急性肺栓塞 (PE) 具有很高的发病率,是西方世界心血管死亡的第三大原因。 它占院内死亡人数的 5-10%,对美国来说相当于每年 200,000 人死亡。 最近的登记和队列研究表明,大约 10% 的急性肺栓塞患者在诊断后的前 1 至 3 个月内死亡。 观察幸存者超过 3 个月的研究报告了 PE 后 2-3 年内慢性血栓栓塞性肺动脉高压 (CTEPH) 的发生率为 1-5%。 CTEPH 是血栓栓塞性疾病的一种致残性长期并发症,对患者的生活质量和预后有负面影响。

所有指南机构普遍推荐全身静脉溶栓治疗大面积肺栓塞,但对于次大面积肺栓塞仍存在争议。 在最近的荟萃分析中,8 项次大规模 PE 试验(1993-2014,n=1775)的亚组分析表明,溶栓治疗与死亡率降低(1.39% 对 2.92%)相关,但与大出血增加(7.74%)相关对比 2.25%).11 这些结果主要由最大的随机试验(PEITHO,1006 名患者)推动,该试验比较了单一的、适应体重的静脉注射。 推注替奈普酶联合标准抗凝治疗。

几个观察性非对照系列已经证明了导管导向技术在改善临床和血液动力学参数以及减少凝块负荷方面的功效,同时展示了良好的安全性。 ULTIMA 试验是第一个将标准固定剂量的 USAT(15 小时内每肺 10mg rtPA)和 AC 与单独 AC 进行比较的 sPE CDI 随机对照试验。 在 USAT 组中,但在肝素组中,平均 RV/LV 比率在 24 小时时显着降低,但在 90 天时两组之间变得相当。 与肝素组相比,USAT 组的 RV 收缩功能在 24 小时和 90 天均有显着改善。 在两个研究组中,轻微的出血并发症都很少见,并且没有大出血并发症。 SEATTLE II 试验是一项评估 USAT 有效性的单臂研究,也显示 48 小时后 RV/LV 比值有所改善。
比较急性 PE 的不同导管导向疗法的数据有限。 大多数最近的导管定向干预系列都专门使用 USAT;然而,将这种方式与标准多侧孔导管输注进行比较的比较有效性数据有限。 初步的、不受控制的数据相互矛盾。 Lin 及其同事对 33 名高危 PE 患者进行的一项系列研究表明,在 CDT 组中,USAT 可通过血管造影清除血栓负荷,同时发生更多出血事件。 Kuo 及其同事指出,在最近发表的一项多中心前瞻性登记的早期结果中,USAT 和 CDT 在结果和治疗细节方面没有差异。 匹兹堡大学小组对 63 名患者的回顾性分析表明,两种治疗方式之间可能没有差异,表明两组的结果率相似,例如存活率、血流动力学稳定和超声心动图参数,手术时间和溶解剂量相似时间调整的队列。 在所有这些研究中都不能低估选择偏差。
