Een multicenter, open-label, eenarmig, voor en na overstaponderzoek om de werkzaamheid, veiligheid en verdraagbaarheid van Alemtuzumab te evalueren bij pediatrische patiënten met relapsing-remitting multiple sclerose (RRMS) met ziekteactiviteit op eerdere ziektemodificerende therapie (DMT)
Hoofddoel:
Om de werkzaamheid, veiligheid en verdraagbaarheid van alemtuzumab intraveneus (IV) bij pediatrische deelnemers van 10 tot minder dan (
Secundaire doelstelling:
Om de farmacokinetiek (PK), farmacodynamiek (PD), vorming van anti-drug antilichamen (ADA) en mogelijke effecten van alemtuzumab op andere ziektekenmerken van multiple sclerose (MS), zoals cognitie en kwaliteit van leven (QoL), te beoordelen.

Beschrijving
Inclusiecriteria :
- Deelnemers met RRMS in de leeftijd van 10 jaar tot
- Ondertekende schriftelijke geïnformeerde toestemming/instemming verkregen van de deelnemer en de wettelijke vertegenwoordiger van de deelnemer (ouder of voogd) volgens de lokale regelgeving.
- Uitgebreide Disability Status Scale (EDSS)-score van 0,0 tot 5,0 (inclusief) bij screening.
- Ten minste 2 geregistreerde MS-aanvallen en ten minste 1 MS-aanval (terugval) in het afgelopen jaar tijdens behandeling met een bèta-interferontherapie (IFNB) of glatirameeracetaat (GA) na ten minste 6 maanden op die therapie te zijn geweest en momenteel nog steeds dezelfde therapie volgen.
- Ten minste 1 van de volgende:
- >=1 nieuwe of vergrotende T2 hyperintense laesie of gadoliniumversterkende laesie tijdens dezelfde eerdere therapie (IFNB of GA), of
- Twee of meer terugvallen in het voorgaande jaar, of
- Minstens 2 MS DMT's geprobeerd.
Uitsluitingscriteria:
- Elke progressieve of niet-terugkerende vorm van MS.
- Voorwaarden/situaties zoals:
- Onmogelijkheid om aan specifieke protocolvereisten te voldoen.
- Huidige deelname aan een ander interventioneel klinisch onderzoek. Deelnemers die worden behandeld met een vergelijkingsmiddel dat is goedgekeurd voor opname bij screening (INF of GA) kunnen in aanmerking komen voor deze studie.
- Deelnemer is de onderzoeker of een subonderzoeker, onderzoeksassistent, apotheker, studiecoördinator, ander personeel of familielid daarvan dat direct betrokken is bij de uitvoering van het protocol.
- Niet-meewerkende deelnemer of een andere aandoening waardoor de deelnemer mogelijk niet voldoet aan de onderzoeksprocedures naar de mening van de onderzoeker.
- Geestelijke toestand waardoor de deelnemer of ouder/voogd de aard, reikwijdte en mogelijke gevolgen van het onderzoek niet kan begrijpen.
- Klinisch relevante cardiovasculaire, hepatische, neurologische, endocriene of andere belangrijke systemische ziekte die de implementatie van het protocol of de interpretatie van de onderzoeksresultaten bemoeilijkt of die de deelnemer in gevaar zou brengen door deel te nemen aan de studie naar de mening van de onderzoeker.
- Geschiedenis van drugs- of alcoholmisbruik.
- Geschiedenis van bekende positiviteit van het humaan immunodeficiëntievirus (HIV).
- Vrouwelijke deelnemers die zwanger zijn of borstvoeding geven of die van plan waren om tijdens het onderzoek zwanger te worden.
- Niet bereid om in te stemmen met het gebruik van een zeer effectieve anticonceptiemethode tijdens een kuur met alemtuzumab en gedurende 4 maanden na die kuur (alleen vruchtbare deelnemers).
- Vrouwelijke deelnemers die zijn begonnen met menstrueren (d.w.z. in de vruchtbare leeftijd zijn) en niet bereid of niet in staat zijn om getest te worden op zwangerschap.
- Eerdere behandeling met alemtuzumab.
- Behandeling met natalizumab, daclizumab, fingolimod, methotrexaat, azathioprine, cyclosporine of mycofenolaatmofetil in de laatste 6 maanden voorafgaand aan de screening, of vastgesteld door de behandelend arts dat er nog immuunsuppressie was door deze of andere MS-behandelingen.
- Behandeling met teriflunomide in de afgelopen 12 maanden, behalve als de deelnemer de aanbevolen eliminatieprocedure heeft ondergaan volgens de samenvatting van de productkenmerken (SPC).
