를 받았어야 합니다.
- RECIST v1.1에 정의된 측정 가능하거나 평가 가능한 질병. 기준 및/또는 복막 암종증 지수(PCI)
- 참여자는 선별 복강경 검사에서 PCI 점수 > 30이거나 광범위한 소장 장혈관 침범이 있는 세포 축소 수술 대상자가 아닌 것으로 평가되어야 합니다.

- QT 간격을 연장하는 것으로 알려진 약물 또는 CYP3A4, 2C8을 강력하게 억제하는 것으로 알려진 약물의 사용이 필요한 참가자. 동일한 약동학적 상호작용을 갖지 않는 대안이 발견될 수 있는 경우 스크리닝 시 이러한 제제에 대한 참가자가 연구에 허용됩니다.

실험적: 1/ Intraperitoneal (IP) Catheter Placement and Bidirectional Chemotherapy
Intraperitoneal (IP) and intravenous (IV) paclitaxel administration with oral nilotinib.

경구용 닐로티닙은 1일 2회 300mg을 투여합니다.

Intravenous (IV) paclitaxel will be infused over 1 hour on Day 2 of the first week of Cycle 1, followed by Day 1 of the subsequent treatment weeks; IV paclitaxel will be dosed at 60 mg/m2 for Week 1 of Cycle 1 and, if tolerated, at 80 mg/m2 for subsequent treatments.

We calculated the rate of downstaging of peritoneal disease burden to become resectable based on PCI score of initial and subsequent laparoscopy, or magnetic resonance imaging and/or computed tomography imaging if laparoscopy is not planned.
Disease burden will be considered stable when PCI score at laparoscopy (lap) #3 is < 4 points higher or lower compared to PCI score at lap #2.
Complete Response is PCI≤ 5 with negative histology of at least 3 peritoneal biopsies of suspect nodules and washings with negative cytology.
Partial Response (PR) is at least 4 points decrease in PCI.
Progressive Disease (PD) has at least 4 points increase in PCI.
Stable Disease is neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for PD, taking as reference the PCI.
To document PCI scoring for each participant, the rubric will be completed following each laparoscopy.

Outcomes from QOL comparing results before to after treatment: Quality of life is assessed using the Functional Assessment of Cancer Therapy - Colorectal (FACT-C) questionnaire, a validated instrument that measures health-related quality of life.
The FACT-C consists of the following subscales: Physical Well-Being (PWB): 7 items; score range 0-28, Social/Family Well-Being (SWB): 7 items; score range 0-28, Emotional Well-Being (EWB): 6 items; score range 0-24, Functional Well-Being (FWB): 7 items; score range 0-28, Colorectal Cancer Subscale (CCS):9 items; score range 0-36.The FACT-C total score is derived by summing all five subscale scores.
Total score range: 0-144.
For all subscales and the total score, higher values represent better quality of life and lower values represent worse quality of life.
