Intravenöses und intraperitoneales Paclitaxel und orales Nilotinib bei Peritonealkarzinose durch Eierstock-, Darm- oder Blinddarmkrebs
Phase-II-Studie mit intravenösem und intraperitonealem Paclitaxel und oralem Nilotinib bei Peritonealkarzinose aufgrund von kolorektalem, Blinddarm-, Dünndarm-, Magen-, Eierstock- oder anderem gynäkologischen Primärkrebs
Hintergrund:
Tumore, die sich von anderen Krebsarten wie Blinddarm-, Dickdarm- oder Eierstockkrebs auf die Bauchschleimhaut ausgebreitet haben, werden als Peritonealkarzinose bezeichnet. In den meisten Fällen sind die Ergebnisse schlecht. Forscher wollen eine neue Behandlung testen.
Zielsetzung:
Um zu erfahren, ob die Kombination von oralem Nilotinib plus Paclitaxel, das intravenös und direkt in den Bauch verabreicht wird, Tumore so weit reduzieren kann, dass Menschen operiert werden können.
Teilnahmeberechtigung:
Erwachsene ab 18 Jahren mit Peritonealkarzinose, die für eine Operation zu weit verbreitet ist.

Elektrokardiogramm
Laparoskopie. Sie erhalten eine Vollnarkose. Es werden kleine Schnitte in ihrem Bauch gemacht. Gewebe- und Flüssigkeitsproben werden entnommen.

Die Teilnehmer haben bis zu 4 weitere Laparoskopien. Während des ersten Eingriffs wird ein Port unter der Bauchhaut platziert (ein IP-Port). Es wird an einem Katheter befestigt, der in ihrem Bauch platziert wird.
Die Teilnehmer erhalten eine Behandlung in 3-Wochen-Zyklen für 3 oder 6 Zyklen. Sie nehmen Nilotinib zweimal täglich oral ein. Sie erhalten Paclitaxel per IP-Port (einmal pro Zyklus) und per IV (zweimal pro Zyklus). Nach den Zyklen 3 und 6 werden sie einer Laparoskopie und CT-Scans unterzogen. Dann können sie Nilotinib einnehmen und erhalten intravenös Paclitaxel für bis zu 1 Jahr.
Bei Studienbesuchen wiederholen die Teilnehmer einige Screening-Tests.
Etwa 6 Wochen nach Behandlungsende und dann alle 3 Monate für 3 Jahre werden die Teilnehmer zu Nachsorgebesuchen beim NIH oder bei ihrem örtlichen Arzt.

Hintergrund:
- Unbehandelt ist die Peritonealkarzinose einheitlich tödlich; Trotz Fortschritten in der systemischen Chemotherapie, der zytoreduktiven Chirurgie und der intraperitonealen Chemotherapie bleibt das Überleben für die Mehrheit der Patienten schlecht
- Die Kombination von oralem Nilotinib und intravenösem Paclitaxel hat in einer laufenden Phase-I-Studie am NIH vorklinische und klinische Synergien bei der Behandlung von soliden Tumoren gezeigt
- Die Synergie von oralem Nilotinib mit intraperitonealem Paclitaxel muss noch charakterisiert werden
- Diese Studie umfasst die Kombination von intravenösem und intraperitonealem Paclitaxel und oralem Nilotinib bei nicht resezierbarer Peritonealkarzinose aus kolorektalen, appendicealen, Dünndarm-, Magen-, Eierstock- oder anderen gynäkologischen Primärhistologien
Zielsetzung:
-Bewertung der Wirksamkeit einer bidirektionalen Chemotherapie mit intraperitonealem und intravenösem Paclitaxel und oralem Nilotinib durch Berechnung der Downstaging-Rate der peritonealen Krankheitslast bis zur Resektabilität, basierend auf dem Peritoneal Carcinomatosis Index (PCI)
Teilnahmeberechtigung:
- Teilnehmer >= 18 Jahre mit histologisch bestätigter nicht-muzinöser Peritonealkarzinose der kolorektalen, Blinddarm-, Dünndarm-, Magen-, Eierstock- oder anderen gynäkologischen Primärhistologie
- Nachgewiesene Resistenz oder fehlendes Ansprechen auf mindestens eine bereits zugelassene und verfügbare systemische Chemotherapie
- Keine allergischen Reaktionen in der Vorgeschichte, die auf Verbindungen mit ähnlicher chemischer oder biologischer Zusammensetzung wie bei Studienmedikamenten zurückzuführen sind
- Keine intraperitoneale Chemotherapie innerhalb der letzten sechs Monate
- Wird als nicht in der Lage angesehen, sich einer vollständigen Zytoreduktion zu unterziehen
Design:
- Offene, nicht randomisierte Phase-II-Studie
- Nach Bestätigung der Eignung werden zum Zeitpunkt der diagnostischen Laparoskopie Biopsien entnommen und ein intraperitonealer Katheter für die anschließende Verabreichung der Chemotherapie platziert
- Es werden bis zu 6 Zyklen geplant, mit Restaging-Laparoskopie und Biopsien nach den Zyklen 3 und 6