- Eerdere behandeling met mitoxantron, cyclofosfamide, cladribine, rituximab, ocrelizumab, leflunomide of een andere cytotoxische therapie.
- Eerdere behandeling met eventuele onderzoeksmedicatie (geneesmiddel dat voor geen enkele dosis of voor enige indicatie was goedgekeurd). Het gebruik van een onderzoeksgeneesmiddel dat later is goedgekeurd en niet-standaard gebruik van een geregistreerd medicijn (bijvoorbeeld het gebruik van een andere dosis dan de dosis die wordt vermeld op de etikettering van het geregistreerde product of het gebruik van een goedgekeurde therapie voor een alternatieve indicatie) was geen uitsluiting. Voorafgaande behandeling met kruidengeneesmiddelen of voedingssupplementen was ook toegestaan.
- Intolerantie voor gepulseerde corticosteroïden, vooral een voorgeschiedenis van steroïde psychose.
- Geschiedenis van maligniteit.
- Eerder gedocumenteerde voorgeschiedenis van trombocytopenie of aantal bloedplaatjes bij screening < ondergrens van normaal (LLN).
- Elke handicap die is opgelopen door een trauma of een andere ziekte die, naar de mening van de onderzoeker, de evaluatie van de handicap als gevolg van MS zou kunnen verstoren.
- Deelnemers met bekende type 1 overgevoeligheid of anafylactische reacties op de werkzame stoffen of een van de hulpstoffen, of intolerantie voor aciclovir of het therapeutische equivalent ervan.
- Ernstige systemische ziekte of andere ziekte die, naar de mening van de onderzoeker, de veiligheid van de deelnemer in gevaar zou brengen of de interpretatie van onderzoeksresultaten zou verstoren, bijv. huidige maagzweerziekte of andere aandoeningen die vatbaar kunnen zijn voor bloedingen, immuuncytopenieën, reumatoïde artritis, systemische lupus erythematosus, andere bindweefselaandoeningen, vasculitis, inflammatoire darmziekte, ernstige psoriasis.
- Medische, psychiatrische, cognitieve of andere aandoeningen die, naar de mening van de onderzoeker, het vermogen van de deelnemer in gevaar brengen om de informatie van de deelnemer te begrijpen, om geïnformeerde toestemming te geven, om te voldoen aan het onderzoeksprotocol of om het onderzoek te voltooien.
- Ernstige psychiatrische stoornis die naar de mening van de onderzoeker niet voldoende onder controle is door behandeling.
- Epileptische aanvallen die niet voldoende onder controle zijn door behandeling.
- Aandoeningen gerelateerd aan magnetische resonantiebeeldvorming (MRI): aandoeningen die de MRI-opname en/of interpretatie van MRI-resultaten kunnen verstoren (bijv. claustrofobie, orthopedische implantaten/behandelingen, orthodontische behandelingen enz.).
- Bekende bloedingsstoornis (bijv. dysfibrinogenemie, factor IX-deficiëntie, hemofilie, de ziekte van Von Willebrand, gedissemineerde intravasculaire coagulatie, fibrinogeendeficiëntie, stollingsfactordeficiëntie).
- Voorgeschiedenis van invasieve schimmelinfecties.
- Actieve infectie, bijv. diepe weefselinfectie, die de onderzoeker als voldoende ernstig beschouwt om deelname aan het onderzoek uit te sluiten.
- Volgens de onderzoeker loopt de deelnemer een hoog risico op infectie (bijv. verblijfskatheter, dysfagie met aspiratie, decubituszweer, voorgeschiedenis van eerdere aspiratiepneumonie of recidiverende urineweginfectie).
Bovenstaande informatie is niet bedoeld om alle overwegingen te bevatten die relevant zijn voor de mogelijke deelname van een deelnemer aan een klinische proef.

Experimenteel: Alemtuzumab
- alemtuzumab - Dosis 1 (eerste kuur) van alemtuzumab zal 5 opeenvolgende dagen intraveneus worden toegediend, gevolgd door dosis 2 (tweede kuur) op 3 opeenvolgende dagen 12 maanden na de eerste kuur.
Premedicatie (methylprednisolon, prednisolon, H1-antagonist [antihistaminicum], H2-antagonist, paracetamol, aciclovir) zal voorafgaand aan toediening van alemtuzumab worden toegediend.

