Paclitaxel Intravenoso e Intraperitoneal e Nilotinibe Oral para Carcinomatose Peritoneal de Câncer de Ovário, Colorretal ou de Apêndice
Estudo de Fase II de Paclitaxel Intravenoso e Intraperitoneal e Nilotinib Oral para Carcinomatose Peritoneal de Colorretal, Apendiceal, Intestino Delgado, Gástrico, Ovariano ou Outro Câncer Primário Ginecológico
Fundo:
Os tumores que se espalharam para o revestimento do abdome a partir de outros tipos de câncer, como câncer de apêndice, cólon ou ovário, são chamados de carcinomatose peritoneal. Na maioria dos casos, os resultados são ruins. Os pesquisadores querem testar um novo tratamento.
Objetivo:
Para saber se a combinação de nilotinibe oral mais paclitaxel administrado por via intravenosa e diretamente no abdômen pode reduzir tumores o suficiente para que as pessoas façam cirurgia.
Elegibilidade:
Adultos com 18 anos ou mais com carcinomatose peritoneal muito disseminada para cirurgia.

Eletrocardiograma
Laparoscopia. Eles receberão anestesia geral. Pequenos cortes serão feitos em seu abdômen. Amostras de tecidos e fluidos serão coletadas.

Tomografias computadorizadas de seu torso
Os participantes terão até mais 4 laparoscopias. Durante o primeiro procedimento, uma porta será colocada sob a pele do abdômen (uma porta IP). Ele será anexado a um cateter que é colocado em seu abdômen.
Os participantes receberão tratamento em ciclos de 3 semanas, por 3 ou 6 ciclos. Eles tomarão nilotinibe por via oral duas vezes ao dia. Eles receberão paclitaxel pela porta IP (uma vez por ciclo) e por IV (duas vezes por ciclo). Após os ciclos 3 e 6, eles farão uma laparoscopia e tomografias computadorizadas. Então eles podem tomar nilotinibe e receber paclitaxel IV por até 1 ano.
Nas visitas do estudo, os participantes repetirão alguns testes de triagem.
Cerca de 6 semanas após o término do tratamento e depois a cada 3 meses durante 3 anos, os participantes farão consultas de acompanhamento no NIH ou com o médico local.

Fundo:
- A carcinomatose peritoneal é uniformemente fatal se não for tratada; apesar dos avanços na quimioterapia sistêmica, cirurgia citorredutora e quimioterapia intraperitoneal, a sobrevida permanece baixa para a maioria dos pacientes
- A combinação de nilotinibe oral e paclitaxel intravenoso demonstrou sinergismo pré-clínico e clínico no tratamento de tumores sólidos, com um estudo de Fase I em andamento no NIH
- A sinergia de nilotinibe oral com paclitaxel intraperitoneal ainda precisa ser caracterizada

Objetivo:
-Avaliar a eficácia da quimioterapia bidirecional usando paclitaxel intraperitoneal e intravenoso e nilotinibe oral, calculando a taxa de diminuição da carga de doença peritoneal para tornar-se ressecável, com base no Índice de Carcinomatose Peritoneal (PCI)
Elegibilidade:
- Participantes >= 18 anos de idade com carcinomatose peritoneal não mucinosa confirmada histologicamente de histologia primária colorretal, apendicular, intestinal, gástrica, ovariana ou outra histologia ginecológica primária
- Resistência demonstrada ou falta de resposta a pelo menos uma linha de quimioterapia sistêmica já aprovada e disponível
- Sem história de reações alérgicas atribuídas a compostos de composição química ou biológica semelhante aos medicamentos do estudo
- Sem quimioterapia intraperitoneal nos últimos seis meses
- Considerado incapaz de passar por citorredução completa
Projeto:
- Estudo aberto de fase II, não randomizado
- Após a confirmação da elegibilidade, no momento da laparoscopia diagnóstica, serão feitas biópsias e um cateter intraperitoneal será colocado para posterior administração de quimioterapia
- Serão planejados até 6 ciclos, com reestadiamento de laparoscopia e biópsias após os ciclos 3 e 6

- CRITÉRIO DE INCLUSÃO:
Para ser elegível para participar deste estudo, um indivíduo deve atender a todos os critérios a seguir.
- Confirmação histológica de carcinomatose peritoneal não mucinosa de colo-retal, apêndice, intestino delgado, gástrica, ovariana ou outra ginecológica (isto é, endometrial, trompa de Falópio, peritoneal primária, cervical) primária pelo Laboratório de Patologia, NCI.
- Os participantes devem ter sido tratados com pelo menos uma linha de quimioterapia sistêmica aprovada, com resistência demonstrada ou falta de resposta
- Doença mensurável ou avaliável conforme definido pelo RECIST v1.1. critérios e/ou pelo Índice de Carcinomatose Peritoneal (ICP)
- Os participantes devem ser avaliados para não serem candidatos à cirurgia citorredutora, com pontuação PCI > 30 na laparoscopia de triagem ou com extenso envolvimento seroso do intestino delgado
- Idade >= 18 anos
- Estado de desempenho ECOG <= 2 (Karnofsky >= 60%).
Os participantes devem ter funções adequadas de órgãos e medulas, conforme definido abaixo:
- Leucócitos >= 3.000/mcL
- Contagem absoluta de neutrófilos >= 1.000/mcL
- Plaquetas >= 100.000/mcL
- Bilirrubina total dentro de <= 1,5x limite superior institucional do normal (LSN)
- AST (SGOT)/ALT (SGPT) <= 2,5x LSN institucional
- Amilase e lipase sérica <= 1,5x LSN institucional
- Potássio e magnésio séricos acima do limite inferior institucional do normal
- Creatinina <= 1,5 mg/dL ou depuração de creatinina >= 60 mL/min/1,73 m2 para participantes com níveis de creatinina acima do normal calculado usando eGFR
- A participante que amamenta deve concordar em interromper a amamentação.
- As mulheres com potencial para engravidar devem concordar em usar contracepção adequada (método hormonal ou de barreira de controle de natalidade; abstinência) antes da entrada no estudo, durante a participação no estudo e por 90 dias após o último tratamento do estudo. Se uma mulher suspeitar que está grávida enquanto estiver participando deste estudo, ela deve informar seu médico assistente imediatamente.

