Dożylny i dootrzewnowy paklitaksel i doustny nilotynib w raku otrzewnej z powodu raka jajnika, jelita grubego lub wyrostka robaczkowego
Badanie fazy II dożylnego i dootrzewnowego paklitakselu i doustnego nilotynibu w leczeniu raka otrzewnej z raka jelita grubego, wyrostka robaczkowego, jelita cienkiego, żołądka, jajnika lub innego pierwotnego raka ginekologicznego
Tło:
Guzy, które rozprzestrzeniły się na wyściółkę jamy brzusznej z innych nowotworów, takich jak rak wyrostka robaczkowego, okrężnicy lub jajnika, nazywane są rakiem otrzewnej. W większości przypadków wyniki są słabe. Naukowcy chcą przetestować nowy lek.
Cel:
Aby dowiedzieć się, czy połączenie doustnego nilotynibu i paklitakselu podawanego dożylnie i bezpośrednio do jamy brzusznej może zmniejszyć guzy na tyle, aby ludzie mogli poddać się operacji.

Laparoskopia. Dostaną znieczulenie ogólne. Małe nacięcia zostaną wykonane w ich brzuchu. Zostaną pobrane próbki tkanek i płynów.
Ankiety na temat ich stanu zdrowia
Tomografia komputerowa ich tułowia
Uczestnicy będą mieli jeszcze do 4 laparoskopii. Podczas pierwszego zabiegu port zostanie umieszczony pod skórą brzucha (port IP). Zostanie on przymocowany do cewnika umieszczonego w jamie brzusznej.
Uczestnicy otrzymają kurację w cyklach 3-tygodniowych, po 3 lub 6 cykli. Będą przyjmować nilotynib doustnie dwa razy dziennie. Otrzymają paklitaksel przez port IP (raz na cykl) i IV (dwa razy na cykl). Po cyklach 3 i 6 przejdą laparoskopię i tomografię komputerową. Następnie mogą przyjmować nilotynib i paklitaksel dożylny przez okres do 1 roku.
Podczas wizyt studyjnych uczestnicy będą powtarzać niektóre badania przesiewowe.
Około 6 tygodni po zakończeniu leczenia, a następnie co 3 miesiące przez 3 lata, uczestnicy będą mieli wizyty kontrolne w NIH lub u miejscowego lekarza.

- Badanie to obejmuje połączenie dożylnego i dootrzewnowego paklitakselu oraz doustnego nilotynibu w przypadku nieoperacyjnego raka otrzewnej z jelita grubego, wyrostka robaczkowego, jelita cienkiego, żołądka, jajników lub innych pierwotnych histologii ginekologicznych
Cel:
-Ocena skuteczności dwukierunkowej chemioterapii z użyciem paklitakselu podawanego dootrzewnowo i dożylnie oraz doustnego nilotynibu poprzez obliczenie tempa zmniejszania się stopnia zaawansowania choroby otrzewnej do resekcji na podstawie wskaźnika raka otrzewnej (PCI)
Uprawnienia:
- Uczestnicy >= 18 lat z potwierdzonym histologicznie nieśluzowym rakiem otrzewnej jelita grubego, wyrostka robaczkowego, jelita cienkiego, żołądka, jajników lub innym pierwotnym badaniem histologicznym ginekologicznym
- Wykazano oporność lub brak odpowiedzi na co najmniej jedną linię już zatwierdzonej i dostępnej chemioterapii ogólnoustrojowej
- Brak historii reakcji alergicznych przypisywanych związkom o podobnym składzie chemicznym lub biologicznym do badanych leków
- Brak chemioterapii dootrzewnowej w ciągu ostatnich sześciu miesięcy
- Uznany za niezdolnego do całkowitej cytoredukcji

- Otwarte, nierandomizowane badanie fazy II
- Po potwierdzeniu kwalifikacji, w czasie laparoskopii diagnostycznej, zostaną pobrane biopsje i założony cewnik dootrzewnowy do późniejszego podania chemioterapii
- Zostanie zaplanowanych do 6 cykli, z powtórną laparoskopią i biopsjami po cyklach 3 i 6

- KRYTERIA PRZYJĘCIA:
Aby kwalifikować się do udziału w tym badaniu, osoba musi spełniać wszystkie poniższe kryteria.

