Allogene menselijke cellen (hMSC) via intraveneuze toediening bij patiënten met licht astma (ASTEC)
Een fase I-onderzoek om de veiligheid, verdraagbaarheid en potentiële werkzaamheid van allogene menselijke mesenchymale stamcelinfusie bij patiënten met licht astma te evalueren
Studie Overzicht
Gedetailleerde beschrijving
Er zal een fase 1-onderzoek worden uitgevoerd om de veiligheid te testen van twee doses van beenmerg afgeleide MSC's (20.000.000 en 100.000.000) toegediend via perifere intraveneuze infusie.
Groep 1: 3 proefpersonen krijgen een enkele toediening van allogene hMSC's: 20 miljoen cellen toegediend via perifere intraveneuze infusie Groep 2: 3 proefpersonen krijgen een enkele toediening van allogene hMSC's: 100 miljoen cellen toegediend via perifere intraveneuze infusie Tussentijdse veiligheidsanalyse zal worden uitgevoerd vier weken nadat het eerste vak in elk cohort is ingeschreven. Voortdurende veiligheid en verdraagbaarheid met beoordeling van ongewenste voorvallen (AE's) zal bij elk bezoek worden beoordeeld. Werkzaamheidsparameters (longfunctietesten, diffusiecapaciteit (DLCO), longvolumes, 6-minuten looptest (6MWT) en dyspnoe/kwaliteit van leven [QOL] vragenlijsten) zullen elke 12 weken worden beoordeeld totdat de studie is voltooid. Bij elk bezoek worden klinische laboratoriumtests uitgevoerd om de veiligheid te beoordelen.

Beschrijving
Inclusiecriteria:
- Geef schriftelijke geïnformeerde toestemming
- tussen 18 en 65 jaar oud zijn op het moment van ondertekening van de geïnformeerde toestemming
- een klinische diagnose van astma hebben voorafgaand aan de screening in overeenstemming met de richtlijnen van de American Thoracic Society/European Respiratory Society
- ACQ boven 1,25
- een rookgeschiedenis hebben van in totaal minder dan 10 pakjaren en de afgelopen 12 maanden niet hebben gerookt
- Voer een positieve methacholine-uitdaging uit bij de screening en herhaal een positieve methacholine-uitdaging bij het basisbezoek (14 dagen later)
- Normale of milde obstructieve spirometrie hebben
- Een normale rechterhartfunctie hebben zoals gedocumenteerd door Doppler-echo of rechterhartkatheterisatie
- Als vrouw, chirurgisch steriel zijn, postmenopauzaal (meer dan 1 jaar) of anticonceptie met dubbele barrière toepassen
- Onderwerpen kunnen niet-medicamenteuze therapieën krijgen, waaronder zuurstofsuppletie van niet meer dan 2 l/minuut, en longrevalidatie
- Onderwerpen kunnen standaard astmamedicatie gebruiken, waaronder inhalatiecorticosteroïden - langwerkende bèta-agonisten in een dosis van niet meer dan 1 mg van een fluticason-equivalent
Uitsluitingscriteria:
- Heb een actieve infectie die niet wordt behandeld
- Niet in staat zijn om een van de beoordelingen uit te voeren die vereist zijn voor eindpuntanalyse.
- krijgt momenteel (of heeft binnen vier weken na screening) experimentele middelen gekregen voor de behandeling van astma
- actief op de lijst staat (of verwacht in de nabije toekomst te worden vermeld) voor transplantatie van een orgaan
- klinisch belangrijke abnormale laboratoriumwaarden voor screening hebben: bloedonderzoeken (Hematologie, Chemie, CBC inclusief aantal eosinofielen) resultaten die niet binnen de normale limieten liggen (volgens UMHC Laboratory Reference Ranges) Een ernstige comorbide ziekte hebben die, naar de mening van de onderzoeker, kan de veiligheid of therapietrouw van de patiënt in gevaar brengen of een succesvolle afronding van het onderzoek in de weg staan
- Bekende allergieën voor penicilline of streptomycine
- Wees een ontvanger van een orgaantransplantatie
- Een klinische voorgeschiedenis van maligniteit hebben binnen 5 jaar (d.w.z. patiënten met een eerdere maligniteit moeten gedurende 5 jaar ziektevrij zijn), behalve curatief behandeld basaalcelcarcinoom
- Een niet-pulmonale aandoening hebben die de levensduur beperkt tot minder dan een jaar.
- Een geschiedenis van drugs- of alcoholmisbruik hebben in de afgelopen 24 maanden.
- Wees serumpositief voor HIV, hepatitis BsAg of Viremia hepatitis C
- Momenteel deelnemen (of hebben deelgenomen in de afgelopen 30 dagen) aan een onderzoek naar een therapeutisch middel of apparaat.
- Overgevoelig zijn voor dimethylsulfoxide (DMSO)
- Een rustzuurstofverzadiging (SpO2) op kamerlucht hebben van meer dan 93% op zeeniveau of meer dan 88% op een hoogte boven 5.000 voet boven zeeniveau (1524 meter)
