신경생리학적 도구로 측정한 상지 피라미드로 관련 경직 환자에서 고대비저대비 펄스 지속 시간 신경근 전기 자극(NMES)의 효과: 무작위 대조 시험
경직은 피라미드로 손상으로 인한 상위 운동 신경원 병변 환자의 50% 이상에게 영향을 미치는 일반적인 증상입니다. 현재의 약물 및 물리 치료 재활 방법에도 불구하고, 이전 연구들은 상지 경직 관리에 대한 신경근 전기 자극(NMES)의 유익한 역할을 강조해 왔습니다. 그러나 적용 매개변수의 이질성으로 인해 경직 관리를 위한 표준화된 프로토콜이 부족합니다. 본 연구의 목적은 피라미드로 관련 경직 환자에게 고대비 저대비 펄스 지속 시간 신경근 전기 자극이 상지 경직에 미치는 영향을 조사하는 것입니다.
총 45명의 환자가 고펄스 지속 시간 NMES(HPD-NMES) 또는 저펄스 지속 시간 NMES(LPD-NMES) 또는 대조군(표준 치료를 받음)에 1:1:1 비율로 무작위 배정됩니다. 무작위 배정은 독립 연구자가 기초 평가 전에 난수 생성기를 사용하여 참가자를 세 그룹 중 하나에 할당하여 수행됩니다. 각 그룹은 15분의 기존 물리치료 프로토콜을 받게 됩니다. HPD-NMES와 LPD-NMES는 각각 고 및 저 펄스 지속 시간의 추가 30분 NMES 프로토콜을 받게 됩니다. 중재 전후 경직은 전자 각도계를 통한 팔꿈치 관절의 관절 가동 범위(ROM), 수정 애쉬워스 척도(MAS) 및 표면 근전도(EMG)를 사용하여 평가됩니다. 또한, 수정 바델 지수(MBI) 및 세계보건기구 장애 평가 일정 12판(WHODAs)이 참가자의 삶의 질 평가에 사용됩니다. 통계 분석은 NMES가 EMG-전기생리학적 매개변수와 임상 평가 척도 모두에 미치는 영향을 강조하는 것을 목표로 합니다. 추가적으로, 두 NMES 펄스 지속 시간 중 어느 것이 경직 수준 감소에 더 유익한 결과를 가져왔는지 결정하려고 합니다.

경련성(spasticity)은 상위 운동 신경 증후군과 관련된 임상 표현형으로 특징지을 수 있으며, 피라미드로의 병변과 높은 상관관계가 있어 환자의 삶의 질 저하로 이어집니다. 경련성 평가에는 임상 평가 척도(Modified Ashworh Scale, Modified Tardieu Scale, Composite Spasticity Scale 등)와 표면 근전도를 통한 신경생리학적 접근법(Hoffmann 반사(H-반사), M파, Hmax/Mmax 비율(또는 M/H 진폭 비율) 등)이 포함되며, 이는 경련성에서 증가하는 것으로 보입니다. 약물 및 비약물적 개입을 포함한 광범위한 치료 방식이 경련성 관리에 개발되었습니다. 비약물적 개입 중, 신경근 전기 자극을 통한 물리치료는 신장 반사 흥분성 감소, 상호 억제 촉진 및 척수 흥분성 조절을 통해 상지 및 하지 경련성을 관리하는 데 사용될 수 있습니다.
이 무작위 대조 시험은 다음과 같이 구성됩니다:
- 참가자를 대조군(그룹 A, n=15)과 두 개의 중재군: 그룹 B(고 펄스 지속 시간 신경근 전기 자극/HPD-NMES, n=15), 그룹 C(저 펄스 지속 시간 신경근 전기 자극/LPD-NMES, n=15)로 무작위 배정합니다.
- 전통적 물리치료 훈련 프로그램을 세션당 15분, 주 3회, 총 6주 동안 시행(그룹 A, B, C)하고, 신경근 전기 자극 프로토콜을 세션당 30분, 주 3회, 총 6주 동안 시행합니다(그룹 B=HPD-NMES 및 그룹 C=LPD-NMES).
- 표면 근전도를 통한 기초선 및 중재 후(6주) 1차 경련성 평가 도구.
- 기초선 및 중재 후(6주) 2차 경련성 평가 도구(Modified Ashworth Scale/MAS, 관절 가동 범위/ROM, Modified Barthel Index/MBI 및 12판 세계보건기구 장애 평가 일정 2.0/WHODAS 2.0).

- 이전 피라미드로 신경 손상
- 인지 기능 저하
- 피부 손상
- 경직이 있는 상지의 이전 근골격계 기능 장애
- 경직이 있는 상지에 금속 잔여물 존재
- 경련 또는 정신 질환 존재
- 심각한 기형 또는 비만 (BMI >30kg/m2)
- 관상동맥 또는 기타 심혈관 질환 병력 (심부 정맥 혈전증, 폐색전증)

관절가동범위(ROM) 점수의 변화: 전자 각도계를 사용하여 경직으로 인한 팔꿈치 관절의 관절가동범위를 정확하게 측정합니다.
환자는 검사대에 누운 자세로, 경직이 있는 쪽이 검사자 쪽을 향하도록 배치됩니다.
그런 다음 검사자는 각도계를 해부학적 기준점에 정렬합니다. 각도계의 기저부는 상완골의 외측 상과 위에, 고정 축은 상완골을 따라, 가동 축은 요골을 따라 정렬합니다.
팔꿈치 굴곡 각도가 기록되며, 측정값을 145°(완전한 팔꿈치 굴곡의 관절가동범위)에서 빼서 팔꿈치 신전을 계산합니다.
팔꿈치 신전 점수의 감소는 경직 수준의 감소를 나타냅니다.

여섯 가지 영역으로 나뉩니다: i) 인지, ii) 이동성, iii) 자기 관리, iv) 대인 관계, v) 일상 생활 활동 및 vi) 참여.
각 항목은 1(어려움 없음)에서 5(극심한 어려움 또는 수행 불가)까지 평가됩니다.
