Longitudinalne badanie historii naturalnej dorosłych i dzieci z zespołem Angelmana
Celem tego badania obserwacyjnego jest poznanie naturalnego przebiegu zespołu Angelmana (AS) u dzieci i dorosłych z potwierdzoną diagnozą genetyczną AS. Główne pytania, na które ma odpowiedzieć badanie, to:
- Jak zmieniają się umiejętności rozwojowe, takie jak komunikacja, zdolności motoryczne i zachowania adaptacyjne, w ciągu 1 roku u osób z AS?
- Czy istnieją określone wzorce w aktywności mózgu lub śnie, które są związane ze zmianami objawów AS w czasie?
Uczestnicy będą:
- Odwiedzać miejsce badania 5 razy w ciągu 1 roku (co około 3 miesiące) w celu przeprowadzenia ocen.
- Wypełniać testy i kwestionariusze dotyczące rozwoju, zachowań i snu z pomocą opiekunów.
- Przechodzić elektroencefalogramy (EEG) w celu pomiaru aktywności mózgu oraz nosić w domu urządzenie monitorujące sen (do zbierania danych z aktigrafii).

Szczegółowy opis
- Długoterminowe, naturalne badanie historii 40 osób z AS, wzbogacone o populację nastoletnią.
- Ma na celu ustalenie danych normatywnych, które będą informować przyszły projekt i interpretację badań klinicznych dla AS oraz wspierać rozwój bardziej precyzyjnych narzędzi diagnostycznych i ukierunkowanych interwencji terapeutycznych dla AS.
- Śledzenie zachowań adaptacyjnych, komunikacji, trudnych zachowań, ogólnego stanu zdrowia, ogólnej ciężkości choroby i powszechnych współwystępujących schorzeń (w tym padaczki i zaburzeń snu) poprzez standaryzowane oceny.
- Dane biomarkerowe zebrane za pomocą zapisów EEG i aktigrafii zostaną przeanalizowane w celu oceny zmian związanych z wiekiem w wynikach klinicznych i pomiarach neurofizjologicznych.
- Uczestnicy będą uczestniczyć w maksymalnie 5 osobistych wizytach badawczych w ciągu 12 miesięcy: Tydzień 0, Tydzień 13, Tydzień 26, Tydzień 39, Tydzień 52.

Kryteria włączenia:
- Uczestnik ma pierwotne kliniczne rozpoznanie zespołu Angelmana z udokumentowaną zmiennością genetyczną wpływającą na funkcję genu UBE3A w ludzkim locus 15q11.2-q13.3. Współwystępujące schorzenia (np. zaburzenia ze spektrum autyzmu, mózgowe porażenie dziecięce, niepełnosprawność intelektualna) są dopuszczalne; jednak zespół Angelmana musi być pierwotnym rozpoznaniem klinicznym.
- Uczestnik jest płci męskiej lub żeńskiej (przypisanej przy urodzeniu) i ma ≥1 rok życia podczas pierwszej wizyty badawczej.
- Uczestnik ma partnera badawczego, który spełnia poniższe kryteria partnera badawczego.
- Uczestnik, jeśli nie jest w stanie wyrazić świadomej zgody, ma odpowiedniego przedstawiciela ustawowego, który ma co najmniej 18 lat i jest chętny oraz zdolny do wyrażenia świadomej zgody w imieniu uczestnika zgodnie z aktualnymi wytycznymi Międzynarodowej Rady Harmonizacji (ICH) i obowiązującymi przepisami instytucjonalnymi.
Osoby muszą spełniać następujące kryteria, aby zostać zarejestrowane jako partnerzy badawczy:
- Partner badawczy jest rodzicem lub głównym opiekunem, który ma co najmniej 18 lat.
- Partner badawczy ma stały kontakt z uczestnikiem i, w opinii badacza, jest wystarczająco zaznajomiony z aktualnym stanem uczestnika, aby dostarczyć dokładne i aktualne informacje.
- Partner badawczy posiada wystarczającą znajomość języka angielskiego, aby wypełnić oceny partnera badawczego.
- Partner badawczy jest chętny i zdolny do wyrażenia świadomej zgody we własnym imieniu zgodnie z wytycznymi ICH i obowiązującymi przepisami instytucjonalnymi.
- Partner badawczy jest, w opinii badacza, wiarygodny i kompetentny; chętny i zdolny do towarzyszenia uczestnikowi we wszystkich wizytach badawczych oraz przestrzegania procedur badania; dostępny telefonicznie lub mailowo w razie potrzeby; oraz wystarczająco zaznajomiony z aktualnym stanem/stanami uczestnika, aby dostarczyć dokładne i aktualne informacje dotyczące zdrowia i dobrostanu uczestnika.
Kryteria wykluczenia:
- Uczestnik ma co najmniej jedną dodatkową znaną nieprawidłowość genetyczną poza ludzkim locus 15q11.2-q13.3 powodującą prawdopodobną lub znaną niepełnosprawność rozwojową.
- Co najmniej jedno standardowe leczenie (lek lub terapia wspomagająca) stosowane przez uczestnika zostało zmienione w ciągu 28 dni (4 tygodni) poprzedzających pierwszą wizytę badawczą. Leczenia obejmują, ale nie ograniczają się do: dawek leków przeciwpadaczkowych, leków do zarządzania zachowaniem, leków nasennych, gabapentyny, kannabidiolu, specjalnych diet, suplementów, terapii logopedycznej, terapii zajęciowej, stosowanej analizy zachowania (ABA), interwencji psychospołecznych, fizjoterapii lub wsparcia żywieniowego.
- Uczestnik ma niestabilną padaczkę, zdefiniowaną jako wizytę na oddziale ratunkowym lub hospitalizację z powodu problemów związanych z napadami w ciągu 28 dni (4 tygodni) poprzedzających pierwszą wizytę badawczą.
- Uczestnik ma zdolność do zajścia w ciążę i jest w ciąży, karmi piersią lub nie stosuje odpowiedniej metody antykoncepcji; abstynencja jest akceptowalna.
- Uczestnik ma klinicznie istotną historię nowotworu; klinicznie istotne nieprawidłowe wyniki badań; klinicznie istotną chorobę układu sercowo-naczyniowego, hematologiczną, wątrobową, mięśniową, neurologiczną lub nerkową; lub doświadczył innych zdarzeń klinicznych, które, w opinii badacza, uniemożliwiają udział.
- Uczestnik ma w wywiadzie leczenie jakąkolwiek terapią komórkową lub genową, w tym oligonukleotydami antysensownymi lub terapiami edycji genów.
- Uczestnik otrzymał jakąkolwiek terapię badawczą inną niż terapia komórkowa lub genowa w ciągu 28 dni lub 5 okresów półtrwania (w zależności od tego, co jest dłuższe) poprzedzających pierwszą wizytę badawczą.
- Uczestnik jest obecnie zapisany lub planuje zapisać się do badania interwencyjnego obejmującego badany lek lub urządzenie podczas planowanego okresu obserwacji.
- Uczestnik ma znane przeciwwskazanie do elektroencefalografii, aktigrafii lub jakiejkolwiek innej procedury badawczej opisanej w harmonogramie ocen.
- Uczestnik lub partner badawczy jest, w opinii badacza, z jakiegokolwiek innego powodu nieodpowiedni do udziału, w tym z powodu niemożności spełnienia wymagań badania.
