Varighet av immunkontrollpunktterapi ved lokalt avansert eller metastatisk urothelial karsinom: en randomisert fase 3-ikke-inferioritetsstudie (IMAGINE)

Denne fase III-studien sammenligner overlevelse hos urotelkreftpasienter som stopper behandling med immunkontrollpunkthemmere etter å ha blitt behandlet i omtrent ett år med de pasientene som fortsetter behandling med immunkontrollpunkthemmere. Immunterapi med monoklonale antistoffer, som avelumab, durvalumab, pembrolizumab, atezolizumab og nivolumab, kan hjelpe kroppens immunsystem til å angripe kreften, og kan forstyrre tumorcellenes evne til å vokse og spre seg. Å stoppe immunkontrollpunkthemmere tidlig kan fortsatt få svulsten til å krympe og pasienter kan ha lignende overlevelsesrater som pasientene som fortsetter behandlingen. II. For å sammenligne PFS etter immunrelaterte (ir)RECIST-kriterier. III. For å bestemme behandlingsfritt intervall (TFI) etter seponering av immunkontrollpunkthemmer (ICI). (Arm B) IV. For å bestemme responsraten etter RECIST 1.1-kriterier etter ICI-rechallenge. (Arm B) V. Å vurdere uønskede hendelser i hver studiearm ved hjelp av Common Terminology Criteria for Adverse Events (CTCAE) 5.0.

OVERSIGT: Pasienten blir randomisert til 1 av 2 armer.

ARM A (FORTSETTELSE AV ICI-BEHANDLING): Pasienter får enten pembrolizumab intravenøst ​​(IV) over 30 minutter på dag 1, nivolumab IV over 30 minutter på dag 1 og 15, atezolizumab IV over 30-60 minutter på dag 1, durvalumab IV over 60 minutter på dag 1 og 15, eller avelumab IV over 60 minutter på dag 1 og 15. Sykluser gjentas hver 21. eller 42. dag for pembrolizumab, hver 21. dag for atezolizumab og 28. dag for nivolumab, durvalumab og avelumab i fravær av sykdomsprogresjon eller uakseptabel toksisitet.

Etter avsluttet studiebehandling, oppstart av ny behandling eller tilbaketrukket samtykke, følges pasientene opp ved 4 uker, og deretter hver 6. måned i 5 år etter registrering.

ARM B (AVSLUTNING AV ICI-BEHANDLING): Pasienter som får ICI-behandling vil avbryte ICI-behandling innen 1 sykluslengde etter randomisering. Sykluslengden bestemmes av ICI-kuren pasienten får ved randomisering. Ved sykdomsprogresjon kan pasienter starte den samme ICI-behandlingen de fikk ved randomisering etter legens skjønn.

Etter avsluttet studiebehandling følges pasientene opp etter 4 uker, og deretter hver 6. måned i 5 år etter registrering.

Progressionsfri overlevelse (PFS) er tiden pasienter er i live uten sykdomsprogresjon. Progresjon vil bli vurdert ved bruk av Response Evaluation Criteria in Solid Tumors (RECIST) 1.1-kriterier. Stratifiserte Cox-modeller vil bli brukt for å sammenligne resultatene mellom de to behandlingsgruppene.

Andel med immunrelatert progresjonsfri overlevelse (iPFS) etter 12 måneder
Tidsramme: 12 måneder
Immunrelatert progressjonsfri overlevelse (iPFS) er tiden pasienter lever uten immunrelatert progresjon. Progresjon vil bli vurdert ved hjelp av immunrelaterte (ir)RECIST-kriterier. Stratifiserte Cox-modeller vil bli brukt til å sammenligne resultatene mellom de to behandlingsgruppene.

Behandlingsfri intervall er tiden pasienter er uten behandling. En stratifisert Cox-modell vil bli brukt for å evaluere dette resultatet.

Tidsramme: Opptil 5 år
Svarprosenten er prosentandelen av pasienter med respons etter gjenoppstart av immunhemmerterapi (ICI) etter progresjon etter registrering. Svarprosenten vil bli vurdert ved bruk av RECIST 1.1. En chi-kvadrat-test (eller Fishers eksakte test) vil bli brukt for å avgjøre om det er en sammenheng mellom den beste svulstresponsen før deltakelse i forsøket og den som oppnås ved ICI-gjenutfordring.

Antall pasienter som opplever bivirkninger (AEs) av grad 3 eller høyere
Tidsramme: 2,5 år
Bivirkninger (AEs) er helseplager som oppstår under behandling.
AEs vil bli vurdert ved hjelp av Common Terminology Criteria for Adverse Events versjon 5.0.
