Na een acute episode van COVID-19 ervaren veel patiënten aanhoudende of terugkerende symptomen die hun kwaliteit van leven aanzienlijk aantasten. De meest voorkomende symptomen zijn vermoeidheid, kortademigheid, cognitieve stoornissen en pijn, maar alle soorten symptomen zijn gemeld. De heterogeniteit van de symptomen en hun potentiële pathofysiologie maakt geïndividualiseerd en multidisciplinair management essentieel.
Het primaire doel van deze studie is het evalueren van de verandering in kwaliteit van leven na 6 maanden bij patiënten met aanhoudende symptomen na een acute episode van COVID-19 na 6 weken gepersonaliseerde multidisciplinaire poliklinische behandeling versus gebruikelijke zorg.

Na een acute episode van COVID-19 ervaren veel patiënten aanhoudende of terugkerende symptomen die hun kwaliteit van leven aanzienlijk aantasten. De meest voorkomende symptomen zijn vermoeidheid, kortademigheid, cognitieve stoornissen en pijn, maar alle soorten symptomen zijn gemeld. De heterogeniteit van de symptomen en hun potentiële pathofysiologie maakt geïndividualiseerd en multidisciplinair management essentieel.
Het primaire doel is het evalueren van de verandering in kwaliteit van leven na 6 maanden bij patiënten met aanhoudende symptomen na een acute episode van COVID-19 na 6 weken gepersonaliseerde multidisciplinaire poliklinische behandeling versus gebruikelijke zorg. De secundaire doelstellingen zijn het evalueren van de evolutie van de levenskwaliteit na 3 maanden, de evolutie van de belangrijkste aanhoudende symptomen (vermoeidheid, kortademigheid, cognitieve klachten, pijn), de tevredenheid van de patiënt, de voorspellende en verklarende factoren van de evolutie.
Dit is een prospectieve gerandomiseerde open-label studie in twee parallelle armen: gepersonaliseerde multidisciplinaire poliklinische interventie versus gebruikelijke zorg (wachtlijst). Dit zorgprogramma van 6 weken omvat:
- groepseducatiesessies inclusief een psycho-educatiecomponent (1 sessie / week)
- een gepersonaliseerd oefenprotocol (van 1 sessie supervisie tot 3 sessies begeleide oefeningen per week) aangepast aan de resultaten van de VO2max inspanningstest
- als disfunctionele gezondheidsopvattingen worden geïdentificeerd (SSD-12-score ≥ 26): cognitieve groepstherapie en gedragstherapie (2 sessies per week, waarvan minstens 1 persoonlijk).
- bij cognitieve klachten en/of neuropsychologische stoornissen: een cognitief remediëringsprotocol (1 groepssessie plus 2 thuissessies per week)

Inclusiecriteria:
Een eerste episode van symptomatische COVID-19 hebben gehad volgens een van de volgende criteria:
- SARS-Cov-2 PCR+
- SARS-Cov-2 + antigene test
- SARS-Cov-2 + serologie
- Langdurige anosmie/ageusie met plotseling begin
- Typische thorax-CT-scan
- U heeft nog steeds ten minste één van de eerste symptomen en mogelijk nieuwe symptomen zoals vermoeidheid, kortademigheid, cognitieve stoornissen of pijn langer dan 4 weken na het begin van de acute fase van de ziekte.
- Deze aanvankelijke en aanhoudende symptomen kunnen niet beter worden verklaard door een andere diagnose waarvan niet bekend is dat deze verband houdt met Covid-19.
- Deze symptomen zijn de oorzaak van een verandering in kwaliteit van leven en algemeen functioneren die door de patiënt als significant wordt beschouwd.
- Een indicatie hebben voor ofwel begeleide oefentherapie en/of cognitieve gedragstherapie en/of cognitieve remediatie Uitsluitingscriteria
- Medische noodsituatie die management vereist, niet verenigbaar met onderzoek
- Neuropsychiatrische stoornis die waarschijnlijk cognitieve functies verandert, voorafgaand aan de Covid-19-episode
- Medische contra-indicatie voor training zoals pericarditis of progressieve myocarditis
- Patiënt onder medische bijstand van de staat
- Patiënt die geen Frans spreekt
- Zwangerschap in volle gang
