Klinická studie zdravotnických prostředků PROXERA PSOMED 20 a PROXERA PSOMED 40 pro léčbu plakovité psoriázy a nepustulózní palmoplantární psoriázy (PASI)
Hodnocení účinnosti a bezpečnosti zdravotnického prostředku PROXERA PSOMED 20 při redukci psoriatických plaků a zdravotnického prostředku PROXERA PSOMED 40 při redukci nedokonalosti nepustulózní palmoplantární psoriázy u pacientů s plakovou psoriázou: sponzorovaná, intervenční, jednocentrová, randomizovaná, jednoduše zaslepená, nitropacientsky kontrolovaná studie (PASI)
Tato intervenční studie s jedním centrem byla navržena k posouzení účinnosti a bezpečnosti dvou CE označených lokálních zdravotnických prostředků: PROXERA PSOMED 20 je zkoumán u účastníků s ne-dlaně-ploskovou plakovou psoriázou, zatímco PROXERA PSOMED 40 je zkoumán u účastníků s ne-pustulózní dlaně-ploskovou psoriázou.
Cílem studie je zjistit, zda léčba cílové oblasti přiděleným zdravotnickým prostředkem vede k většímu klinickému zlepšení ve srovnání s neošetřenou kontrolní oblastí na protilehlé straně u stejného účastníka. U PROXERA PSOMED 20 je primární hodnocení provedeno po 4 týdnech léčby. U PROXERA PSOMED 40 je primární hodnocení provedeno po 8 týdnech léčby.
Sekundární cíle zahrnují posouzení bezpečnosti, místní snášenlivosti, celkového průběhu psoriázy, jejího dopadu na kvalitu života a dalších parametrů místní aktivity onemocnění v cílových oblastech. Účastníci budou aplikovat přidělený studijní prostředek jednou denně na vybranou cílovou oblast, podstoupí klinická hodnocení na plánovaných návštěvách a poskytnou informace o bezpečnosti a snášenlivosti po celou dobu studie.

Kritéria pro zařazení:
PROXERA PSOMED 20 - normalizační krém, 20% močovina
- Diagnóza plakovité psoriázy (ne dlaně a chodidla) po dobu alespoň 1 roku PROXERA PSOMED 40 - intenzivní exfoliační gel, 40% močovina
- Diagnóza nepustulózní psoriázy dlaní a chodidel po dobu alespoň 1 roku
Společná kritéria pro zařazení:
- Věk 18-65 let
- "Mírná" závažnost psoriázy podle:
- Psoriasis Area Severity Index (PASI) <10
- Dermatologic Life Quality Index (DLQI) <10
- Body Surface Area (BSA) <10
Kritéria pro vyloučení:
- Nesnášenlivost přípravku
- Současná léčba systémovými imunomodulačními léky (např. metotrexát, cyklosporin, apremilast)
- Současná léčba biologickými léky jako anti-TNFα, anti-IL17 nebo anti-IL23.
- Současná lokální léčba topickými kortikosteroidy (TCS), topickými inhibitory kalcineurinu (TCI), analogy vitaminu D a jejich kombinacemi.
- Těhotné nebo kojící ženy

Experimentální: Intrapatientní léčba přípravkem PROXERA PSOMED 20
Jednoskupinová, randomizovaná, jednoduše zaslepená, vnitropacientsky kontrolovaná větev studie.
Účastníci jsou přiřazeni k léčbě podle diagnózy: účastníci s ne-dlaně-plantární plakovou psoriázou dostávají PROXERA PSOMED 20 jednou denně po dobu 4 týdnů na jednu cílovou plaku, zatímco účastníci s ne-pustulární dlaně-plantární psoriázou dostávají PROXERA PSOMED 40 jednou denně po dobu 8 týdnů na jednu cílovou oblast dlaně nebo chodidla.
V obou kohortách zůstává srovnatelná kontralaterální cílová oblast neléčena jako kontrola.
U PROXERA PSOMED 40 je léčba přerušena na 3 dny každých 14 dní.

Aplikuje se samostatně účastníkem jednou denně ve stejnou denní dobu na vybranou cílovou plak po dobu 4 týdnů, podle pravidla jednotky konečku prstu. Kontralaterální srovnatelná cílová oblast zůstává neléčená jako kontrola. Proxera Psomed 40: Topický zdravotnický prostředek ve formě gelu obsahující 40 % močoviny, dodávaný v tubě o objemu 100 ml. Aplikuje se samostatně účastníkem jednou denně ve stejnou denní dobu na vybranou cílovou oblast dlaně nebo chodidla po dobu 8 týdnů, podle pravidla jednotky konečku prstu. Léčba je přerušena na 3 dny každých 14 dní. Kontralaterální srovnatelná cílová oblast zůstává neléčená jako kontrola.

Účastníci s nepustulární palmoplantární psoriázou si sami aplikují přípravek PROXERA PSOMED 40 jednou denně ve stejnou denní dobu na jednu cílovou oblast dlaně nebo chodidla po dobu 8 týdnů.
Dávka se aplikuje podle pravidla jednotky konečku prstu.
Srovnatelná kontralaterální cílová oblast zůstává neléčená jako kontrola.

For each target site, the local score is calculated as (erythema + induration + scaling) × extent and ranges from 0 to 72, with higher scores indicating more severe disease.
