Popis
Kritéria pro zařazení a vyloučení pacientů s MDD. 1、Kritéria zařazení:
- současná diagnóza velké depresivní poruchy, první epizoda nebo recidiva podle Diagnostického a statistického manuálu duševních poruch, páté vydání (DSM-5);
- věk 18–60 let; pohlaví bez omezení;
- tíže příznaků MDD musí být >14 bodů na Hamiltonově škále deprese-17 (HAMD-17);
- současná kognitivní dysfunkce, definovaná jako celkové skóre ≤ 70 v testu substituce číslicových symbolů (DSST);
- způsobilí pacienti užívali inhibitory zpětného vychytávání serotoninu (SSRI) ve stabilních dávkách alespoň 1 týden nebo ne;
- vzdělání vyšší než základní škola, schopnost porozumět a spolupracovat při plnění studijních postupů;
- dobrovolná účast v této studii a podepsání informovaného souhlasu před zařazením.
2、Kritéria vyloučení:
- současná nebo minulá diagnóza jakékoli jiné poruchy než velké depresivní poruchy podle kritérií DSM-5;
- skóre na YMRS je >8;
- jedinci, kteří podstoupili jiný zásah kromě SSRI v posledních šesti měsících nebo nyní, nebo kteří to plánují během jednoho měsíce;
- jedinci se silnými pocity sebeobviňování, sebepoškozování nebo sebevražednými sklony (skóre položky sebevraždy HDRS-17 ≥ 3);
- jedinci se závažnými fyzickými onemocněními, včetně srdečního selhání, renálního selhání, závažné jaterní dysfunkce, hypertyreózy nebo hypotyreózy; nebo anamnéza závažného traumatu mozku nebo organické mozkové patologie (např. intracerebrální krvácení, rozsáhlý mozkový infarkt, encefalitida, epilepsie), stejně jako neurologická onemocnění;
- jedinci s jakýmkoli stupněm patologie sítnice, včetně dystrofie sítnice, věkem podmíněné makulární degenerace, diabetické retinopatie, katarakty, glaukomu nebo jiných očních onemocnění;
- jedinci s fotosenzitivními stavy, jako je systémový lupus erythematodes, porfyrie, chronická fotodermatitida, solární urtikarie, nebo ti, kteří aktuálně užívají léky, které mohou zvýšit fotosenzitivitu (např. fenothiaziny, antimalarika, propranolol, hypericin, stimulanty nebo chronická léčba nesteroidními protizánětlivými léky);
- těhotné nebo kojící ženy;
- jedinci s kontraindikacemi pro magnetickou rezonanci (MRI), jako je přítomnost kovových implantátů bezpečných pro MRI nebo klaustrofobie;
- jedinci, které výzkumník z jiných důvodů považuje za nevhodné pro zařazení do této studie.
Kritéria pro ukončení a odstoupení:
- účastníci splní jedno z výše uvedených kritérií vyloučení po zařazení;
- aktuální léčebný režim jedinců vyžaduje změnu;
- účastníci, kteří nespolupracují nebo se dobrovolně ze studie odhlásí;
- kvůli závažným nežádoucím účinkům účastníci nemohou tolerovat fototerapii;
- účastníci, kteří nedodržují intervenci studijního protokolu tři po sobě jdoucí dny nebo po kumulativní dobu delší než sedm dní;
- zrušení studie.

- věk 18–60 let; pohlaví bez omezení; a praváci;
- bez současné kognitivní dysfunkce, definované jako celkové skóre ≤ 70 v testu substituce číslicových symbolů (DSST);
- vzdělání vyšší než základní škola, schopnost porozumět a spolupracovat při plnění studijních postupů;
- dobrovolná účast v této studii a podepsání informovaného souhlasu před zařazením.
2. Kritéria vyloučení
- současná nebo celoživotní diagnóza jakýchkoli psychiatrických poruch, nebo anamnéza zneužívání nebo závislosti na látkách/drogách;
- aktuálně nebo dříve diagnostikována závažná somatická onemocnění, jako je srdeční selhání, renální selhání, závažná jaterní dysfunkce nebo hypertyreóza/hypotyreóza;
- anamnéza závažného traumatického poranění mozku nebo organických mozkových lézí;
- těhotné nebo kojící ženy;
- jedinci s kontraindikacemi pro magnetickou rezonanci (MRI), jako je přítomnost kovových implantátů bezpečných pro MRI nebo klaustrofobie;
- jedinci, které výzkumník z jiných důvodů považuje za nevhodné pro zařazení do této studie.

Komparátor placeba: DRL control group
Eligible MDD participants will be randomly subjected to the DRL control group.
The light source intensity will be set at <100 lux and placed 0.6 meters away from the MDD participant.
They will be required to staring at the light source for 2 seconds every 5 minutes.
The DRL control group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for four weeks, 6 days per week.

Using a dim red light box as the placebo, with an intensity of <100lux and a main wavelength of 690.4nm

Experimentální: BLT experimental group
Eligible MDD participants will be randomly subjected to the BLT experimental group.
The source intensity of BLT experimental group will be set at 10000 lux and placed 0.45 meters away from the MDD participant.
They will be required to staring at the light source for 2 seconds every 5 minutes.
The BLT experimental group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for 4 weeks, 6 days per week.

The task has been included as a subtest in the Brief Assessment of Cognition in Schizophrenia; in this task, subjects are required to write numerals 1-9 as matches to nonmeaningful symbols on a response sheet for 90 s, as based on a key provided to them, with higher scores reflecting better performance.
