Clostridioides Difficile 감염: 임상 진화 및 세균 제거를 분석하는 전향적 코호트 (DECLIC)
Clostridioides difficile(이전 Clostridium)은 환자가 노출될 수 있는 환경에서 포자 형태(저항성 형태)로 발견되는 박테리아입니다. 이 박테리아는 원래 Clostridium속에 속했으나 2016년 16S 리보솜 RNA를 분석한 결과 이 박테리아와 구별되었습니다. 포자를 섭취하면 영양 형태(박테리아의 활성 형태)로 발아하여 소화 미생물군에 서식하는 그람 양성 간균의 모습을 보일 수 있습니다. 박테리아에 의한 소화기 군집화의 예비 단계는 특정 요인, 특히 비위관 탐색, 제산제 등에 의해 촉진됩니다. 항생제는 소화 미생물군(장내 세균총 불균형)의 박테리아를 파괴하여 C. difficile의 이식을 촉진합니다. 특정 균주(독성 유발성으로 알려짐)는 독소 A(TcdA) 및 B(TcdB) ± 세 번째 독소(이진 독소 또는 CDT)와 같은 주요 병독성 인자를 생성하여 특히 소화기 감염의 주요 임상 징후를 유발합니다. C. difficile 감염의 위험 요인(진행성 암, 면역억제 등).
Clostridioides difficile 감염(CDI)은 종종 자발적으로 해결되는 대장염이 없는 단순한 장액성 설사부터 위막성 대장염, 장 천공 또는 패혈성 쇼크와 같은 합병증이 있는 심각한 형태에 이르기까지 예후가 매우 좋지 않은 다양한 임상 증상이 특징입니다.
이러한 유형의 CDI 관리는 주로 유럽 ESCMID, 영국 및 미국 IDSA 지침에서 권장하는 대로 피닥소마이신(FDX) 또는 반코마이신(VAN)과 같은 항클로스트리듐 디피실리 항생제를 10일 동안 경구 투여하는 데 의존합니다. 경구 메트로니다졸은 처음 두 분자를 이용할 수 없는 경우(공동체 사용)에만 권장됩니다. 이러한 치료에도 불구하고 CDI의 주요 특징 중 하나는 재발률이 높아 전체 사례의 25%에 달할 수 있다는 것입니다.
