Patiënten met nierfalen die onderhoudsdialyse (HD) ondergaan, hebben doorgaans een zeer laag niveau van fysieke activiteit (PA), en dit gaat gepaard met een sterk verhoogde morbiditeit en mortaliteit.
Volgens onderzoeken is regelmatige lichaamsbeweging gunstig voor patiënten in alle stadia, en de huidige aanbevelingen voor de preventie en behandeling van bijwerkingen bij ZvH-patiënten, vooral bij ouderen, is regelmatige lichaamsbeweging omdat het de fysieke prestaties en de PA verbetert. Momenteel heeft, vanwege de onbekendheid van het personeel van dialysecentra en nefrologen met de voordelen van lichaamsbeweging, slechts 10% van de klinieken in de wereld een plan om te oefenen tijdens de ZvH. Daarom zijn oefenprogramma's voor thuisgebruik een geschikte optie voor patiënten om hun kosten te verlagen en het gemakkelijk te maken om ze na te leven.
Thuistraining heeft het potentieel om gebruik te maken van een training met een hoger volume en een hogere intensiteit als de activiteit wordt gecontroleerd. Veel van deze programma's zijn echter zonder toezicht en dit is een van de grootste nadelen van oefenprogramma's voor thuisgebruik. Gebrek aan voorkennis over de veiligheid en voordelen van oefenprogramma's, angst voor blessures en een gebrek aan interesse of motivatie vormen belemmeringen om thuis te oefenen.
Telerehabilitatie is revalidatiediensten die worden verleend aan patiënten van afgelegen locaties met behulp van informatie- en communicatietechnologieën. Verschillende onderzoeken hebben het gebruik van op de gezondheidszorg gebaseerde zelfmanagementinterventies bij patiënten met chronische nierziekten gerapporteerd.
Er is echter verder onderzoek nodig om beter te begrijpen in hoeverre deze technieken acceptabel, veilig en potentieel effectief zijn voor het ondersteunen van individuen die een ZvH-behandeling ondergaan, gezien hun unieke behoeften en risicoprofiel. Dit is onbekend. Onze studie probeert deze kloof te dichten door een pilot-evaluatie uit te voeren van synchrone thuisgebaseerde tele-oefeningsinterventie, ontworpen voor ZvH-patiënten.

Beschrijving
Inclusiecriteria:
- 18 jaar en ouder zijn
- Regelmatige HD in het centrum, 3 keer per week
- minimaal 1 jaar stabiele ZvH-geschiedenis hebben voltooid
- Zonder hartinfarct in de afgelopen 3 maanden
- toestemming van hun artsen
- beschikken over beslissingsbevoegdheid om hen in staat te stellen geïnformeerde toestemming te geven voor deelname aan het onderzoek
- toegang hebben tot een smart device (bijv. smartphone, laptop of tablet) en internettoegang hebben
Uitsluitingscriteria:
1) Instabiele hartstatus (angina pectoris, gedecompenseerd congestief hartfalen, ernstige arterioveneuze stenose, ongecontroleerde aritmieën, enz.) 2) Actieve infectie of acute medische ziekte 3) Hemodynamische instabiliteit 4) Labiele glykemische controle 5) Niet in staat om te oefenen (amputatie van de onderste ledematen met geen prothese) 6) ernstige pijn aan het bewegingsapparaat hebben in rust of bij minimale activiteit 7) Niet in staat zijn om zonder hulp te zitten, staan of lopen (loophulpmiddel zoals wandelstok of rollator toegestaan) 8) Kortademigheid hebben in rust of bij activiteiten van het dagelijks leven ( NYHAKlasse IV) 9) personen met deelname aan lichaamsbeweging ≥ 3 keer per week die ≥ 2 van de domeinen aanpakte 10) Instabiele ZvH-behandeling en veranderend (titrerend) medicatieregime 11) Overmatige interdialytische gewichtstoename (> 4 kg sinds de laatste ZvH of inspanning sessie) 12) Myocardinfarct in de afgelopen 3 maanden

Experimenteel: Oefengroep
De deelnemers aan de studiegroep krijgen op niet-dialysedagen thuis een online gepersonaliseerd oefenprogramma meegekregen.
Elke sessie duurt 40 tot 45 minuten gedurende 3 dagen per week gedurende 12 weken, in totaal 36 sessies.

The participants in the study group will be given an online personalized exercise program at home in non dialysis days.
Synchronous tele-exercise will be delivered using the free teleconference application (app) (Google Meets software).
The groups of tele-exercises will be private and the professional will send the link for each training session and will control the access of the participants.
Each session will be 40 to 45 min in duration for 3 days per week over 12 weeks, 36 sessions in total.

Geen tussenkomst: Control group
Participants allocated to the control group will serve as a wait-list usual-care comparator.
They will receive their usual nephrological care and will be instructed to maintain their usual treatment regimen and customary dietary and physical activity patterns without structured exercise during the 12-week control period.
After study completion, they will be offered access to the same tele-exercise program.

Adherence will be quantified using two metrics.
Overall session persistence will be calculated as the total number of sessions attended, including partially attended sessions, across all intervention participants divided by the total number of scheduled sessions across all intervention participants.
Within-session completeness will be calculated as the percentage of prescribed exercises completed per session, excluding preparation time and skipped items.

Change in daily physical activity level

To examine the effect of home-based synchronous tele-exercise on daily activity level by using Low Physical activity questionnaire. The questionnaire consists of 11 items that assess various parameters of physical activity within the past 7 days. These parameters include the amount of time spent walking around the neighborhood, for fitness or pleasure, and for transportation purposes, as well as the average duration of sedentary and sitting activities. The questionnaire also calculates the kilocalories expended during light, moderate, vigorous, and total physical activities. |

Change in Fried frailty phenotype score

To examine the effect of home-based synchronous tele-exercise on change in Fried frailty phenotype score. Frailty will be assessed using the Fried frailty phenotype, based on five criteria: unintentional weight loss of 10 pounds or more in the prior year by self-report; exhaustion based on responses to two questions about energy; low physical activity based on self-reported frequency of mildly, moderately, and very energetic activity, with low physical activity considered present when both moderately and very energetic activities were reported as occurring hardly ever or never; slow gait speed based on gender- and height-stratified cutoffs; and weak grip strength based on gender- and BMI-stratified cutoffs. |
