Impact van telemonitoring van intraoculaire druk thuis op controle van intraoculaire druk en progressie van glaucoom - een gerandomiseerde controlestudie
Het doel van deze klinische studie is om een studie uit te voeren waarbij DrDeramus-patiënten willekeurig worden ingedeeld in IOP-telemonitoring thuis in combinatie met op smartphones gebaseerde interventie (Management Paradigm I) of alleen op smartphones gebaseerde interventie (Management Paradigm II), met de doelstellingen om te vergelijken (1 ) GAT IOP-metingen over de gehele onderzoeksperiode (primaire uitkomstmaat) en (2) de snelheid waarmee de retinale zenuwvezellaag (RNFL) dunner wordt (secundaire uitkomstmaat) tussen de twee managementparadigma's. We veronderstellen dat DrDeramus-patiënten die zijn gerandomiseerd naar Management Paradigm I (1) lagere niveaus van IOP zullen bereiken, en (2) een langzamere verdunning van de retinale zenuwvezellaag (RNFL) en ganglioncel binnenste plexiforme laag (GCIPL) zullen hebben in vergelijking met degenen die zijn gerandomiseerd naar Managementparadigma II vanwege een nauwkeurigere beoordeling van de IOD om IOD-verlagende therapie te begeleiden, zou haalbaar zijn in Managementparadigma I.
Het heeft tot doel:
om (1) GAT IOP-metingen over de gehele onderzoeksperiode (primaire uitkomstmaat) en (2) de percentages van RNFL-verdunning (secundaire uitkomstmaat) tussen de twee managementparadigma's te vergelijken.
De deelnemers wordt gevraagd om te doen,
- Managementparadigma I: krijgt een iCare Home en wordt geïnstrueerd om wekelijks 6 IOP-metingen te meten en te uploaden (2 dagen per week, 1 meting in de vroege ochtend (5 uur tot 9 uur), 1 tijdens de middag (12 uur tot 16.00 uur) en 13.00 uur 's avonds (19.00 tot 23.00 uur)) naar een beveiligde server via iCare CLINIC (het aantal wekelijkse IOP-metingen volgt het aantal wekelijkse bloeddrukmetingen in de HyperLink-studie). De ochtendmeting omvat twee metingen, waarbij de eerste in rugligging wordt verkregen voordat u uit bed stapt en de tweede in rechtopstaande positie direct daarna. Patiënten kunnen extra IOP-metingen doen in rugligging als ze wakker worden in bed uit hun slaap, evenals op andere momenten van de dag, maar dit is niet verplicht. Deze aanvullende IOD-metingen worden niet meegenomen in behandelbeslissingen tijdens de onderzoeksperiode.
- Managementparadigma II: Patiënten zullen worden behandeld met een topische prostaglandine-analoog na basislijn-IOD-metingen.

Inclusiecriteria:
- Nieuw gediagnosticeerd primair openkamerhoekglaucoom (POAG)
- Beste gecorrigeerde gezichtsscherpte (VA) ≥20/40 voor het (de) ingesloten oog (ogen)
Uitsluitingscriteria:
- IOD >35 mm Hg
- Droge ogen syndroom
- Centrale hoornvliesdikte <500μm of >600μm
- Het niet voltooien van de iCare Home-certificeringsprocedure bij de basisbezoeken
- Slechts één oog met functioneel zicht
- Onvermogen om betrouwbare VF uit te voeren
- Pathologische bijziendheid (ogen met axiale lengte ≥26 mm met lakscheuren en chorioretinale atrofie)
- Suboptimale kwaliteit van OCT-afbeeldingen (hieronder beschreven in RNFL-beeldvorming)
- Eerdere intraoculaire chirurgie of corneale refractieve chirurgie anders dan ongecompliceerde cataractextractie
- Diabetische retinopathie/maculopathie

Actieve vergelijker: Management Paradigm I: Home IOP telemonitoring with smart phone-based intervention
Before treatment initiation, patients perform 4 weeks of baseline home IOP measurements, then continue to obtain at least six IOP measurements per week (two readings in the early morning, and one each at mid-day and evening on two days per week) and upload readings via the iCare CLINIC cloud platform. Investigators review home IOP data every 4 weeks and adjust topical IOP-lowering therapy according to a predefined algorithm whenever fewer than 75% of home readings are below the target IOP based on disease severity. Every 4 weeks, patients receive smartphone text messages indicating whether the home-based target IOP has been achieved and reminding them about eye-drop adherence; patients report missed doses by text, and a nurse calls patients who do not reply or who submit fewer than 20 home IOP readings in 4 weeks. |
Participants use the iCare HOME rebound tonometer to perform self-measurements of intraocular pressure at home for 30 months. After a 4-week baseline period with at least 24 measurements (early morning supine and sitting, mid-day, and evening on different days), patients continue to obtain at least six IOP measurements per week and upload readings to the secure iCare CLINIC cloud system. Investigators review home IOP profiles every 4 weeks and use a predefined algorithm to determine whether treatment goals are met and whether glaucoma therapy should be intensified. |

Actieve vergelijker: Management paradigm II: Standard care and smart phone-based intervention
Topical IOP-lowering therapy is escalated according to the same predefined stepwise protocol used in Management Paradigm I when target IOP is not reached. No home IOP telemonitoring is used; treatment decisions rely on clinic-measured IOP and structural/functional assessments. Every 4 weeks, patients receive smartphone text messages indicating whether the target pressure has been attained based on the latest clinic Goldmann measurement and reminding them about eye-drop adherence; patients report missed doses by text, and clinic staff follow up by phone as needed. |
Every 4 weeks, participants receive automated or investigator-generated smartphone text messages summarizing whether their individualized target intraocular pressure has been achieved and reminding them to adhere to prescribed eye-drop therapy.
In Management Paradigm I, the feedback is based on home IOP readings from the iCare HOME device, whereas in Management Paradigm II it is based on the most recent clinic Goldmann applanation measurement.
Participants reply by text with the number of missed eye-drop doses over the past 4 weeks, and clinic staff contact patients by telephone when replies or monitoring data are insufficient.
