Přirozená historie Angelmanova syndromu (GLOW-AS)
Longitudinální přirozená historická studie dospělých a dětí s Angelmanovým syndromem
Cílem této observační studie je zjistit více o přirozeném průběhu Angelmanova syndromu (AS) u dětí a dospělých s potvrzenou genetickou diagnózou AS. Hlavní otázky, na které se studie zaměřuje, jsou:
- Jak se vývojové dovednosti, jako je komunikace, motorické schopnosti a adaptivní chování, mění v průběhu 1 roku u lidí s AS?
- Existují specifické vzorce v mozkové aktivitě nebo spánku, které jsou spojeny se změnami příznaků AS v čase?
Účastníci budou:
- Navštívit studijní centrum 5krát během 1 roku (přibližně každé 3 měsíce) k hodnocení.
- Vyplnit testy a dotazníky o vývoji, chování a spánku s pomocí svých pečovatelů.
- Podstoupit elektroencefalografii (EEG) k měření mozkové aktivity a nosit doma zařízení pro monitorování spánku (k získání aktigrafických dat).

Detailní popis
- Longitudinální, přirozená historická studie 40 jedinců s AS, obohacená o adolescentní populaci.
- Usiluje o stanovení normativních dat pro informování budoucího návrhu a interpretace klinických studií pro AS a pro podporu vývoje přesnějších diagnostických nástrojů a cílených terapeutických intervencí pro AS.
- Sledování adaptivního chování, komunikace, problémového chování, celkového zdraví, celkové závažnosti onemocnění a běžných souběžných stavů (včetně epilepsie a poruch spánku) prostřednictvím standardizovaných hodnocení.
- Data biomarkerů získaná prostřednictvím EEG a aktigrafických záznamů budou analyzována pro posouzení změn souvisejících s věkem v klinických výsledcích a neurofyziologických měřeních.
- Účastníci absolvují až 5 osobních návštěv studie v průběhu 12 měsíců: týden 0, týden 13, týden 26, týden 39, týden 52.

Kritéria zařazení:
- Účastník má primární klinickou diagnózu Angelmanova syndromu s dokumentovanou genetickou variantou ovlivňující funkci genu UBE3A v lidském lokusu 15q11.2-q13.3. Souběžné stavy (např. porucha autistického spektra, dětská mozková obrna, mentální postižení) jsou povoleny; Angelmanův syndrom však musí být primární klinickou diagnózou.
- Účastník je mužského nebo ženského pohlaví (přiřazeného při narození) a ve věku ≥1 roku při první studijní návštěvě.
- Účastník má studijního partnera, který splňuje níže uvedená kritéria pro studijního partnera.
- Účastník, pokud není schopen poskytnout informovaný souhlas, má vhodného zástupce, který je starší 18 let a je ochoten a schopen poskytnout informovaný souhlas jménem účastníka v souladu s platnými směrnicemi Mezinárodní rady pro harmonizaci (ICH) a příslušnými institucionálními předpisy.
Jednotlivci musí splňovat následující kritéria, aby mohli být zařazeni jako studijní partneři:
- Studijní partner je rodič nebo primární pečovatel starší 18 let.
- Studijní partner má pravidelný kontakt s účastníkem a podle názoru zkoušejícího má dostatečné znalosti o aktuálním stavu účastníka, aby mohl poskytnout přesné a aktuální informace.
- Studijní partner má dostatečnou znalost anglického jazyka k vyplnění hodnocení studijního partnera.
- Studijní partner je ochoten a schopen poskytnout informovaný souhlas za sebe v souladu se směrnicemi ICH a příslušnými institucionálními předpisy.
- Studijní partner je podle názoru zkoušejícího spolehlivý a kompetentní; ochoten a schopen doprovázet účastníka na všechny studijní návštěvy a dodržovat studijní postupy; dostupný telefonicky nebo e-mailem podle potřeby; a dostatečně informovaný o aktuálním stavu účastníka, aby mohl poskytnout přesné a aktuální informace o zdraví a pohodě účastníka.
Kritéria vyloučení:
- Účastník má alespoň jednu další známou genetickou abnormalitu mimo lidský lokus 15q11.2-q13.3 způsobující pravděpodobné nebo známé vývojové postižení.
- Během 28 dnů (4 týdnů) před první studijní návštěvou byla změněna alespoň jedna standardní léčba (lék nebo doplňková terapie), kterou účastník užívá. Léčba zahrnuje, ale není omezena na dávky antiepileptik, léků na zvládání chování, léků na spánek, gabapentin, kanabidiol, speciální diety, doplňky stravy, logopedii, ergoterapii, aplikovanou behaviorální analýzu (ABA), psychosociální intervence, fyzioterapii nebo nutriční podporu.
- Účastník má nestabilní epilepsii, definovanou jako návštěvu pohotovosti nebo hospitalizaci z důvodu záchvatů během 28 dnů (4 týdnů) před první studijní návštěvou.
- Účastník je plodný a je buď těhotný, kojí, nebo nepoužívá vhodnou metodu antikoncepce; abstinence je přijatelná.
- Účastník má klinicky významnou anamnézu malignity; klinicky významné abnormální výsledky testů; klinicky významné kardiovaskulární, hematologické, jaterní, svalové, neurologické nebo renální onemocnění; nebo prodělal jiné klinické události, které podle názoru zkoušejícího činí účast nevhodnou.
- Účastník má v anamnéze léčbu jakoukoli buněčnou nebo genovou terapií, včetně antisense oligonukleotidů nebo genových editačních terapií.
- Účastník obdržel jakoukoli experimentální terapii jinou než buněčnou nebo genovou terapii během 28 dnů nebo 5 poločasů (podle toho, co je delší) před první studijní návštěvou.
- Účastník je aktuálně zařazen nebo plánuje zařazení do intervenční studie zahrnující experimentální látku nebo zařízení během plánovaného pozorovacího období.
- Účastník má známou kontraindikaci k elektroencefalografii, aktigrafii nebo jakémukoli jinému studijnímu postupu popsanému v harmonogramu hodnocení.
- Účastník nebo studijní partner je podle názoru zkoušejícího nevhodný k účasti z jakéhokoli jiného důvodu, včetně neschopnosti splnit studijní požadavky.