- Mierzalna lub możliwa do oceny choroba zgodnie z definicją RECIST v1.1. kryteriów i/lub według wskaźnika raka otrzewnej (PCI)
- Należy ocenić, czy uczestnicy nie są kandydatami do zabiegu cytoredukcyjnego, z wynikiem PCI > 30 w przesiewowej laparoskopii lub z rozległym zajęciem błony surowiczej jelita cienkiego
- Wiek >= 18 lat
- Stan sprawności wg ECOG <= 2 (Karnofsky >= 60%).
Uczestnicy muszą mieć odpowiednią funkcję narządów i szpiku, jak zdefiniowano poniżej:
- Leukocyty >= 3000/ml
- Bezwzględna liczba neutrofili >= 1000/ml
- Płytki >= 100 000/ml
- Stężenie bilirubiny całkowitej w granicach <= 1,5x górnej granicy normy (GGN) obowiązującej w danej placówce
- AspAT (SGOT)/ ALT (SGPT) <= 2,5x ULN w placówce
- Amylaza i lipaza w surowicy <= 1,5x ULN w placówce
- Stężenie potasu i magnezu w surowicy powyżej dolnej granicy normy obowiązującej w placówce
- Kreatynina <= 1,5 mg/dl lub klirens kreatyniny >= 60 ml/min/1,73 m2 dla uczestników z poziomami kreatyniny powyżej normy instytucjonalnej obliczonymi przy użyciu eGFR
- Karmiąca piersią uczestniczka musi wyrazić zgodę na przerwanie karmienia piersią.
- Kobiety w wieku rozrodczym muszą wyrazić zgodę na stosowanie odpowiedniej antykoncepcji (hormonalnej lub barierowej metody antykoncepcji; abstynencja) przed włączeniem do badania, w czasie trwania udziału w badaniu i przez 90 dni po ostatnim leczeniu w ramach badania. Jeśli kobieta podejrzewa, że jest w ciąży podczas udziału w tym badaniu, powinna natychmiast poinformować o tym swojego lekarza prowadzącego.

- Uczestnicy muszą wyrazić zgodę na wspólną rejestrację w protokole pobierania tkanek 13C0176, Nowotwór, tkanka prawidłowa i próbki od pacjentów poddawanych ocenie lub chirurgicznej resekcji guzów litych
KRYTERIA WYŁĄCZENIA:
Osoba spełniająca którekolwiek z poniższych kryteriów zostanie wykluczona z udziału w tym badaniu.
- Uczestnicy, którzy otrzymują jakiekolwiek inne badane środki lub otrzymywali badany środek w ciągu 30 dni przed rozpoczęciem leczenia w ramach badania.
- Historia reakcji alergicznych przypisywanych związkom o podobnym składzie chemicznym lub biologicznym do badanych leków.
- Uczestnicy, którzy otrzymali ogólnoustrojową (tj. doustną lub dożylną) chemioterapię w ciągu 5 okresów półtrwania podanego(ych) pojedynczego(ych) środka(ów) lub którzy przeszli poważną operację w ciągu ostatnich 12 tygodni przed rozpoczęciem leczenia w ramach badania.
- Przebyta chemioterapia dootrzewnowa w ciągu ostatnich 6 miesięcy.
- Uczestnicy wymagający zastosowania leków wydłużających odstęp QT lub silnie hamujących CYP3A4, 2C8. Uczestnicy przyjmujący takie środki w czasie badania przesiewowego mogą wziąć udział w badaniu, jeśli można znaleźć alternatywę, która nie ma takich samych interakcji farmakokinetycznych.
- Niekontrolowana współistniejąca choroba, w tym między innymi trwająca lub czynna infekcja, objawowa zastoinowa niewydolność serca, niestabilna dusznica bolesna, zaburzenia rytmu serca lub choroba psychiczna/sytuacje społeczne, które ograniczają zgodność z wymogami badania. Uwaga: Żaden przedmiot nie zostanie wykluczony na podstawie sytuacji społecznej przed konsultacjami z Departamentem Pracy Socjalnej.
- Kobiety w ciąży są wykluczone z tego badania.
- Pacjenci z HIV, u których występuje wykrywalne miano wirusa lub których ART zawiera leki wydłużające odstęp QTc lub inhibitory CYP3A4, niezależnie od miana wirusa. (UWAGA: Pacjenci z HIV, którzy mają niewykrywalne miano wirusa i otrzymywali stabilne dawki ART, które nie wydłużają odstępu QT lub są silnymi inhibitorami CYP3A4, 2C8, kwalifikują się).
- Odstęp QTcF >= 450 ms na początku badania lub wrodzony zespół wydłużonego QT.
- Ponad 3 litry wodobrzusza obecne podczas początkowej laparoskopii lub historia więcej niż dwóch procedur paracentezy w ciągu 30 dni przed pierwszą laparoskopią.
- Zaawansowana niewydolność wątroby, na co wskazuje marskość wątroby klasy C wg klasyfikacji Childa-Pugha.

