Popis
Kritéria pro zařazení:
- identifikovat se jako čínský přistěhovalec;
- být ve věku 18-70 let;
- mít v lékařském záznamu diagnózu diabetu 2. typu (T2D);
- během posledních 12 měsíců jste měli schůzku s lékařem pro běžnou péči T2D;
- mají nejnovější HbA1c alespoň 8 %;
- být ochoten přijímat krátká videa týkající se správy T2D; a
- vlastnit smartphone nebo, pokud jej nemají, být ochoten a schopen používat studijní smartphone.
Kritéria vyloučení:
- neschopný nebo neochotný poskytnout informovaný souhlas;
- neschopnost se smysluplně zapojit do intervence (např. nekorigované poškození zraku a sluchu);
- neochotný přijmout randomizační přidělení;
- je těhotná, plánuje otěhotnět v příštích 6 měsících nebo otěhotní během studie; nebo
- kojí (např. mohou mít potenciální dietní omezení).

Experimentální: VIDEO
Účastníci zařazení do větve VIDEO obdrží jedno stručné video zaměřené na vzdělávání a podporu sebeovládání diabetu (DSMES) týdně po dobu 24 týdnů.
Videa budou doručena prostřednictvím textové zprávy.

Videa DSMES poskytují účastníkům důležité vzdělávání v oblasti diabetu a umožňují jim stát se aktivovaným pacientem na individuální úrovni.
|

Experimentální: VIDEO+CHW
Účastníci zařazení do větve VIDEO+CHW obdrží po dobu 24 týdnů jedno stručné video DSMES týdně, kromě dvoutýdenních podpůrných hovorů od komunitního zdravotnického pracovníka (CHW).
Videa DSMES budou doručena prostřednictvím textové zprávy.

Videa DSMES poskytují účastníkům důležité vzdělávání v oblasti diabetu a umožňují jim stát se aktivovaným pacientem na individuální úrovni.
CHW posoudí sociální determinanty zdraví (SDOH) účastníků v péči o diabetes 2. typu a spojí je s dostupnými zdroji v komunitě.

Participants assigned to the CONTROL group will continue to receive usual care.
At the end of the study, they will receive DSMES videos.
