Wpływ testosteronu i suplementacji żywieniowej, osobno i łącznie, na niekorzystne skutki niedożywienia u osób starszych
Celem tego badania jest określenie, jaki wpływ ma roczna terapia testosteronem i suplementem diety, pojedynczo i łącznie, na niekorzystne skutki niedożywienia u starszych mężczyzn i kobiet mieszkających w społeczności.

Szczegółowy opis
Stawiamy hipotezę, że doustna terapia androgenami i suplementacja żywieniowa będą miały dodatkowe korzystne efekty u starszych niedożywionych osób żyjących w społeczności; nastąpi zmniejszenie liczby hospitalizacji, upadków i utraty masy ciała w połączeniu z poprawą parametrów funkcjonalnych i wzrostem beztłuszczowej masy ciała.
Niniejsze badanie ma na celu zbadanie wpływu 12-miesięcznej suplementacji pokarmu doustnego i terapii androgenowej, osobno i łącznie, na (i) częstotliwość i długość hospitalizacji; (ii) stan funkcjonalny; (iii) skład ciała i waga; (iv) warunki mieszkaniowe; (v) śmiertelność wśród niedożywionych mężczyzn i kobiet w wieku 65 lat i starszych.

Kryteria przyjęcia:
- Mężczyźni i kobiety w wieku 65 lat lub starsi, którzy są niedożywieni, jak zdefiniowano na podstawie wyniku Mini Nutritional Assessment (MNA) <24 ORAZ 1 lub więcej z następujących kryteriów: *wskaźnik masy ciała (BMI) poniżej 22 kg/m2 ; *utrata masy ciała > 7,5% w ciągu 3 miesięcy przed włączeniem do badania
- Samodzielne życie w społeczności (nie w szpitalu, domu opieki lub hostelu)
- Zrozumieć i podpisać dokument świadomej zgody, być w stanie komunikować się z badaczem oraz zrozumieć i przestrzegać wymagań badania.
- Kobiety stosujące estrogen lub inną hormonalną terapię zastępczą (HTZ) mogą wziąć udział (patrz kryteria wykluczenia dotyczące wyjątków), podobnie jak kobiety, które nie stosują HTZ. Jeśli kobieta stosuje HTZ, musi przyjmować stabilną dawkę przez co najmniej 3 miesiące przed włączeniem do badania. Jeśli nie stosowała HTZ, nie mogła jej stosować przez co najmniej 3 miesiące przed włączeniem do badania.
Kryteria wyłączenia:
- Otępienie na co wskazuje wynik < 23 w skali Folstein's Mini Mental State Examination (MMSE)
- Podwyższony poziom hematokrytu (HCT) (>50%)
- Przebyty lub obecny rak gruczołu krokowego u mężczyzn (stężenie swoistego antygenu prostaty (PSA) [> prawidłowy zakres związany z wiekiem i/lub nieregularna prostata w badaniu prostaty]) lub rak piersi.
- Istniejące wcześniej oznaki lub objawy androgenne u kobiet budzących niepokój osoby badanej lub badacza (niski głos, hirsutyzm, trądzik, androgenne wypadanie włosów).
- Depresja (wynik Yesavage Geriatric Depression Scale (GDS) > 11)
- Niemożność uczestniczenia w badaniu DEXA lub spełnienia innych wymagań badania
- Znacząca niewydolność serca (klasa III i wyższa wg NYHA)
- Znacząco nieprawidłowe wyniki testów czynności wątroby (AlAT, GGT, bilirubina lub ALP ponad 2 razy powyżej górnej granicy normy)
- Zespół nerczycowy; 24-godzinne wydalanie białka z moczem > 3 gramy (jeśli wyjściowa analiza moczu wykaże białkomocz > 1+, zostanie wykonane oznaczenie ilościowe) ORAZ/LUB obliczony klirens kreatyniny (za pomocą równania Baracskaya i Jarjoury dla pacjentów w podeszłym wieku [klirens Cr = 4,4/kreatynina w surowicy (mmol /L) + (88-wiek)](63) < 30 ml/min) I/LUB stężenie kreatyniny w surowicy > 0,2 mmol/l.
- Po amputacji (niedokładne oszacowania wskaźnika masy ciała, klirensu kreatyniny i składu ciała).
- Każda choroba, która w opinii badacza może doprowadzić do śmierci w ciągu jednego roku
- Testosteron lub inna terapia androgenowa (w tym DHEA i tibolon) w ciągu czterech miesięcy przed rozpoczęciem badania.
- Leki zawierające cyklosporynę lub barbiturany
