COVID-19의 급성 발병 후 많은 환자들이 삶의 질에 상당한 손상과 함께 지속적이거나 재발하는 증상을 경험합니다. 가장 흔한 증상은 피로, 호흡곤란, 인지 장애 및 통증이지만 모든 유형의 증상이 보고되었습니다. 증상의 이질성과 잠재적인 병리생리학은 개별적이고 종합적인 관리를 필수적으로 만듭니다.
이 연구의 주요 목적은 COVID-19의 급성 발병 후 6주간의 개인화된 다학제 외래 환자 관리와 일반적인 치료 후 지속적인 증상이 있는 환자의 6개월 삶의 질 변화를 평가하는 것입니다.

상세 설명
COVID-19의 급성 발병 후 많은 환자들이 삶의 질에 상당한 손상과 함께 지속적이거나 재발하는 증상을 경험합니다. 가장 흔한 증상은 피로, 호흡곤란, 인지 장애 및 통증이지만 모든 유형의 증상이 보고되었습니다. 증상의 이질성과 잠재적인 병리생리학은 개별적이고 종합적인 관리를 필수적으로 만듭니다.
1차 목적은 COVID-19의 급성 발병 후 지속적인 증상이 있는 환자에서 6주간 개인화된 다학제 외래 환자 관리와 일반적인 치료를 받은 후 6개월 동안 삶의 질 변화를 평가하는 것입니다. 2차 목적은 3개월째 삶의 질의 변화, 주요 지속 증상(피로, 호흡곤란, 인지불안, 통증)의 변화, 환자의 만족도, 변화의 예측 및 설명 요인을 평가하는 것이다.
이것은 두 개의 병렬 부문에서 진행되는 전향적 무작위 공개 연구입니다: 개인화된 다학제 외래 환자 개입 대 일반적인 치료(대기자 명단). 이 6주 간병 프로그램에는 다음이 포함됩니다.
- 심리 교육 구성 요소를 포함한 그룹 교육 세션(1 세션/주)
- VO2max 운동 테스트 결과에 맞게 조정된 맞춤형 운동 훈련 프로토콜(매주 1회 감독에서 지도 운동 3회 세션)
- 기능 장애가 있는 건강 신념이 확인된 경우(SSD-12 점수 ≥ 26): 그룹 인지 및 행동 치료(직접 1회 이상을 포함하여 주당 2회 세션).
- 인지 불만 및/또는 신경 심리학적 장애가 있는 경우: 인지 교정 프로토콜(주당 그룹 세션 1회 + 가정 세션 2회)

포함 기준:
다음 기준 중 하나에 따라 증상이 있는 COVID-19의 초기 에피소드가 있었습니다.
- SARS-Cov-2 PCR +
- SARS-Cov-2 + 항원 검사
- SARS-Cov-2 + 혈청학
- 갑작스럽게 발병하는 장기간의 후각 상실증/무지증
- 일반적인 흉부 CT 스캔
- 질병의 급성기가 시작된 후 4주가 지나도 여전히 초기 증상 중 하나 이상과 피로, 호흡곤란, 인지 장애 또는 통증을 포함한 새로운 증상이 있을 수 있습니다.
- 이러한 초기 및 지속적인 증상은 Covid-19와 관련이 없는 것으로 알려지지 않은 다른 진단으로 더 잘 설명되지 않습니다.
- 이러한 증상은 환자가 중요하다고 생각하는 삶의 질과 전반적인 기능의 변화의 원인입니다.
- 감독 운동 및/또는 인지 행동 요법 및/또는 인지 교정에 대한 적응증이 있음 제외 기준
- 연구와 양립할 수 없는 관리가 필요한 의학적 응급 상황
- Covid-19 에피소드 이전에 인지 기능을 변화시킬 가능성이 있는 신경 정신 장애
- 심낭염 또는 진행성 심근염과 같은 운동 훈련에 대한 의학적 금기
- 국가 의료 지원을 받고 있는 환자
- 프랑스어를 구사하지 못하는 환자
- 임신 진행 중
