가정 내압 원격 모니터링이 안압 조절 및 녹내장 진행에 미치는 영향 - 무작위 통제 시험
이 임상 시험의 목표는 녹내장 환자를 스마트폰 기반 개입(관리 패러다임 I) 또는 스마트폰 기반 개입 단독(관리 패러다임 II)과 결합된 가정 IOP 원격 모니터링에 무작위 배정하는 연구를 수행하는 것입니다. ) 전체 연구 기간에 걸친 GAT IOP 측정(1차 결과 측정) 및 (2) 2개의 관리 패러다임 사이의 망막 신경 섬유층(RNFL) 얇아짐 비율(2차 결과 측정). 우리는 관리 패러다임 I에 무작위 배정된 녹내장 환자가 무작위 배정된 환자에 비해 (1) 더 낮은 수준의 안압에 도달하고 (2) 망막 신경 섬유층(RNFL) 및 신경절 세포 내부 망상층(GCIPL)이 얇아지는 속도가 더 느리다는 가설을 세웁니다. 관리 패러다임 II는 안압 저하 요법을 안내하기 위해 안압을 더 정확하게 평가하기 때문에 관리 패러다임 I에서 실현 가능할 것입니다.
다음을 목표로 합니다.
(1) 전체 연구 기간 동안의 GAT IOP 측정(1차 결과 측정) 및 (2) 두 관리 패러다임 사이의 RNFL 감소율(2차 결과 측정)을 비교합니다.
참가자가 할 것을 요구할 것입니다,
- 관리 패러다임 I: iCare Home이 제공되고 매주 6개의 IOP 측정을 측정하고 업로드하도록 지시받게 됩니다(주 2일, 이른 아침(오전 5시~오전 9시)에 1회 측정, 정오(오후 12시)에 1회 측정) ~ 오후 4시) 및 저녁 1시(오후 7시 ~ 오후 11시)) iCare CLINIC을 통해 보안 서버로 전송합니다(주간 IOP 측정 횟수는 HyperLink 연구의 주간 혈압 측정 횟수를 따릅니다). 아침 측정에는 침대에서 일어나기 전에 바로 누운 자세에서 얻은 첫 번째 판독값과 직후에 똑바로 세운 자세에서 얻은 두 번째 판독값이 포함됩니다. 환자는 잠에서 깨는 경우뿐만 아니라 하루 중 다른 시간에도 바로 누운 자세에서 추가 IOP 측정을 할 수 있지만 필수 사항은 아닙니다. 이러한 추가 IOP 측정은 연구 기간 동안 치료 결정에 포함되지 않습니다.

- 새로 진단된 원발성 개방각 녹내장(POAG)
- 포함된 눈의 최고 교정 시력(VA) ≥20/40
제외 기준:
- 안압 >35mmHg
- 안구건조증
- 중앙 각막 두께 <500μm 또는 >600μm
- 기본 방문 시 iCare Home 인증 절차를 완료하지 못한 경우
- 기능적 시력을 가진 한쪽 눈만
- 신뢰할 수 있는 VF를 수행할 수 없음
- 병적근시
- OCT 이미지의 차선 품질(RNFL 이미징에서 아래에 설명됨)
- 합병증이 없는 백내장 적출 이외의 이전의 안내 수술 또는 각막 굴절 수술
- 당뇨망막병증/황반병증

활성 비교기: Management Paradigm I: Home IOP telemonitoring with smart phone-based intervention
Before treatment initiation, patients perform 4 weeks of baseline home IOP measurements, then continue to obtain at least six IOP measurements per week (two readings in the early morning, and one each at mid-day and evening on two days per week) and upload readings via the iCare CLINIC cloud platform. Investigators review home IOP data every 4 weeks and adjust topical IOP-lowering therapy according to a predefined algorithm whenever fewer than 75% of home readings are below the target IOP based on disease severity. Every 4 weeks, patients receive smartphone text messages indicating whether the home-based target IOP has been achieved and reminding them about eye-drop adherence; patients report missed doses by text, and a nurse calls patients who do not reply or who submit fewer than 20 home IOP readings in 4 weeks. |
Participants use the iCare HOME rebound tonometer to perform self-measurements of intraocular pressure at home for 30 months. After a 4-week baseline period with at least 24 measurements (early morning supine and sitting, mid-day, and evening on different days), patients continue to obtain at least six IOP measurements per week and upload readings to the secure iCare CLINIC cloud system. Investigators review home IOP profiles every 4 weeks and use a predefined algorithm to determine whether treatment goals are met and whether glaucoma therapy should be intensified. |

활성 비교기: Management paradigm II: Standard care and smart phone-based intervention
Topical IOP-lowering therapy is escalated according to the same predefined stepwise protocol used in Management Paradigm I when target IOP is not reached. No home IOP telemonitoring is used; treatment decisions rely on clinic-measured IOP and structural/functional assessments. Every 4 weeks, patients receive smartphone text messages indicating whether the target pressure has been attained based on the latest clinic Goldmann measurement and reminding them about eye-drop adherence; patients report missed doses by text, and clinic staff follow up by phone as needed. |
Every 4 weeks, participants receive automated or investigator-generated smartphone text messages summarizing whether their individualized target intraocular pressure has been achieved and reminding them to adhere to prescribed eye-drop therapy.
In Management Paradigm I, the feedback is based on home IOP readings from the iCare HOME device, whereas in Management Paradigm II it is based on the most recent clinic Goldmann applanation measurement.
Participants reply by text with the number of missed eye-drop doses over the past 4 weeks, and clinic staff contact patients by telephone when replies or monitoring data are insufficient.
