HPV 관련 암 예방 RCT
예방: 안전망에서 HPV 백신 접종을 촉진하기 위한 실무 기반 접근 방식 - 무작위 대조 시험(RCT)

제안된 연구는 워싱턴 시골 카운티의 4개 지역사회 진료소에서 HPV 예방접종률을 높이기 위한 진료소 기반 지원의 효과를 평가할 것입니다. 이 연구는 환자 및 제공자 입력을 기반으로 메시징을 맞춤화하기 위한 부트 캠프 번역입니다. 제안된 연구에는 다음이 포함됩니다. (1) 환자 및 제공자 입력을 기반으로 메시징을 맞춤화하기 위한 부트 캠프 번역; (2) 성인 부모/보호자 참가자에게 개입을 받을 일정을 배정하는 무작위 대조 시험(RCT)을 방지합니다. 상세 설명
PREVENT 연구의 임상 시험은 Sea Mar Community Health Centers가 운영하는 시골 진료소에서 진행될 두 가지 인간 유두종 바이러스(HPV) 예방 접종 환자 알림 중재 부문에 대한 환자 무작위 대조 시험(RCT)입니다. BCT(BootCamp 번역) 활동의 결과는 영어와 스페인어로 진행되며 PREVENT RCT 개발 및 메시징 전략에 대한 정보를 제공합니다. BCT 활동은 RCT의 일부가 아닙니다.
PREVENT RCT는 다음 번 필요한 HPV 백신 접종 완료와 다회 접종 HPV 백신 시리즈의 적시 완료를 평가하는 3군 환자 무작위 대조 시험을 실시할 것입니다. 각 시험의 연구 부문은 자동 알림, 자동 및 실시간 알림, 일반 진료로 구성됩니다. 임상시험을 위해 선정된 아동 및 청소년의 부모/보호자는 주 예방접종 등록부에 연결된 전자 건강 기록에 적용되는 확립된 연구 기준을 사용하여 선택됩니다. 최소 위험 연구로서, 개입에 대해서만 조사자는 사전 동의 포기를 신청할 것입니다. RCT는 표준 치료의 일부로 제공되며 환자는 자신이 임상시험에 참여하고 있다는 사실을 알지 못합니다. 전달되는 백신 접종 메시지에는 BCT 활동 및 지역 정책에 따라 거부 선택 사항이 포함됩니다.
BCT 중에 정의된 특정 개입에 따라 아동 및 청소년(C/A)의 부모/보호자(P/C)는 부모가 연령에 맞는 C/A에 대한 HPV 백신을 접종하도록 권장하는 알림을 원하는 만큼 전송받게 됩니다. 알림에는 문자 메시지, 자동 전화 통화, 우편 편지, 실시간 통화 또는 환자 내비게이션이 포함될 수 있습니다. 알림은 공급업체(자동 알림) 또는 진료소 직원(실시간 알림)에 의해 전달됩니다. 알림은 영어와 스페인어로 전달되며, 영어나 스페인어 이외의 언어를 사용하는 소수의 환자에게 실시간 알림을 제공하기 위해 통역사 서비스가 사용될 수 있습니다. 조사관은 전자 건강 기록 데이터를 사용하여 HPV 예방 접종 사건을 주요 관심 결과로 기록할 것입니다. 조사관은 또한 주어진 개입 구성 요소를 받는 환자의 비율로 정의되는 각 개입 구성 요소의 도달 범위를 평가할 것입니다. 연구자들은 또한 연구 기간 동안 각 연구군에 대해 HPV가 아닌 다른 권장 백신(예: Tdap 또는 수막구균)을 접종받은 환자의 비율로 정의된 놓친 기회를 평가할 것입니다. 목표 3은 프로그램에 대한 반응, 구현 및 장기 지속 가능성과 관련된 요소, 추가 임상 기반 개입 기회를 평가하기 위해 환자 및 제공자 수준의 질적 데이터를 수집합니다. 활발한 연구 모집 기간 동안 조사관은 6개월마다 데이터 안전 모니터 위원회 회의를 소집합니다. 조사관은 당사의 보급 계획에 따라 연구 결과와 연구 제품을 보급할 것입니다. 연구 결과는 농촌 지역사회에서 HPV 예방접종에 대한 대규모 다단계 시험의 기초가 될 것입니다.
