Het primaire doel is om te bepalen of behandeling met carbamazepine bij patiënten met een ernstige leveraandoening als gevolg van alfa-1-antitrypsinedeficiëntie leidt tot een significante vermindering van de hepatische accumulatie van ATZ.
De overige doelstellingen zijn:
Om te bepalen of behandeling met carbamazepine leverfibrose vermindert bij alfa-1-antitrypsine-deficiënte patiënten met ernstige leverziekte. Om te bepalen of behandeling met carbamazepine de portale druk verlaagt bij alfa-1-antitrypsine-deficiënte patiënten met ernstige leverziekte. Om te bepalen of behandeling met carbamazepine veilig is en wordt verdragen door patiënten met een ernstige leveraandoening veroorzaakt door alfa-1-deficiëntie. Om te bepalen of behandeling met carbamazepine leidt tot stabilisatie van de ernst van de ziekte zoals gemeten door de MELD-scores.

Beschrijving
Inclusiecriteria:
- Leeftijd groter dan of gelijk aan 14 jaar tot jonger dan of gelijk aan 80 jaar.
- Alfa-1-antitrypsinedeficiëntie bevestigd door ZZ- of SZ-fenotype en serumniveau
- < 83mg/dl.
- HVPG groter dan of gelijk aan 10 mmHg, tenzij collaterale vaten worden gevisualiseerd via transveneuze biopsie.
Uitsluitingscriteria:
- Child-Pugh-score groter dan of gelijk aan 12. Serum totaal bilirubine > 5 mg/dl. INR > 2,2.

Actieve vergelijker: Geneesmiddel-carbamazepine (Tegretol XR)
Eén arm krijgt geneesmiddel-carbamazepine (Tegretol XR). Alle proefpersonen hebben een ernstige leveraandoening als gevolg van alfa-1-antitrypsinedeficiëntie.

Om de kans op overgevoeligheidsreacties te verkleinen, wordt bij de proefpersonen gestart met 400 mg/dag verdeeld over 2 doses en wordt de dosis wekelijks verhoogd met 200 mg/dag totdat een stabiele therapeutische concentratie wordt bereikt met een dosis van niet meer dan 1200 mg/dag (of 1000 mg/dag). bij proefpersonen jonger dan 15 jaar).
De CBZ-tabletten worden ingekapseld..

Eén arm krijgt Carbamazepine (Tegretol-XR) placebo. Alle proefpersonen hebben een ernstige leveraandoening als gevolg van alfa-1-antitrypsinedeficiëntie.

Carbamazepine (Tegretol XR)Placebo - de proefpersonen zullen worden gestart met 400 mg/dag in 2 doses en de dosis zal wekelijks worden verhoogd met 200 mg/dag tot een dosis wordt bereikt die niet hoger is dan 1200 mg/dag (of 1000 mg/dag bij proefpersonen minder dan ouder dan 15 jaar).
De placebogroep krijgt ingekapselde tabletten zonder carbamazepine.

Het primaire resultaat zal zijn om het effect van carbamazepine op de ATZ-belasting in de lever te bepalen.

Het effect van carbamazepine op de ATZ-belasting in de lever zal worden gemeten aan de hand van het aantal hepatocyten met PAS+/diastase-resistente bolletjes en/of steady-state-niveaus van ATZ door middel van immunoblotanalyse.

Voor de secundaire uitkomsten zullen we het effect van behandeling met carbamazepine op leverfibrose bepalen op basis van Siriusroodkleuring en hydroxyprolineconcentratie en of behandeling met carbamazepine de portale druk verandert zoals bepaald door hepatische veneuze drukgradiënt.
