Gedetailleerde beschrijving

Onderzoek KTE-C19-102 (NCT02601313) nam deel aan deelnemers met r/r MCL die zijn behandeld met maximaal 5 eerdere behandelingen, waaronder een Bruton's tyrosinekinaseremmer (BTKi) in cohort 1 en cohort 2. Echter, om te voldoen aan FDA Postmarketing Requirement Cohort 3 wordt toegevoegd aan de studie. Het omvat deelnemers met r/r MCL die zijn behandeld met maximaal 5 eerdere regimes, maar die niet eerder zijn behandeld met een BTKi.

De primaire analyse in Cohort 1 en Cohort 2 is al afgerond. Gegevens voor Cohort 3 zullen afzonderlijk worden geanalyseerd. Deze aparte inschrijving is dus alleen voor Cohort 3.

Belangrijkste opnamecriteria:

  • Tot 5 eerdere regimes voor MCL. Voorafgaande therapie moet anthracycline- of bendamustine-bevattende chemotherapie en anti-CD20 monoklonale antilichaamtherapie hebben omvat. Individuen mogen niet eerder met een BTKi zijn behandeld.
  • Minimaal 1 meetbare laesie
  • Aantal bloedplaatjes ≥ 75.000/uL
  • Creatinineklaring (zoals geschat door Cockcroft Gault) ≥ tot 60 cc/min
  • Cardiale ejectiefractie ≥ 50%, geen bewijs van pericardiale effusie zoals bepaald door een echocardiogram (ECHO) of multigated acquisitie (MUGA), en geen klinisch significante elektrocardiogram (ECG) bevindingen
  • Basislijn zuurstofverzadiging > 92% op kamerlucht

Belangrijkste uitsluitingscriteria:

  • Bekende voorgeschiedenis van infectie met humaan immunodeficiëntievirus (HIV) of hepatitis B (HBsAG-positief) of hepatitis C-virus (anti-HCV-positief). Personen met een voorgeschiedenis van hepatitis-infectie moeten hun infectie hebben gewist zoals bepaald door standaard serologische en genetische testen
  • Geschiedenis van een epileptische aandoening, cerebrovasculaire ischemie / bloeding, dementie, cerebellaire ziekte, hersenoedeem, posterieur reversibel encefalopathiesyndroom of een auto-immuunziekte met betrokkenheid van het centrale zenuwstelsel (CZS)
  • Aanwezigheid van een schimmel-, bacteriële, virale of andere infectie die niet onder controle is of IV-antimicrobiële middelen vereist voor behandeling

Experimenteel: Brexucabtagene autoleucel (KTE-X19)

Participants with relapsed or refractory (r/r) mantle cell lymphoma (MCL) who have been treated with up to 5 prior regimens but have not received prior therapy with a Bruton's tyrosine kinase inhibitor (BTKi) will receive the following treatment during the study:

  • A conditioning chemotherapy regimen of fludarabine 30 mg/m^2/day and cyclophosphamide 500 mg/m^2/day for 3 days (Day -5 to Day -3).
  • A single infusion of brexucabtagene autoleucel at a target dose of 2×10^6 anti-cluster of differentiation 19 (CD19) chimeric antigen receptor (CAR) transduced autologous T cells/kg, with a maximum flat dose of 2 x 10^8 anti-CD19 CAR T cells for participants > 100 kg on Day 0.

EORTC QLQ-C30 included functional scales (physical, role, cognitive, emotional, and social), global health status/quality of life (QoL) scale, symptom scales (fatigue, pain, nausea/vomiting), and single items scales (dyspnoea, appetite loss, insomnia, constipation/diarrhea and financial difficulties). Most questions use 4 point scale (1 'Not at all' to 4 'Very much'; 2 questions used 7-point scale (1 'very poor' to 7 'Excellent'). Scores are averaged, transformed to 0-100 scale. Higher scores for functional scales and for the global health status/QoL scale indicate a higher level of functioning and a better health-related QoL, whereas higher scores in symptom scales represent a higher level of symptoms. Deterioration of score was defined as worsened by at least 1 level from screening. Participants with answer "Yes" to the EORTC functional scale questionnaire were reported.
