Uno Studio Longitudinale di Storia Naturale di Adulti e Bambini con la Sindrome di Angelman
L'obiettivo di questo studio osservazionale è comprendere la progressione naturale della sindrome di Angelman (AS) in bambini e adulti con una diagnosi genetica confermata di AS. Le principali domande a cui mira a rispondere sono:
- Come cambiano le abilità di sviluppo, come comunicazione, capacità motorie e comportamenti adattativi, in un periodo di 1 anno nelle persone con AS?
- Esistono specifici pattern nell'attività cerebrale o nel sonno associati ai cambiamenti nei sintomi dell'AS nel tempo?
I partecipanti dovranno:
- Visitare il sito dello studio 5 volte nell'arco di 1 anno (circa ogni 3 mesi) per le valutazioni.
- Completare test e questionari sullo sviluppo, i comportamenti e il sonno con l'aiuto dei loro caregiver.
- Sottoporsi a elettroencefalogrammi (EEG) per misurare l'attività cerebrale e indossare un dispositivo di monitoraggio del sonno a casa (per raccogliere dati di attigrafia).

- Studio longitudinale di storia naturale su 40 individui con AS, arricchito per una popolazione adolescenziale.
- Si propone di stabilire dati normativi per informare la progettazione e l'interpretazione futura degli studi clinici per l'AS e per supportare lo sviluppo di strumenti diagnostici più precisi e interventi terapeutici mirati per l'AS.
- Tracciare il comportamento adattivo, la comunicazione, i comportamenti problematici, la salute generale, la gravità complessiva della malattia e le condizioni comuni co-occorrenti (inclusi epilessia e disturbi del sonno) attraverso valutazioni standardizzate.
- I dati sui biomarcatori raccolti tramite EEG e registrazioni actigrafiche saranno analizzati per valutare i cambiamenti legati all'età negli esiti clinici e nelle misure neurofisiologiche.
- I partecipanti parteciperanno fino a 5 visite di studio in persona nel corso di 12 mesi: Settimana 0, Settimana 13, Settimana 26, Settimana 39, Settimana 52.

Descrizione
Criteri di inclusione:
- Il partecipante ha una diagnosi clinica primaria di sindrome di Angelman con variazioni genetiche documentate che influenzano la funzione del gene UBE3A all'interno del locus umano 15q11.2-q13.3. Condizioni co-occorrenti (ad esempio, disturbo dello spettro autistico, paralisi cerebrale, disabilità intellettiva) sono consentite; tuttavia, la sindrome di Angelman deve essere la diagnosi clinica primaria.
- Il partecipante è di sesso maschile o femminile (sesso assegnato alla nascita) e ha un'età ≥1 anno al momento della prima visita di studio.
- Il partecipante ha un partner di studio che soddisfa i criteri per il partner di studio di seguito riportati.
- Il partecipante, se incapace di fornire il consenso informato, ha un rappresentante legale appropriato di almeno 18 anni di età, disposto e in grado di fornire il consenso informato per conto del partecipante in conformità con le linee guida attuali dell'International Council for Harmonisation (ICH) e le normative istituzionali applicabili.
Gli individui devono soddisfare i seguenti criteri per essere arruolati come partner di studio:
- Il partner di studio è un genitore o un caregiver primario di almeno 18 anni di età.
- Il partner di studio ha un contatto costante con il partecipante e, a giudizio dello sperimentatore, è sufficientemente informato sulle condizioni del partecipante per fornire informazioni accurate e aggiornate.
- Il partner di studio ha una sufficiente conoscenza della lingua inglese per completare le valutazioni del partner di studio.
- Il partner di studio è disposto e in grado di fornire il consenso informato per proprio conto in conformità con le linee guida ICH e le normative istituzionali applicabili.
- Il partner di studio è, a giudizio dello sperimentatore, affidabile e competente; disposto e in grado di accompagnare il partecipante a tutte le visite di studio e rispettare le procedure di studio; raggiungibile per telefono o email se necessario; e sufficientemente informato sulle condizioni attuali del partecipante per fornire informazioni accurate e aggiornate riguardanti la salute e il benessere del partecipante.
Criteri di esclusione:
- Il partecipante ha almeno un'ulteriore anomalia genetica nota al di fuori del locus umano 15q11.2-q13.3 che causa una probabile o nota disabilità dello sviluppo.
- Almeno un trattamento standard di cura (medicazione o terapia adiuvante) utilizzato dal partecipante è stato modificato durante i 28 giorni (4 settimane) precedenti la prima visita di studio. I trattamenti includono, ma non sono limitati a, dosi di farmaci antiepilettici, farmaci per la gestione comportamentale, farmaci per il sonno, gabapentin, cannabidiolo, diete speciali, integratori, logopedia, terapia occupazionale, analisi comportamentale applicata (ABA), interventi psicosociali, fisioterapia o supporto nutrizionale.
- Il partecipante ha un'epilessia instabile, definita come un accesso al pronto soccorso o un ricovero ospedaliero per problemi correlati alle crisi epilettiche nei 28 giorni (4 settimane) precedenti la visita di studio iniziale.
- Il partecipante è in età fertile ed è incinta, allatta o non utilizza un metodo contraccettivo adeguato; l'astinenza è accettabile.
- Il partecipante ha una storia clinicamente rilevante di neoplasia; risultati di test anormali clinicamente significativi; malattie cardiovascolari, ematologiche, epatiche, muscolari, neurologiche o renali clinicamente significative; o ha sperimentato altri eventi clinici che, a giudizio dello sperimentatore, rendono la partecipazione non idonea.
- Il partecipante ha una storia di trattamento con qualsiasi terapia cellulare o genica, inclusi oligonucleotidi antisenso o terapie di editing genico.
- Il partecipante ha ricevuto qualsiasi terapia sperimentale diversa da una terapia cellulare o genica entro 28 giorni o 5 emivite (a seconda di quale sia più lunga) precedenti la visita di studio iniziale.
- Il partecipante è attualmente arruolato o prevede di arruolarsi in uno studio interventistico che coinvolge un agente o dispositivo sperimentale durante il periodo di osservazione pianificato.
- Il partecipante ha una controindicazione nota all'elettroencefalografia, all'attigrafia o a qualsiasi altra procedura di studio descritta nel programma delle valutazioni.
- Il partecipante o il partner di studio è, a giudizio dello sperimentatore, non idoneo alla partecipazione per qualsiasi altro motivo, inclusa l'incapacità di soddisfare i requisiti dello studio.
