Descrição
Critério de inclusão:
- Fornecimento de consentimento informado por escrito (pelo paciente ou pessoa designada de acordo com os regulamentos locais) antes de qualquer procedimento específico do estudo.
- Maiores de 18 anos, masculino ou feminino.
Doença arterial coronariana (DAC) estável documentada preenchendo qualquer um dos seguintes:
- História de angina pectoris estável com evidência angiográfica de DAC (estenose de diâmetro ≥ 50%) nas principais artérias, ou seja, tronco principal esquerdo, descendente anterior esquerdo, circunflexa esquerda e artérias coronárias direitas.
- História de infarto do miocárdio (IM) prévio
- História de revascularização coronária, ou seja, intervenção coronária percutânea (ICP) ou revascularização miocárdica (CABG), não incluindo a ICP eletiva durante a internação índice
- História documentada de diabetes mellitus tipo 2.
Pelo menos 24 horas após, mas dentro de 14 dias de ICP eletiva angiograficamente bem-sucedida sem complicações.
- Estenose do diâmetro residual pós-procedimento das lesões tratadas < 20% em pacientes com implante de stent ou < 50% naqueles com angioplastia por balão
- Fluxo de grau 3 pós-trombólise no infarto do miocárdio (TIMI) em vasos tratados
- Teste de troponina cardíaca negativo antes da ICP eletiva índice.
- Tomar clopidogrel 75 mg por dia por pelo menos 7 dias ou tomar clopidogrel 75 mg por dia por menos de 7 dias, mas com dose de ataque de 300 a 600 mg de clopidogrel antes da ICP.
- Tomar ácido acetilsalicílico (AAS) 100 mg por dia durante pelo menos 7 dias ou tomar AAS 100 mg por dia durante menos de 7 dias, mas com dose de carga de 300 mg de AAS antes da ICP.
Mulheres com potencial para engravidar (ou seja, mulheres que não estão na pós-menopausa ou cirurgicamente estéreis) devem:
- ter um teste de gravidez de urina ou sangue negativo na inscrição e antes da randomização;
- atualmente usando um contraceptivo hormonal e concorda em continuar a usá-lo, além de usar contracepção local de dupla barreira (ou seja, dispositivo intra-uterino mais espermicida e preservativo para parceiro masculino) desde a triagem até a conclusão do estudo.
Critério de exclusão:
- Pacientes que tiveram síndrome coronariana aguda (SCA) dentro de 12 meses após a triagem.
- Ocorrência de infarto do miocárdio (IM) relacionado à ICP eletiva indexada (IM tipo 4a) ou infarto do miocárdio relacionado à trombose de stent (IM tipo 4b) de acordo com a Terceira Definição Universal de Infarto do Miocárdio.
- Uso de agentes antitrombóticos parenterais, por exemplo, inibidores da glicoproteína IIb/IIIa (GPIs), bivalirudina, heparina não fracionada, enoxaparina ou fondaparinux dentro de 24 horas após a triagem.
- Uso de qualquer agente antitrombótico oral, exceto Clopidogrel e AAS, até 30 dias após a triagem.
- Quaisquer outras indicações (por exemplo, fibrilação atrial, válvula cardíaca protética, tromboembolismo venoso, trombose ventricular etc.)
- Terapia concomitante com inibidores moderados ou fortes do citocromo P-450 (CYP) 3A, substratos do CYP 3A com índice terapêutico estreito ou indutores fortes do CYP 3A durante o período do estudo.
- Terapia concomitante com inibidores moderados ou fortes do CYP 2C19, substratos do CYP 2C19 com índice terapêutico estreito ou indutores fortes do CYP 2C19 durante o período do estudo.
Aumento do risco de sangramento, incluindo:
- sangramento gastrointestinal (GI) recente (dentro de 30 dias após a triagem);
- qualquer história de sangramento intracraniano, intraocular, retroperitoneal ou espinhal;
- trauma grave recente (até 30 dias após a triagem) ou cirurgia importante;
- hipertensão sustentada não controlada (pressão arterial sistólica [PAS] > 180 mmHg ou pressão arterial diastólica [PAD] > 100 mmHg);
- história de distúrbios hemorrágicos que podem aumentar o risco de sangramento, por exemplo, hemofilia, doença de von Willebrand;
- incapacidade de descontinuar a terapia concomitante necessária com anti-inflamatórios não esteróides não seletivos (AINEs) na triagem;
- contagem de plaquetas inferior a 100.000/mm3 ou hemoglobina < 10 g/dL.
- Contra-indicação ou outro motivo pelo qual AAS, Clopidogrel ou Ticagrelor não devem ser administrados (por exemplo, hipersensibilidade, sangramento ativo [incluindo sangramento patológico ativo], qualquer tendência a sangramento [defeitos de coagulação], insuficiência hepática moderada e grave, risco de bradicardia, obstrução pulmonar crônica doença, asma crônica ou ativa, hiperuricemia, gota, etc.).
- História de intolerância ao AAS, Clopidogrel ou Ticagrelor.
- Pacientes agendados para CABG durante o período do estudo.
- Paciente requer diálise ou tem depuração de creatinina (Clcr) < 30 mL/min, conforme calculado pela equação de Cockcroft-Gault: Clcr = (140 - Idade) × WT / (72 × Scr) (× 0,85 para mulheres), onde WT é peso em kg, Scr é a creatinina sérica em mg/dL.
- Quaisquer condições instáveis agudas ou crônicas nos últimos 30 dias ou outras condições que, na opinião do investigador, possam colocar o paciente em risco ou influenciar o resultado do estudo (por exemplo, câncer ativo, risco de não adesão, risco por ter perdido seguimento).
- Participação em outro estudo experimental de medicamento ou dispositivo dentro de 30 dias após a triagem.
- Envolvimento no planejamento e condução do estudo (aplica-se a investigadores, funcionários contratados da organização de pesquisa e funcionários do local do estudo).
- História de dependência de drogas ou abuso de álcool nos últimos 2 anos.
- Doação de sangue recente (até 30 dias após a triagem).
- Gravidez conhecida, amamentação ou intenção de engravidar durante o período do estudo.