Beschreibung
- EINSCHLUSSKRITERIEN:
Um an dieser Studie teilnehmen zu können, muss eine Person alle der folgenden Kriterien erfüllen.
- Histologische Bestätigung einer nicht muzinösen Peritonealkarzinose durch kolorektale, appendizeale, Dünndarm-, Magen-, Eierstock- oder andere gynäkologische (d. h. Endometrium, Eileiter, primäre Peritoneal-, Zervix-) Primäre durch das Labor für Pathologie, NCI.
- Die Teilnehmer müssen mit mindestens einer Linie einer zugelassenen systemischen Chemotherapie mit nachgewiesener Resistenz oder fehlendem Ansprechen behandelt worden sein
- Messbare oder auswertbare Krankheit gemäß RECIST v1.1. Kriterien und/oder nach Peritoneal Carcinomatosis Index (PCI)
- Die Teilnehmer müssen so beurteilt werden, dass sie keine Kandidaten für eine zytoreduktive Operation sind, mit einem PCI-Score > 30 bei der Screening-Laparoskopie oder mit einer ausgedehnten Beteiligung der Dünndarmserosale
- Alter >= 18 Jahre
- ECOG-Leistungsstatus <= 2 (Karnofsky >= 60 %).
Die Teilnehmer müssen über eine angemessene Organ- und Markfunktion verfügen, wie unten definiert:
- Leukozyten >= 3.000/μl
- Absolute Neutrophilenzahl >= 1.000/μl
- Blutplättchen >= 100.000/μl
- Gesamtbilirubin innerhalb <= 1,5x der institutionellen Obergrenze des Normalwerts (ULN)
- AST (SGOT)/ ALT (SGPT) <= 2,5x institutionelle ULN
- Serumamylase und -lipase <= 1,5x institutionelle ULN
- Kalium und Magnesium im Serum größer als die institutionelle Untergrenze des Normalwerts
- Kreatinin <= 1,5 mg/dl oder Kreatinin-Clearance >= 60 ml/min/1,73 m2 für Teilnehmer mit Kreatininspiegeln über dem institutionellen Normalwert, berechnet mit eGFR
- Die stillende Teilnehmerin muss zustimmen, das Stillen einzustellen.
- Frauen im gebärfähigen Alter müssen vor Beginn der Studie, für die Dauer der Studienteilnahme und für 90 Tage nach der letzten Studienbehandlung einer angemessenen Empfängnisverhütung (hormonelle oder Barrieremethode zur Empfängnisverhütung; Abstinenz) zustimmen. Sollte eine Frau während der Teilnahme an dieser Studie vermuten, schwanger zu sein, sollte sie unverzüglich ihren behandelnden Arzt informieren.
- Fähigkeit des Teilnehmers zu verstehen und die Bereitschaft, ein schriftliches Einverständniserklärungsdokument zu unterzeichnen.
- Die Teilnehmer müssen der gemeinsamen Registrierung für das Gewebeentnahmeprotokoll 13C0176, Tumor, normales Gewebe und Proben von Patienten, die sich einer Bewertung oder chirurgischen Resektion von soliden Tumoren unterziehen, zustimmen
AUSSCHLUSSKRITERIEN:
Eine Person, die eines der folgenden Kriterien erfüllt, wird von der Teilnahme an dieser Studie ausgeschlossen.
- Teilnehmer, die andere Prüfpräparate erhalten oder innerhalb von 30 Tagen vor Beginn der Studienbehandlung ein Prüfpräparat erhalten haben.
- Vorgeschichte von allergischen Reaktionen, die auf Verbindungen mit ähnlicher chemischer oder biologischer Zusammensetzung zurückzuführen sind, um Arzneimittel zu untersuchen.
- Teilnehmer, die eine systemische (d. h. orale oder intravenöse) Chemotherapie innerhalb von 5 Halbwertszeiten der einzelnen verabreichten Wirkstoffe erhalten haben oder die sich innerhalb der letzten 12 Wochen vor Beginn der Studienbehandlung einer größeren Operation unterzogen haben.
- Vorherige intraperitoneale Chemotherapie innerhalb der letzten 6 Monate.
- Teilnehmer, die Medikamente einnehmen müssen, von denen bekannt ist, dass sie das QT-Intervall verlängern oder CYP3A4, 2C8 stark hemmen. Teilnehmer, die zum Zeitpunkt des Screenings solche Wirkstoffe einnehmen, dürfen an der Studie teilnehmen, wenn eine Alternative gefunden werden kann, die nicht die gleichen pharmakokinetischen Wechselwirkungen aufweist.
- Unkontrollierte interkurrente Erkrankung, einschließlich, aber nicht beschränkt auf, anhaltende oder aktive Infektion, symptomatische dekompensierte Herzinsuffizienz, instabile Angina pectoris, Herzrhythmusstörungen oder psychiatrische Erkrankungen/soziale Situationen, die die Einhaltung der Studienanforderungen einschränken würden. Hinweis: Kein Fach wird aufgrund einer sozialen Situation vor Rücksprache mit dem Fachbereich Soziale Arbeit ausgeschlossen.
- Schwangere Frauen sind von dieser Studie ausgeschlossen.
- Patienten mit HIV, die eine nachweisbare Viruslast haben oder deren ART unabhängig von der Viruslast QTc-verlängernde Medikamente oder CYP3A4-Inhibitoren enthält. (HINWEIS: Patienten mit HIV, die eine nicht nachweisbare Viruslast haben und stabile ART-Dosen erhalten haben, die das QT-Intervall nicht verlängern oder die ein starker CYP3A4-, 2C8-Hemmer sind, sind geeignet).
- QTcF-Intervall von >= 450 ms bei Studieneintritt oder angeborenes Long-QT-Syndrom.
- Mehr als 3 Liter Aszites bei der anfänglichen Laparoskopie oder Vorgeschichte von mehr als zwei Parazenteseverfahren in den 30 Tagen vor der ersten Laparoskopie.
- Fortgeschrittenes Leberversagen, angezeigt durch Zirrhose der Child-Pugh-Klasse C.
- Sensorische/motorische Neuropathie >= Grad 2