EDSS was an ordinal scale in half-point increments that qualified disability in participants with MS.
It consisted of 8 ordinal rating scales assessing seven functional systems (FS) (pyramidal, cerebellar, brainstem, sensory, bowel and bladder, visual, cerebral, and other).
Ambulation scoring was done to conclude evaluation.
EDSS steps 1.0 to 4.5 referred to participants with MS who were fully ambulatory, while EDSS steps 5.0 to 9.5 were defined by the impairment to ambulation.
Individual FS scores were then used in conjugation with ambulation score to obtain total EDSS score which ranged from 0 (normal neurological examination) to 10 (death due to MS) in half-point increments, where higher scores indicated worst outcomes.
Baseline for EDSS was defined as the last non-missing value prior to the first course of alemtuzumab.

Relapses were defined as new or recurrent neurological symptoms not associated with fever or infection, lasting at least 24 hours, and accompanied by new objective neurological findings upon examination by the neurologist and documented by the functional system scores.

Visual display of 6 simple figures arranged in 2x3 matrix on separate pages was shown to participants for 3 consecutive 10-second trials.
After each trial, participants drew as many designs as accurately as they could in correct location.
They were asked to reproduce designs in exact layout after 25-minute delay filled with other distractor tasks.
Forced-choice recognition trial was administered immediately after delayed memory trial.
Optional copy trial was included at end of test where participants were asked to copy figure display accurately.
Scoring of immediate, delayed recall and copy trials were based on accuracy of drawings and location of figures.
Score range 0 to 12 per trial; total score: 0 to 36 for all 3 trials.
Higher scores: better visuospatial memory.

The SDMT was used to assess cognitive impairment and involved a simple substitution task that normal children and adults could easily perform.
Using a reference key, the examinee/participant had 90 seconds to pair specific numbers with given geometric figures.
Responses were only in oral form for this study and administration time was just 5 minutes.
The number of corrected substitutions and number of completed responses were recorded.
The score for each was the number of correctly coded items ranging 0-110 in 90 seconds; higher scores indicated a better outcome.

The PedsQL™ measurement model was a modular approach to measure health-related quality of life (HRQOL) in healthy children and adolescents and those with acute and chronic health conditions.
It included a child and teen self-report, and parents' report of children and teen.
Each report consisted of 23 items and 4 scales (physical, emotional, social and school functioning).
Each item used a 5-point rating scale (from 0='it is never a problem' to 4='it is almost always a problem').
Items are reverse scored and linearly transformed to a 0 (almost always) to 100 (never) scale.
Total scores (0 to 100) for each report were mean of specific items where higher score indicated better HRQoL.

Pediatric NeuroQoL was measurement system that evaluated and monitored physical, mental and social effects experienced by children living with neurological conditions.
Physical effects (fatigue, pain) and mental effects (cognitive function, anxiety, depression) was assessed.
Each domain had 8 to 10 items and participants indicated how often they experienced feelings and circumstances related to each domain on scale 1 to 5 (1='never' to 5='almost always').
This scale was reversed for cognitive function (1='very much' to 5='not at all').
Higher values: worse outcome for fatigue, pain, anxiety, depression, and better cognitive function.
Raw scores were re-scaled to standardized scores with mean=50 and standard deviation=10.
T-score range: 20 to 80. T-score <30 indicated clinically significant impairment.

An adverse event (AE) was any untoward medical occurrence in a participant or clinical investigation participant administered a pharmaceutical product and which did not necessarily have a causal relationship with study treatment.
A serious AE (SAE) was any untoward medical occurrence that at any dose resulted in death, was life-threatening, required inpatient hospitalization or prolongation of existing hospitalization, resulted in persistent or significant disability/incapacity, was a congenital anomaly/birth defect or was a medically important event.
An AESI was an AE (serious or non-serious) of scientific and medical concern specific to the Sponsor's product or program, for which ongoing monitoring and immediate notification by the Investigator to the Sponsor was required.

Number of Participants With Infusion-Associated Reactions (IARs) in the Alemtuzumab Treatment Phase

An IAR was defined as any AE that occurred during alemtuzumab infusion or within the 24 hour post-infusion period.
