Impacto do Telemonitoramento da Pressão Intraocular Domiciliar no Controle da Pressão Intraocular e na Progressão do Glaucoma - Um Estudo de Controle Randomizado
O objetivo deste ensaio clínico é conduzir um estudo randomizando pacientes com glaucoma para telemonitoramento doméstico da PIO combinado com intervenção baseada em smartphone (Management Paradigm I) ou apenas intervenção baseada em smartphone (Management Paradigm II), com os objetivos de comparar (1 ) Medições GAT IOP durante todo o período do estudo (medida de resultado primário) e (2) as taxas de afinamento da camada de fibras nervosas da retina (RNFL) (medida de resultado secundário) entre os dois paradigmas de gerenciamento. Nossa hipótese é que os pacientes com glaucoma randomizados para o Paradigma de Tratamento I (1) atingirão níveis mais baixos de PIO e (2) uma taxa mais lenta de afinamento da camada de fibras nervosas da retina (RNFL) e da camada plexiforme interna de células ganglionares (GCIPL) em comparação com aqueles randomizados para O Paradigma de Gerenciamento II, por ter uma avaliação mais precisa da PIO para orientar a terapia de redução da PIO, seria viável no Paradigma de Gerenciamento I.
Tem como objetivo:
comparar (1) as medições GAT IOP durante todo o período do estudo (medida de resultado primário) e (2) as taxas de afinamento RNFL (medida de resultado secundário) entre os dois paradigmas de gerenciamento.
Os participantes serão solicitados a fazer,
- Paradigma de gerenciamento I: será fornecido um iCare Home e instruído a medir e carregar 6 medições de PIO semanalmente (2 dias por semana, 1 medição no início da manhã (5h às 9h), 1 durante o meio-dia (12h às 16:00) e 1 da noite (19:00 às 23:00)) para um servidor seguro via iCare CLINIC (o número de medições semanais de PIO segue o número de medições semanais de pressão arterial no estudo HyperLink). A medição matinal incluirá duas leituras com a primeira obtida na posição supina antes de sair da cama e a segunda obtida na posição ereta logo após. Os pacientes podem fazer medições adicionais da PIO na posição supina se acordarem na cama após dormir, bem como em outras horas do dia, mas isso não é obrigatório. Essas medições adicionais de PIO não serão incluídas para decisões de tratamento durante o período do estudo.
- Paradigma de tratamento II: Os pacientes serão tratados com um análogo de prostaglandina tópica após as medições basais da PIO.

Critério de inclusão:
- Glaucoma primário de ângulo aberto (GPAA) recém-diagnosticado
- Melhor acuidade visual corrigida (AV) ≥20/40 para o(s) olho(s) incluído(s)
Critério de exclusão:
- PIO > 35 mm Hg
- Síndrome do olho seco
- Espessura central da córnea <500μm ou >600μm
- Falha ao concluir o procedimento de certificação iCare Home nas visitas de linha de base
- Apenas um olho com visão funcional
- Incapacidade de realizar VF confiável
- Miopia patológica (olhos com comprimento axial≥26mm com rachaduras de laca e atrofia coriorretiniana)
- Qualidade abaixo do ideal das imagens de OCT (descritas abaixo na imagem RNFL)
- Cirurgia intraocular anterior ou cirurgia refrativa da córnea, exceto extração de catarata não complicada
- Retinopatia diabética/maculopatia

Comparador Ativo: Management Paradigm I: Home IOP telemonitoring with smart phone-based intervention
Before treatment initiation, patients perform 4 weeks of baseline home IOP measurements, then continue to obtain at least six IOP measurements per week (two readings in the early morning, and one each at mid-day and evening on two days per week) and upload readings via the iCare CLINIC cloud platform. Investigators review home IOP data every 4 weeks and adjust topical IOP-lowering therapy according to a predefined algorithm whenever fewer than 75% of home readings are below the target IOP based on disease severity. Every 4 weeks, patients receive smartphone text messages indicating whether the home-based target IOP has been achieved and reminding them about eye-drop adherence; patients report missed doses by text, and a nurse calls patients who do not reply or who submit fewer than 20 home IOP readings in 4 weeks. |
Participants use the iCare HOME rebound tonometer to perform self-measurements of intraocular pressure at home for 30 months. After a 4-week baseline period with at least 24 measurements (early morning supine and sitting, mid-day, and evening on different days), patients continue to obtain at least six IOP measurements per week and upload readings to the secure iCare CLINIC cloud system. Investigators review home IOP profiles every 4 weeks and use a predefined algorithm to determine whether treatment goals are met and whether glaucoma therapy should be intensified. |

Comparador Ativo: Management paradigm II: Standard care and smart phone-based intervention
Topical IOP-lowering therapy is escalated according to the same predefined stepwise protocol used in Management Paradigm I when target IOP is not reached. No home IOP telemonitoring is used; treatment decisions rely on clinic-measured IOP and structural/functional assessments. Every 4 weeks, patients receive smartphone text messages indicating whether the target pressure has been attained based on the latest clinic Goldmann measurement and reminding them about eye-drop adherence; patients report missed doses by text, and clinic staff follow up by phone as needed. |
Every 4 weeks, participants receive automated or investigator-generated smartphone text messages summarizing whether their individualized target intraocular pressure has been achieved and reminding them to adhere to prescribed eye-drop therapy.
In Management Paradigm I, the feedback is based on home IOP readings from the iCare HOME device, whereas in Management Paradigm II it is based on the most recent clinic Goldmann applanation measurement.
Participants reply by text with the number of missed eye-drop doses over the past 4 weeks, and clinic staff contact patients by telephone when replies or monitoring data are insufficient.
