연구 개요
상세 설명
이것은 양적 연구입니다. Dexcom CGM 데이터는 기록 기간 동안 수집됩니다. 참가자는 두 차례에 걸쳐 세 번의 아침 식사를 테스트하고 각 테스트 식사 후 식후 설문지를 제출하여 테스트를 확인해야 합니다. 식후 기간은 아침 식사 시험 식사 종료 후 3시간입니다. 시험 식사 레시피는 수석 조사관이 제공합니다. 식사의 식이 구성은 식이 분석 프로그램을 사용하여 분석되었습니다. 식사의 혈당 지수 및 부하는 혈당 지수 및 혈당 부하의 국제 표에서 사용 가능한 날짜를 사용하는 이 프로그램에서 추정됩니다(Atkinson et al. 2008; 2021). 대부분의 식품은 다양한 브랜드의 평균 구성을 기반으로 했습니다. 시험식 3에 사용된 첨가 섬유질 빵은 수석 조사관이 산정한 평균 3개의 알려진 브랜드를 사용하였다. 식사는 가족이 제공하는 음식으로 가정 환경에서 이루어집니다. 참가자는 CGM 수치가 4-10mmol/l 사이이고 계획된 테스트 전날 밤에 야간 저혈당증이 없는 날에 테스트 식사를 하도록 요청받을 것입니다. 참가자는 자신의 컨트롤 역할을 합니다. 그들은 제어 식사에 대한 식후 설문지를 작성하도록 요청받을 것입니다. 대조군 식사는 선택하는 일반적인 아침 식사가 될 것입니다. 연구 기간은 첫 번째 참가자 모집에서 시작하여 데이터가 완전히 설명될 때 종료되며 2년으로 예상됩니다.
모집 영국 전역에서 일하는 소아 당뇨병 영양사는 참가자 모집을 돕고 해당 사이트의 주요 조사자(PI)가 되기 위해 등록됩니다.
샘플 크기 샘플 크기 검정력 계산은 64입니다. 이는 작은 효과 크기(0.15), P 값 0.05 및 검정력 0.80, 반복 측정 간의 상관 관계 0.5인 4개의 그룹 및 측정(3회 시험 식사 및 대조군)이 있는 요인 내에서 ANOVA 반복 측정을 사용하는 것을 기반으로 합니다. 임상시험 탈락률은 40%가 넘는 경우가 많다. 따라서 목표는 90명의 참가자를 모집하여 이 탈락률을 허용하고 64개의 샘플 크기를 충족하는 것입니다. 이 정도의 참가자를 모집하는 것이 가능할 수도 있습니다. 방법론 기준선 데이터
관련 변수와 포도당 수준을 비교하기 위해 영양사로부터 다음과 같은 기본 데이터를 수집하여 참가자 모집 단계에서 앞서 논의한 Excel 스프레드시트의 인공 식별자와 함께 수석 조사관에게 보냅니다.

- 현재 인슐린 요법 - 처방된 인슐린 유형 및 해당되는 경우 인슐린 펌프 유형, 즉 개방형 또는 폐쇄형 루프 시스템을 포함합니다.
준비 기간 모집 시 영양사는 인슐린 대 탄수화물 비율(ICR), 인슐린 감수성 인자(ISF) 및 기본 배경 인슐린 용량/비율을 포함하여 참가자의 인슐린 요법에 대한 검토를 준비하도록 요청받을 것입니다. 영양사는 이것이 발생했을 때 수석 조사관에게 알리도록 요청받을 것입니다. 참가자는 이후 테스트 식사를 시작해야 합니다.
포도당 측정 간질 포도당에 대한 데이터는 Dexcom CGM을 통해 수집됩니다. Dexcom CGM 데이터는 NHS Highland 연구 'Clarity Clinic'과 Dexcom CLARITY® Clinic Portal(Dexcom In, San Diego, California(CA), USA)에서 액세스할 수 있습니다. 수석 수사관은 이 클리닉의 관리자입니다. 관리 영양사가 각 참가자의 동의를 얻으면 참가자의 부모의 이메일 주소가 위에서 설명한 기본 정보/데이터와 함께 수석 조사관에게 전송됩니다. 수석 조사관은 Clarity Clinic 계정의 관리자로서 이메일을 통해 참가자를 클리닉에 추가하도록 초대합니다. 초대가 수락되면 기록 기간, 즉 모든 시험 식사와 설문지가 완료될 때까지 유지됩니다. 참가자가 마지막 설문지를 제출하면 Dexcom CLARITY® Clinic Portal에서 제거됩니다.
CGM 데이터 제공 및 당뇨병 팀의 인슐린 요법 검토에 따라 참가자는 각 테스트 식사에 대한 제어 식사(일반적인 아침 식사)와 함께 두 번의 개별적인 경우에 세 가지 테스트 식사 각각을 테스트하도록 무작위 배정됩니다. 무작위화는 라틴 사각형 무작위화를 사용하여 달성됩니다. 이에 사용되는 도구는 http://www.jerrydallal.com/random/randomize.htm입니다. 무작위화는 4개의 블록에서 수행됩니다.
시험 급식은 어린이와 청소년이 즐겨 먹는 음식을 기준으로 합니다. 세 가지 식사가 있습니다. 테스트 식사 1은 우유와 함께 고혈당 지수 시리얼 식사를 포함하고 혈당 부하가 높음(>20), 테스트 식사 2는 단백질 10g이 추가된 동일한 혈당 부하(>20) 시리얼 식사이며 테스트 식사 3은 중간 혈당 부하(10-20). 테스트 식사 1과 2의 경우 연령에 맞는 요구 사항과 식욕을 충족시키기 위해 세 가지 부분 크기 옵션이 있습니다. 참가자는 테스트 식사 1을 위해 두 가지 다른 시리얼 중에서 선택할 수 있고 테스트 식사 2를 위해 단백질 공급원을 선택할 수 있습니다. 그들은 반복 식사를 위해 동일한 시리얼 및 단백질 공급원을 선택하도록 요청받습니다. 그들은 혈당 부하의 역치를 충족시키기 위해 식사의 최소 75%를 소비하도록 요청받을 것입니다. 시험 식사의 지침 및 세부 사항은 부록 11에 나와 있습니다. 제어 식사는 참가자가 선택한 일반적인 아침 식사가 됩니다.
참가자는 당뇨병 팀의 조언에 따라 일반적인 인슐린 요법, 즉 평소 인슐린 용량 및 용량 타이밍을 따르도록 요청받을 것입니다. 테스트 식사와 통제 식사 모두 참가자는 식후 3시간 동안 저혈당증을 치료하는 데 필요한 탄수화물 조절 외에 추가 음식 섭취를 피해야 합니다. 참가자는 또한 물이나 무탄수화물 주스를 제외한 음료를 피해야 합니다. 또한 30분 이상의 신체 활동을 피하도록 요청합니다. 정상적인 일상 활동에 지장을 최소화하기 위해 계획된 신체 활동이 없는 날(예: 아침 체육이 없는 수업일)에 식사를 테스트하는 것이 좋습니다. 위에서 설명한 대로 이 기간 내내 CGM 데이터가 수집됩니다.
