Kryteria przyjęcia:
- identyfikować się jako chiński imigrant;
- mieć od 18 do 70 lat;
- mieć w dokumentacji medycznej stwierdzoną cukrzycę typu 2 (T2D);
- w ciągu ostatnich 12 miesięcy miałeś wizytę u lekarza w celu rutynowej opieki nad T2D;
- mieć ostatnią wartość HbA1c wynoszącą co najmniej 8%;
- wyrażaj chęć otrzymywania krótkich filmów dotyczących zarządzania T2D; I
- posiadać smartfon lub, jeśli go nie posiada, chcieć i móc korzystać ze smartfona do nauki.
Kryteria wyłączenia:
- nie jest w stanie lub nie chce wyrazić świadomej zgody;
- niezdolność do znaczącego udziału w interwencji (np. nieskorygowane uszkodzenie wzroku i słuchu);
- nie chce zaakceptować przydziału randomizacji;
- jest w ciąży, planuje zajść w ciążę w ciągu najbliższych 6 miesięcy lub zajdzie w ciążę w trakcie badania; Lub
- karmi piersią (np. mogą mieć potencjalne ograniczenia dietetyczne).

Filmy DSMES zapewniają uczestnikom ważną edukację dotyczącą cukrzycy i pozwalają im stać się aktywnymi pacjentami na poziomie indywidualnym.
CHW ocenią bariery społeczne determinujące zdrowie (SDOH) uczestników w leczeniu cukrzycy typu 2 i powiążą je z dostępnymi zasobami w społeczności.
