Dit is een multicenter, open-label fase 2-onderzoek ter evaluatie van de veiligheid, werkzaamheid, farmacokinetiek en farmacodynamiek van eenmaal daags oraal toegediende bomedemstat bij patiënten met polycythaemia vera.

Patiënten krijgen gedurende 36 weken dosering en kunnen daarna in aanmerking komen voor aanvullende behandeling.

Patiënten zullen tijdens het onderzoek nauwlettend worden gevolgd voor zowel bijwerkingen door frequente monitoring van klinische tekenen en symptomen als veiligheidslaboratoria. Werkzaamheid en farmacodynamische effecten zullen nauwlettend worden gevolgd door frequente hematologische beoordelingen van perifeer bloed. Tijdens de dosering kunnen indien nodig transfusies of aderlaten worden toegediend in overeenstemming met de standaard institutionele richtlijnen.

Om de veiligheid te waarborgen, zal een veiligheidsadviesraad periodieke beoordelingen van veiligheidsparameters en farmacodynamische markers uitvoeren.

Inclusiecriteria:

  • Diagnose van polycythaemia vera volgens diagnostische criteria van de Wereldgezondheidsorganisatie (WHO) voor myeloproliferatieve neoplasmata
  • Beenmergfibrosescore van Graad 0 of Graad 1
  • Patiënten bij wie ten minste één standaard cytoreductieve therapie niet heeft gefaald om de hematocriet te verlagen
  • Aantal bloedplaatjes ≥250 x 10ˆ9/L
  • Absoluut aantal neutrofielen (ANC) ≥1,5 x 10ˆ9/L
  • Levensverwachting >36 weken.
  • Moet eerdere cytoreductieve therapie gedurende 2 weken (4 weken voor interferon) hebben gestaakt voorafgaand aan de start van het onderzoeksgeneesmiddel.

Uitsluitingscriteria:

  • Eastern Cooperative Oncology Group (ECOG) prestatiestatus van 3 of hoger
  • Onopgeloste behandelingsgerelateerde toxiciteiten van eerdere therapieën (tenzij opgelost tot ≤ Graad 1).
  • Ongecontroleerde actieve infectie.

Baseline data was defined as the data most recently collected prior to the first dose. A Clopper-Pearson (exact binomial) two-sided 95% confidence interval for the percentage of responders was presented. Thrombotic events are defined as: new or recurrent acute myocardial infarction; unstable angina; stroke; transient ischemic attack (TIA); deep venous thrombosis (DVT); pulmonary embolism (PE); thrombotic digital ischemia; other thrombotic events such as peripheral limb ischemia or Budd-Chiari syndrome that are assessed to be due to underlying polycythemia vera (PV); other vascular occlusive events such as symptoms of cardiac, abdominal or peripheral limb ischemia supported by objective evidence of vessel disease and/or ischemia. Baseline data was defined as the data most recently collected prior to the first dose. The number of participants with thrombotic events are reported.
