Primaire preventieve ventriculaire tachycardie-ablatie bij patiënten met een hoog risico die profylactisch een implanteerbare cardioverter-defibrillator krijgen (PRIMARY PREVENT-VT TRIAL)
Deze prospectieve, multicentrische, open-label, gerandomiseerde gecontroleerde studie vergelijkt twee behandelstrategieën bij patiënten met een hoog risico op ischemische cardiomyopathie (ICM) die verwezen zijn voor primaire ICD-implantatie.
Deelnemers worden gerandomiseerd om ofwel profylactische VT-ablatie te krijgen binnen drie maanden na ICD-implantatie, ofwel voortgezette medische behandeling.
Het primaire doel is het beoordelen van de effectiviteit van preventieve VT-ablatie versus medische behandeling bij het verminderen van de last van VT-aritmie.

Deze studie heeft tot doel de impact van profylactische kathetergebaseerde ventriculaire tachycardie (VT)-ablatie te beoordelen in vergelijking met medische behandeling bij hoogrisicopatiënten met ischemische cardiomyopathie (ICM) die een primaire implanteerbare cardioverter-defibrillator (ICD)-implantatie ondergaan.
De primaire uitkomstmaat is de last van VT-aritmieën (VTA), gedefinieerd als het totale aantal VT-gebeurtenissen, één jaar na ICD-implantatie.
De hypothese suggereert dat het gebruik van VT-ablatie met elektroanatomische mapping met hoge dichtheid (EnsiteTM) zowel de behandelde als de gecontroleerde VT-episodes aanzienlijk zal verminderen.
Het onderzoek zal 62 deelnemers willekeurig verdelen in een verhouding van 1:1 over 20 ervaren locaties in Europa.

Inclusiecriteria:
- Leeftijd ≥ 18 jaar (geen leeftijdsgrens)
- Bereid en in staat om geïnformeerde toestemming te geven
- Kandidaat voor een primaire preventie-ICD/CRT-D volgens de ESC-richtlijnen17
- Ischemische cardiomyopathie (ICM) met linkerventrikelejectiefractie (LVEF) ≤ 35%
- Bereid en in staat om een ICD te ontvangen
- Bereid en in staat om kathetergebaseerde VT-ablatie te ondergaan
Een of meer van de volgende:
- MADIT-ICD voordeelscore >75
- Geschiedenis van niet-aanhoudende ventriculaire tachycardie (NSVT)
- Meerdere premature ventriculaire complexen (PVC's ≥10%)
Uitsluitingscriteria:
- Bestaande ICD of CRT-D
- Geschiedenis van aanhoudende VT of VF
- Chronisch nierfalen waarvoor hemodialyse nodig is
- Coronaire revascularisatie in de afgelopen 3 maanden
- ST-Elevatie MI in de afgelopen 3 maanden
- Deelname aan gelijktijdige klinische onderzoeken zonder toestemming van het Coördinatiecentrum (observatieregisters zijn toegestaan met toestemming)
- Momenteel zwanger of van plan om in de nabije toekomst zwanger te worden
- Onvermogen of onwil om zich aan het onderzoeksprotocol te houden
- Onvermogen of onwil om geïnformeerde toestemming te geven voor deelname

Proefpersonen ontvangen kathetergebaseerde ventriculaire tachycardie (VT) ablatie binnen 3 maanden na primaire ICD-implantatie.

Profylactische kathetergebaseerde VT-ablatie verwijst naar een behandelingsstrategie waarbij deelnemers een procedure ondergaan die bekend staat als kathetergebaseerde ventriculaire tachycardie (VT) ablatie, kort nadat ze hun primaire implanteerbare cardioverter-defibrillator (ICD) hebben ontvangen.

Impact van preventieve VT-ablatie op het gebruik van gezondheidszorg, beoordeeld aan de hand van het gemiddelde aantal voorvallen per arm.
Tijdsspanne: Eén jaar na ICD-implantatie.

Beoordelen of preventieve VT-ablatie in vergelijking met medisch management het gebruik van gezondheidszorgmiddelen vermindert bij hoogrisicopatiënten met ICM na primaire ICD-implantatie.
Het gebruik van gezondheidszorgmiddelen zal worden gekwantificeerd door het gemiddelde te nemen van geplande en ongeplande bezoeken aan de spreekkamer, bezoeken aan spoedeisende hulp of spoedeisende hulp, ziekenhuisopnames, herhaalde ablaties of sterfte per arm.
Gegevens over het gebruik van gezondheidszorg zullen worden verzameld bij alle vervolgbezoeken (maanden 3, 6 en 9) en aan het einde van het studiebezoek.
De analyse zal worden uitgevoerd met behulp van een t-test ter vergelijking.

Impact van preventieve VT-ablatie op de kwaliteit van leven met behulp van de Kansas City Cardiomyopathy Questionnaire (KCCQ).
Tijdsspanne: Eén jaar na ICD-implantatie.

Beoordelen of preventieve VT-ablatie in vergelijking met medisch management de kwaliteit van leven verbetert bij hoogrisicopatiënten met ICM na primaire ICD-implantatie, met behulp van de Kansas City Cardiomyopathy Questionnaire (KCCQ) om de kwaliteit van leven te meten.
Scores op de KCCQ variëren van 0 tot 100, waarbij hogere scores een betere kwaliteit van leven aangeven.
Het verzamelen van gegevens met de KCCQ zal plaatsvinden tijdens de baseline- en eindonderzoeksbezoeken.
Verschillen in kwaliteit van leven tussen de onderzoeksgroepen zullen worden geanalyseerd met behulp van de t-test op basis van KCCQ-antwoorden.
