Prewencja pierwotna zdarzeń sercowo-naczyniowych jest często bardziej intensywna u osób z grupy mniejszego ryzyka i odwrotnie (tzw. i ich wykonalności w codziennym życiu każdej osoby. Wytyczne praktyki klinicznej zalecają, aby klinicyści i pacjenci współpracowali w celu opracowania skutecznego i wykonalnego planu profilaktyki, który jest zgodny z ryzykiem sercowo-naczyniowym i świadomymi preferencjami każdej osoby, w procesie zwanym wspólnym podejmowaniem decyzji (SDM). Pomimo dostępności innowacyjnego i skutecznego narzędzia, które szacuje ryzyko sercowo-naczyniowe, pokazuje wpływ i cechy dostępnego stylu życia i farmakologicznych interwencji profilaktycznych, a tym samym może ułatwić dyskusje dotyczące leczenia sercowo-naczyniowego między klinicystami a pacjentami, ten typ SDM nie występuje rutynowo w praktyce.
Wyzwanie polega zatem na określeniu strategii zwiększających adopcję tego typu SDM w rzeczywistych praktykach klinicznych. To 4-letnie badanie – zaproponowane przez multidyscyplinarny zespół z doświadczeniem w kardiologii prewencyjnej, SDM i naukach wdrożeniowych – ma na celu zintegrowanie narzędzia SDM (narzędzie CV Prevention Choice) z praktykami podstawowej opieki zdrowotnej w trzech różnych systemach opieki zdrowotnej w USA. i przestudiuj zarówno narzędzie, jak i dostosowane strategie, które sprzyjają jego przyjęciu i rutynowemu stosowaniu. W badaniu zostanie wykorzystana metoda mieszana, hybrydowa implementacja-efektywność (Typ III) klin krokowy z randomizacją, aby określić:
- Skuteczność wdrożenia (Cel 1) poprzez ocenę ustawień (w tym lokalnych przepływów pracy i polityk), w których wdrażane jest narzędzie CV Prevention Choice oraz zaangażowanie użytkowników w strategie wdrażania; wyniki wdrożenia (np. zasięg, adopcja) związane z tymi strategiami; oraz w jaki sposób wdrożenie sprzyja rutynowemu przyjmowaniu narzędzia SDM i narzędzia CV Prevention Choice w praktykach podstawowej opieki zdrowotnej oraz
- Skuteczność SDM (Cel 2) szacowana na podstawie stopnia, w jakim indywidualne plany profilaktyki CV są wykonalne i zgodne z szacowanym ryzykiem CV i preferencjami każdej osoby.
Badacze stawiają hipotezę, że wysiłki mające na celu ocenę lokalnych potrzeb i wykorzystanie ich do opracowania dostosowanych podejść do wdrażania sprzyjają szerszemu przyjęciu SDM w praktyce. Ponadto stawiają hipotezę, że indywidualne plany opieki profilaktycznej będą zgodne z szacowanym ryzykiem, gdy klinicyści zastosują narzędzie SDM. Kryteria przyjęcia:
- Klinicyści uczestniczący: wszyscy klinicyści, którzy są powiązani z uczestniczącymi praktykami podstawowej opieki zdrowotnej i opiekują się dorosłymi pacjentami kwalifikującymi się do profilaktyki sercowo-naczyniowej, zostaną zaproszeni do udziału.
- Pacjenci-uczestnicy: Dorośli pacjenci (w wieku 40-75 lat) z cukrzycą lub bez cukrzycy, którzy nie doświadczyli zdarzenia klinicznego związanego z zakrzepicą na tle miażdżycowym i otrzymują opiekę profilaktyczną w uczestniczącej przychodni podstawowej opieki zdrowotnej, będą kwalifikować się do udziału.
Kryteria wyłączenia:
- Osoby, które nie mówią po angielsku lub mają jakiekolwiek deficyty poznawcze, które mogłyby wpłynąć na ich zdolność wyrażenia zgody na udział w badaniu, nie zostaną zaproszone do udziału.

A sample of clinical encounters were audio-video recorded, reviewed by trained study staff, and scored using a fidelity checklist with 10 items indicating shared decision making behaviors.
Each item was scored on a on a scale from -1=Behavior was undermined by comment or action to 4=The behavior is exhibited to a very high standard.
An overall average score ranging from -1 to 4 was created by averaging the 10 item scores.
A higher mean score indicates greater fidelity to the core components of shared decision making.

Scores are summed and transformed to a 0-100 scale, with higher scores indicating greater perceived involvement in decision-making.

Effectiveness: The Predicted Marginal Probability of Statin Use is Concordant With Estimated Cardiovascular Risk

Patient risk-concordance was assessed using all of the patient's encounter level data.
Statin prescription status was captured in the EHR up to 30 days after the encounter (i.e., the patient-clinician clinical visit) and their estimated 10-year risk of developing a first atherosclerotic cardiovascular disease (ASCVD) event, calculated using the Pooled Cohort Equation and data from the EHR including: age, race, total cholesterol, HDL cholesterol, systolic blood pressure, smoking status, diabetes status, and whether the individual is receiving treatment for high blood pressure (if systolic blood pressure is greater than 120 mmHg).
Risk concordance was defined as having a statin prescription in groups where ASCVD risk was > or equal to 7.5% and not having a statin prescription if it was < 7.5%.
The least square means, using the patient's encounter level data, provides a marginal probability of the proportion of patients that would have a statin prescription within the risk*arm/group.