Eksperymentalny: 1/ Intraperitoneal (IP) Catheter Placement and Bidirectional Chemotherapy
Intraperitoneal (IP) and intravenous (IV) paclitaxel administration with oral nilotinib.

Doustny nilotynib będzie podawany w dawce 300 mg dwa razy dziennie.
Nilotynib będzie podawany w sposób ciągły od dawki nasycającej (Dzień -4) aż do laparoskopii nr 2 i dalej.

Paclitaxel: Intraperitoneal (IP) paclitaxel will be dosed at 60 mg/m^2 to be infused over 1 hour on Day 1 of each 3-week cycle; participants with unresectable, but stable or responding disease after C1 through C3 will dose increase IP paclitaxel to 80 mg/m2 for Cycles 4-6.
Intravenous (IV) paclitaxel will be infused over 1 hour on Day 2 of the first week of Cycle 1, followed by Day 1 of the subsequent treatment weeks; IV paclitaxel will be dosed at 60 mg/m2 for Week 1 of Cycle 1 and, if tolerated, at 80 mg/m2 for subsequent treatments.

We calculated the rate of downstaging of peritoneal disease burden to become resectable based on PCI score of initial and subsequent laparoscopy, or magnetic resonance imaging and/or computed tomography imaging if laparoscopy is not planned.
Disease burden will be considered stable when PCI score at laparoscopy (lap) #3 is < 4 points higher or lower compared to PCI score at lap #2.
Complete Response is PCI≤ 5 with negative histology of at least 3 peritoneal biopsies of suspect nodules and washings with negative cytology.
Partial Response (PR) is at least 4 points decrease in PCI.
Progressive Disease (PD) has at least 4 points increase in PCI.
Stable Disease is neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for PD, taking as reference the PCI.
To document PCI scoring for each participant, the rubric will be completed following each laparoscopy.

This outcome measure evaluates how many participants with initially inoperable (unresectable) peritoneal carcinomatosis became eligible for surgery after receiving bidirectional chemotherapy (intravenous and intraperitoneal paclitaxel and oral nilotinib).

Response was assessed by the Peritoneal Carcinomatosis Index (PCI) and the Response Evaluation Criteria in Solid Tumors (RECIST).
Disease relapse is CR is PCI≤ 5 with negative histology of at least 3 peritoneal biopsies of suspect nodules and washings with negative cytology.

Outcomes from QOL comparing results before to after treatment: Quality of life is assessed using the Functional Assessment of Cancer Therapy - Colorectal (FACT-C) questionnaire, a validated instrument that measures health-related quality of life.
The FACT-C consists of the following subscales: Physical Well-Being (PWB): 7 items; score range 0-28, Social/Family Well-Being (SWB): 7 items; score range 0-28, Emotional Well-Being (EWB): 6 items; score range 0-24, Functional Well-Being (FWB): 7 items; score range 0-28, Colorectal Cancer Subscale (CCS):9 items; score range 0-36.The FACT-C total score is derived by summing all five subscale scores.
Total score range: 0-144.
For all subscales and the total score, higher values represent better quality of life and lower values represent worse quality of life.
Questionnaires are administered at baseline, immediately prior to treatment Cycles 3 and 6, and at follow-up visits.

Here is the number of participants with serious and/or non-serious adverse events assessed by the Common Terminology Criteria for Adverse Events (CTCAE v5.0).
A non-serious adverse event is any untoward medical occurrence.
A serious adverse event is an adverse event or suspected adverse reaction that results in death, a life-threatening adverse drug experience, hospitalization, disruption of the ability to conduct normal life functions, congenital anomaly/birth defect or important medical events that jeopardize the patient or subject and may require medical or surgical intervention to prevent one of the previous outcomes mentioned.