Experimental: 1/ Intraperitoneal (IP) Catheter Placement and Bidirectional Chemotherapy
Intraperitoneal (IP) and intravenous (IV) paclitaxel administration with oral nilotinib.

We calculated the rate of downstaging of peritoneal disease burden to become resectable based on PCI score of initial and subsequent laparoscopy, or magnetic resonance imaging and/or computed tomography imaging if laparoscopy is not planned.
Disease burden will be considered stable when PCI score at laparoscopy (lap) #3 is < 4 points higher or lower compared to PCI score at lap #2.
Complete Response is PCI≤ 5 with negative histology of at least 3 peritoneal biopsies of suspect nodules and washings with negative cytology.
Partial Response (PR) is at least 4 points decrease in PCI.

Outcomes from QOL comparing results before to after treatment: Quality of life is assessed using the Functional Assessment of Cancer Therapy - Colorectal (FACT-C) questionnaire, a validated instrument that measures health-related quality of life.
The FACT-C consists of the following subscales: Physical Well-Being (PWB): 7 items; score range 0-28, Social/Family Well-Being (SWB): 7 items; score range 0-28, Emotional Well-Being (EWB): 6 items; score range 0-24, Functional Well-Being (FWB): 7 items; score range 0-28, Colorectal Cancer Subscale (CCS):9 items; score range 0-36.The FACT-C total score is derived by summing all five subscale scores.
Total score range: 0-144.

Partial Response (PR) is at least 4 points decrease in PCI.
Progressive Disease (PD) is at least 4 points increase in PCI.
The appearance of one or more new lesions is also considered progression.
Stable Disease (SD) is neither sufficient shrinkage to qualify for PR nor sufficient increase to qualify for PD, taking as reference the PCI.

Here is the number of participants with serious and/or non-serious adverse events assessed by the Common Terminology Criteria for Adverse Events (CTCAE v5.0).
A non-serious adverse event is any untoward medical occurrence.
A serious adverse event is an adverse event or suspected adverse reaction that results in death, a life-threatening adverse drug experience, hospitalization, disruption of the ability to conduct normal life functions, congenital anomaly/birth defect or important medical events that jeopardize the patient or subject and may require medical or surgical intervention to prevent one of the previous outcomes mentioned.
