Kryteria włączenia i wykluczenia dla pacjentów z MDD. 1、Kryteria włączenia:
- aktualna diagnoza dużej depresji, pierwszego epizodu lub nawrotu, zgodnie z piątą edycją Podręcznika Diagnostycznego i Statystycznego Zaburzeń Psychicznych (DSM-5);
- wiek 18-60 lat; płeć bez ograniczeń;
- nasilenie objawów MDD musi wynosić >14 punktów w Skali Depresji Hamiltona-17 (HAMD-17);
- obecnie z dysfunkcją poznawczą, zdefiniowaną jako łączny wynik ≤ 70 w teście zamiany symboli cyfrowych (DSST);
- kwalifikujący się pacjenci otrzymywali selektywne inhibitory wychwytu zwrotnego serotoniny (SSRI) w stabilnych dawkach przez co najmniej 1 tydzień lub nie;
- poziom wykształcenia powyżej szkoły podstawowej, zdolność do zrozumienia i współpracy w realizacji procedur badania;
- dobrowolny udział w tym badaniu i podpisanie świadomej zgody przed włączeniem.
2、Kryteria wykluczenia:
- aktualna lub przeszła diagnoza jakiegokolwiek zaburzenia innego niż duża depresja według kryteriów DSM-5;
- wyniki w YMRS >8;
- osoby, które w ciągu ostatnich sześciu miesięcy lub obecnie przeszły inne interwencje oprócz SSRI lub planują to w ciągu miesiąca;
- osoby z silnym obwinianiem siebie, samookaleczaniem lub tendencjami samobójczymi (wynik punktu samobójczego HDRS-17 ≥ 3);
- osoby z ciężkimi chorobami somatycznymi, w tym niewydolnością serca, niewydolnością nerek, ciężką dysfunkcją wątroby, nadczynnością lub niedoczynnością tarczycy; lub z historią ciężkiego urazu mózgu lub organicznej patologii mózgu (np. krwotok śródmózgowy, rozległy zawał mózgu, zapalenie mózgu, padaczka), a także chorób neurologicznych;
- osoby z jakimkolwiek stopniem patologii siatkówki, w tym dystrofią siatkówki, zwyrodnieniem plamki związanym z wiekiem, retinopatią cukrzycową, zaćmą, jaskrą lub innymi chorobami oczu;
- osoby z chorobami światłoczułymi, takimi jak toczeń rumieniowaty układowy, porfiria, przewlekłe fotodermatozy, pokrzywka słoneczna, lub te obecnie przyjmujące leki, które mogą zwiększać światłoczułość (np. fenotiazyny, leki przeciwmalaryczne, propranolol, hiperycyna, stymulanty lub przewlekłe leczenie niesteroidowymi lekami przeciwzapalnymi);
- kobiety w ciąży lub karmiące piersią;
- osoby z przeciwwskazaniami do rezonansu magnetycznego (MRI), takimi jak obecność implantów metalowych niezgodnych z MRI lub klaustrofobia;
- osoby uznane przez badacza za nieodpowiednie do włączenia do tego badania z innych powodów.
Kryteria wycofania i zakończenia:
- Uczestnicy spełniają jedno z powyższych kryteriów wykluczenia po włączeniu;
- aktualny schemat leczenia uczestnika wymaga zmiany;
- Uczestnicy, którzy nie współpracują lub dobrowolnie wycofują się z badania;
- Z powodu ciężkich zdarzeń niepożądanych uczestnicy nie są w stanie tolerować fototerapii;

Komparator placebo: DRL control group
Eligible MDD participants will be randomly subjected to the DRL control group.
The light source intensity will be set at <100 lux and placed 0.6 meters away from the MDD participant.
They will be required to staring at the light source for 2 seconds every 5 minutes.
The DRL control group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for four weeks, 6 days per week.

Eksperymentalny: BLT experimental group
Eligible MDD participants will be randomly subjected to the BLT experimental group.
The source intensity of BLT experimental group will be set at 10000 lux and placed 0.45 meters away from the MDD participant.
They will be required to staring at the light source for 2 seconds every 5 minutes.
The BLT experimental group will be administered for 40 minutes each day between 7:00 AM and 10:00 AM, lasting for 4 weeks, 6 days per week.
