El objetivo principal es determinar si la terapia con carbamazepina en pacientes con enfermedad hepática grave debido a la deficiencia de alfa-1-antitripsina conduce a una reducción significativa en la acumulación hepática de ATZ.
Los otros objetivos son:
Determinar si el tratamiento con carbamazepina reduce la fibrosis hepática en pacientes con deficiencia de alfa-1-antitripsina con enfermedad hepática grave. Determinar si el tratamiento con carbamazepina reduce la presión portal en pacientes con deficiencia de alfa-1-antitripsina con enfermedad hepática grave. Determinar si el tratamiento con carbamazepina es seguro y tolerado por pacientes con enfermedad hepática grave causada por deficiencia de alfa-1. Determinar si el tratamiento con carbamazepina conduce a la estabilización de la gravedad de la enfermedad medida por las puntuaciones MELD.

Criterios de inclusión:
- Edad mayor o igual a 14 años a menor o igual a 80 años.
- Deficiencia de alfa-1-antitripsina confirmada por fenotipo ZZ o SZ y nivel sérico
- < 83 mg/dl.
- GPVH mayor o igual a 10 mmHg a menos que se visualicen vasos colaterales mediante biopsia transvenosa.
Criterio de exclusión:
- Child Pugh Score mayor o igual a 12. Bilirrubina sérica total > 5 mg/dl. IIN > 2,2.

Comparador activo: Droga-Carbamazepina (Tegretol XR)
Un brazo recibe el fármaco carbamazepina (Tegretol XR). Todos los sujetos tienen enfermedad hepática grave debido a la deficiencia de alfa-1-antitripsina.

Para reducir la probabilidad de reacciones de hipersensibilidad, los sujetos comenzarán con 400 mg/día en 2 dosis y la dosis se incrementará semanalmente en 200 mg/día hasta alcanzar una concentración terapéutica estable con una dosis que no exceda los 1200 mg/día (o 1000 mg/día). en sujetos menores de 15 años).
Las tabletas de CBZ serán encapsuladas.

Comparador de placebos: Droga-Carbamazepina (Tegretol XR) Placebo
Un brazo recibe placebo de carbamazepina (Tegretol-XR). Todos los sujetos tienen enfermedad hepática grave debido a la deficiencia de alfa-1-antitripsina.

Carbamazepina (Tegretol XR) Placebo: los sujetos comenzarán con 400 mg/día en 2 dosis y la dosis se aumentará semanalmente en 200 mg/día hasta alcanzar una dosis que no supere los 1200 mg/día (o 1000 mg/día en sujetos menos mayores de 15 años).
El grupo placebo recibirá tabletas encapsuladas sin Carbamazepina.

El resultado primario será determinar el efecto de la carbamazepina en la carga de ATZ hepática.
Periodo de tiempo: 52 semanas

El efecto de la carbamazepina sobre la carga hepática de ATZ se medirá mediante el número de hepatocitos con PAS+/glóbulos resistentes a la diastasa y/o niveles de ATZ en estado estacionario mediante análisis de inmunotransferencia.

Para los resultados secundarios, determinaremos el efecto del tratamiento con carbamazepina sobre la fibrosis hepática sobre la base de la tinción con rojo de sirio y la concentración de hidroxiprolina y si el tratamiento con carbamazepina cambia la presión portal según lo determinado por el gradiente de presión venosa hepática.
