Клиническое исследование медицинских устройств PROXERA PSOMED 20 и PROXERA PSOMED 40 для лечения бляшечного псориаза и непустулезного ладонно-подошвенного псориаза (PASI)
Оценка эффективности и безопасности медицинского устройства PROXERA PSOMED 20 в уменьшении псориатических бляшек и медицинского устройства PROXERA PSOMED 40 в уменьшении непустулезного ладонно-подошвенного псориатического дефекта у пациентов с бляшечным псориазом: спонсируемое, интервенционное, одноцентровое, рандомизированное, простое слепое, контролируемое внутри пациента исследование (PASI)
Данное интервенционное исследование, проведённое в одном центре, было разработано для оценки эффективности и безопасности двух медицинских устройств для местного применения, имеющих маркировку CE: PROXERA PSOMED 20 изучается у участников с неладонно-подошвенным бляшечным псориазом, а PROXERA PSOMED 40 — у участников с непустулёзным ладонно-подошвенным псориазом.
Цель исследования — определить, приводит ли обработка целевого участка назначенным медицинским устройством к более значительному клиническому улучшению по сравнению с необрабатываемым контралатеральным контрольным участком у того же участника. Для PROXERA PSOMED 20 основная оценка проводится после 4 недель лечения. Для PROXERA PSOMED 40 основная оценка проводится после 8 недель лечения.
Вторичные цели включают оценку безопасности, местной переносимости, общего прогресса псориаза, его влияния на качество жизни и дополнительных параметров локальной активности заболевания на целевых участках. Участники будут наносить назначенное исследуемое устройство один раз в день на выбранный целевой участок, проходить клинические оценки в запланированные визиты и предоставлять информацию о безопасности и переносимости на протяжении всего исследования.

Критерии включения:
PROXERA PSOMED 20 - нормализующий крем, 20% мочевины
- Диагноз бляшечного псориаза (не ладонно-подошвенного) не менее 1 года PROXERA PSOMED 40 - интенсивный отшелушивающий гель, 40% мочевины
- Диагноз непустулезного ладонно-подошвенного псориаза не менее 1 года
Общие критерии включения:
- Возраст 18-65 лет
- «Легкая» степень тяжести псориаза согласно:
- Индекс площади и тяжести псориаза (PASI) <10
- Дерматологический индекс качества жизни (DLQI) <10
- Площадь поражения поверхности тела (BSA) <10
Критерии исключения:
- Непереносимость продукта
- Сопутствующая терапия системными иммуномодулирующими препаратами (например, метотрексат, циклоспорин, апремиласт)
- Сопутствующая терапия биологическими препаратами, такими как анти-ФНОα, анти-ИЛ17 или анти-ИЛ23.
- Сопутствующая местная терапия топическими кортикостероидами (ТКС), топическими ингибиторами кальциневрина (ТИК), аналогами витамина D и комбинациями.

Экспериментальный: Внутрипациентное лечение препаратом PROXERA PSOMED 20
Одноцентровое, рандомизированное, простое слепое, внутрипациентное контролируемое исследование.
Участники распределяются на лечение в соответствии с диагнозом: пациенты с нон-пальмоплантарным бляшечным псориазом получают PROXERA PSOMED 20 один раз в день в течение 4 недель на одной целевой бляшке, в то время как пациенты с нон-пустулезным пальмоплантарным псориазом получают PROXERA PSOMED 40 один раз в день в течение 8 недель на одной целевой области ладони или подошвы.
В обеих когортах сопоставимая контралатеральная целевая область остается необработанной в качестве контроля.
Для PROXERA PSOMED 40 лечение прерывается на 3 дня каждые 14 дней.

Erythema, induration, and scaling are each scored from 0 to 4, while extent of involvement is scored from 0 to 6.
For each target site, the local score is calculated as (erythema + induration + scaling) × extent and ranges from 0 to 72, with higher scores indicating more severe disease.
In participants treated with PROXERA PSOMED 40, the primary analysis compares the change from baseline to Week 8 in the treated target area with the change in the contralateral untreated control area.