CRITÉRIO DE EXCLUSÃO:
Um indivíduo que atender a qualquer um dos seguintes critérios será excluído da participação neste estudo.
- Participantes que estão recebendo quaisquer outros agentes experimentais ou receberam um agente experimental dentro de 30 dias antes do início do tratamento do estudo.
- Histórico de reações alérgicas atribuídas a compostos de composição química ou biológica semelhante aos medicamentos em estudo.
- Participantes que receberam quimioterapia sistêmica (ou seja, oral ou intravenosa) dentro de 5 meias-vidas do(s) agente(s) individual(ais) administrado(s) ou que foram submetidos a cirurgia de grande porte nas últimas 12 semanas antes do início do tratamento do estudo.
- Quimioterapia intraperitoneal anterior nos últimos 6 meses.
- Participantes que requerem o uso de medicamentos conhecidos por prolongar o intervalo QT ou por inibir fortemente o CYP3A4, 2C8. Os participantes em tais agentes no momento da triagem são permitidos no estudo se uma alternativa que não tenha as mesmas interações farmacocinéticas puder ser encontrada.
- Doença intercorrente não controlada, incluindo, mas não se limitando a, infecção contínua ou ativa, insuficiência cardíaca congestiva sintomática, angina pectoris instável, arritmia cardíaca ou doença psiquiátrica/situações sociais que limitariam a conformidade com os requisitos do estudo. Nota: Nenhum sujeito será excluído com base em uma situação social antes da consulta ao Departamento de Serviço Social.
- As mulheres grávidas são excluídas deste estudo.
- Pacientes com HIV que têm carga viral detectável, ou cujo TARV contém Medicamentos Prolongadores do QTc ou Inibidores do CYP3A4, independentemente da carga viral. (OBSERVAÇÃO: pacientes com HIV com carga viral indetectável e em uso de doses estáveis de ART que não prolonguem o intervalo QT ou que sejam fortes inibidores de CYP3A4 e 2C8 são elegíveis).
- Intervalo QTcF de >= 450 ms no início do estudo ou síndrome do QT longo congênito.
- Mais de 3 litros de ascite presente na laparoscopia inicial ou história de mais de dois procedimentos de paracentese nos 30 dias anteriores à laparoscopia inicial.
- Insuficiência hepática avançada, indicada por cirrose Child-Pugh Classe C.
- Neuropatia sensorial/motora >= Grau 2

Experimental: 1/ Intraperitoneal (IP) Catheter Placement and Bidirectional Chemotherapy
Intraperitoneal (IP) and intravenous (IV) paclitaxel administration with oral nilotinib.

O nilotinibe oral será administrado em 300 mg duas vezes ao dia.
O nilotinibe será administrado continuamente a partir da dose de ataque (Dia -4) até a laparoscopia #2 em diante.

Paclitaxel: Intraperitoneal (IP) paclitaxel will be dosed at 60 mg/m^2 to be infused over 1 hour on Day 1 of each 3-week cycle; participants with unresectable, but stable or responding disease after C1 through C3 will dose increase IP paclitaxel to 80 mg/m2 for Cycles 4-6.
Intravenous (IV) paclitaxel will be infused over 1 hour on Day 2 of the first week of Cycle 1, followed by Day 1 of the subsequent treatment weeks; IV paclitaxel will be dosed at 60 mg/m2 for Week 1 of Cycle 1 and, if tolerated, at 80 mg/m2 for subsequent treatments.

Proportion of Participants Who Are Successfully Down Staged to Resectable Based on Peritoneal Carcinomatosis Index (PCI) and Principal Investigator (PI) Discretion Reported Along With a 95% Confidence Interval

We calculated the rate of downstaging of peritoneal disease burden to become resectable based on PCI score of initial and subsequent laparoscopy, or magnetic resonance imaging and/or computed tomography imaging if laparoscopy is not planned.
Disease burden will be considered stable when PCI score at laparoscopy (lap) #3 is < 4 points higher or lower compared to PCI score at lap #2.
Complete Response is PCI≤ 5 with negative histology of at least 3 peritoneal biopsies of suspect nodules and washings with negative cytology.
Partial Response (PR) is at least 4 points decrease in PCI.
Progressive Disease (PD) has at least 4 points increase in PCI.
Stable Disease is neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for PD, taking as reference the PCI.
To document PCI scoring for each participant, the rubric will be completed following each laparoscopy.

Outcomes from QOL comparing results before to after treatment: Quality of life is assessed using the Functional Assessment of Cancer Therapy - Colorectal (FACT-C) questionnaire, a validated instrument that measures health-related quality of life.
The FACT-C consists of the following subscales: Physical Well-Being (PWB): 7 items; score range 0-28, Social/Family Well-Being (SWB): 7 items; score range 0-28, Emotional Well-Being (EWB): 6 items; score range 0-24, Functional Well-Being (FWB): 7 items; score range 0-28, Colorectal Cancer Subscale (CCS):9 items; score range 0-36.The FACT-C total score is derived by summing all five subscale scores.
Total score range: 0-144.
For all subscales and the total score, higher values represent better quality of life and lower values represent worse quality of life.
