Impact de la télésurveillance de la pression intraoculaire à domicile sur le contrôle de la pression intraoculaire et la progression du glaucome - Un essai contrôlé randomisé
Le but de cet essai clinique est de mener une étude randomisant des patients atteints de glaucome vers une télésurveillance de la PIO à domicile associée à une intervention basée sur un smartphone (Management Paradigm I) ou une intervention basée sur un smartphone seule (Management Paradigm II), avec des objectifs à comparer (1 ) Mesures GAT IOP sur toute la période d'étude (mesure de résultat primaire) et (2) les taux d'amincissement de la couche de fibres nerveuses rétiniennes (RNFL) (mesure de résultat secondaire) entre les deux paradigmes de gestion. Nous émettons l'hypothèse que les patients atteints de glaucome randomisés au Management Paradigm I atteindront (1) des niveaux inférieurs de PIO et (2) un taux plus lent d'amincissement de la couche de fibres nerveuses rétiniennes (RNFL) et de la couche plexiforme interne des cellules ganglionnaires (GCIPL) par rapport à ceux randomisés pour Management Paradigm II en raison d'une évaluation plus précise de la PIO pour guider le traitement de réduction de la PIO serait réalisable dans Management Paradigm I.
Il vise à :
pour comparer (1) les mesures GAT IOP sur toute la période d'étude (mesure de résultat primaire) et (2) les taux d'amincissement RNFL (mesure de résultat secondaire) entre les deux paradigmes de gestion.
Les participants seront invités à faire,
- Paradigme de gestion I : recevra un iCare Home et sera chargé de mesurer et de télécharger 6 mesures de la PIO par semaine (2 jours par semaine, 1 mesure tôt le matin (de 5 h à 9 h), 1 pendant la mi-journée (12 h à 16 h) et 1 le soir (19 h à 23 h)) à un serveur sécurisé via iCare CLINIC (le nombre de mesures hebdomadaires de la PIO suit le nombre de mesures hebdomadaires de la pression artérielle dans l'étude HyperLink). La mesure du matin comprendra deux lectures avec la première obtenue en position couchée avant de sortir du lit et la seconde obtenue en position debout juste après. Les patients peuvent prendre des mesures supplémentaires de la PIO en position couchée s'ils se réveillent au lit après avoir dormi, ainsi qu'à d'autres moments de la journée, mais ce n'est pas obligatoire. Ces mesures supplémentaires de la PIO ne seront pas incluses dans les décisions de traitement pendant la période d'étude.
- Paradigme de prise en charge II : les patients seront traités avec un analogue topique de la prostaglandine après les mesures initiales de la PIO.

Critère d'intégration:
- Glaucome primaire à angle ouvert (GPAO) nouvellement diagnostiqué
- Meilleure acuité visuelle corrigée (VA) ≥20/40 pour le ou les yeux inclus
Critère d'exclusion:
- PIO > 35 mm Hg
- Syndrome de l'oeil sec
- Épaisseur cornéenne centrale <500 μm ou > 600 μm
- Non-achèvement de la procédure de certification iCare Home lors des visites de base
- Un seul œil avec une vision fonctionnelle
- Incapacité à effectuer une FV fiable
- Myopie pathologique (yeux de longueur axiale ≥ 26 mm avec fissures de laque et atrophie choriorétinienne)
- Qualité sous-optimale des images OCT (décrite ci-dessous dans l'imagerie RNFL)
- Chirurgie intraoculaire antérieure ou chirurgie réfractive cornéenne autre que l'extraction de la cataracte sans complication
- Rétinopathie diabétique/maculopathie

Comparateur actif: Management Paradigm I: Home IOP telemonitoring with smart phone-based intervention
Before treatment initiation, patients perform 4 weeks of baseline home IOP measurements, then continue to obtain at least six IOP measurements per week (two readings in the early morning, and one each at mid-day and evening on two days per week) and upload readings via the iCare CLINIC cloud platform. Investigators review home IOP data every 4 weeks and adjust topical IOP-lowering therapy according to a predefined algorithm whenever fewer than 75% of home readings are below the target IOP based on disease severity. Every 4 weeks, patients receive smartphone text messages indicating whether the home-based target IOP has been achieved and reminding them about eye-drop adherence; patients report missed doses by text, and a nurse calls patients who do not reply or who submit fewer than 20 home IOP readings in 4 weeks. |
Participants use the iCare HOME rebound tonometer to perform self-measurements of intraocular pressure at home for 30 months. After a 4-week baseline period with at least 24 measurements (early morning supine and sitting, mid-day, and evening on different days), patients continue to obtain at least six IOP measurements per week and upload readings to the secure iCare CLINIC cloud system. Investigators review home IOP profiles every 4 weeks and use a predefined algorithm to determine whether treatment goals are met and whether glaucoma therapy should be intensified. |

Comparateur actif: Management paradigm II: Standard care and smart phone-based intervention
Topical IOP-lowering therapy is escalated according to the same predefined stepwise protocol used in Management Paradigm I when target IOP is not reached. No home IOP telemonitoring is used; treatment decisions rely on clinic-measured IOP and structural/functional assessments. Every 4 weeks, patients receive smartphone text messages indicating whether the target pressure has been attained based on the latest clinic Goldmann measurement and reminding them about eye-drop adherence; patients report missed doses by text, and clinic staff follow up by phone as needed. |
Every 4 weeks, participants receive automated or investigator-generated smartphone text messages summarizing whether their individualized target intraocular pressure has been achieved and reminding them to adhere to prescribed eye-drop therapy.
In Management Paradigm I, the feedback is based on home IOP readings from the iCare HOME device, whereas in Management Paradigm II it is based on the most recent clinic Goldmann applanation measurement.
Participants reply by text with the number of missed eye-drop doses over the past 4 weeks, and clinic staff contact patients by telephone when replies or monitoring data are insufficient.
