Descrição
Critério de inclusão:
- identificar-se como imigrante chinês;
- ter entre 18 e 70 anos;
- ter diagnóstico de diabetes tipo 2 (DT2) no prontuário;
- teve uma consulta médica para cuidados de rotina com DM2 nos últimos 12 meses;
- ter uma HbA1c mais recente de pelo menos 8%;
- estar disposto a receber vídeos breves sobre o manejo do DM2; e
- possuir smartphone ou, caso não possua, estar disposto e ser capaz de utilizar um smartphone de estudo.
Critério de exclusão:
- incapaz ou sem vontade de fornecer consentimento informado;
- incapaz de participar de forma significativa na intervenção (por exemplo, deficiência visual e auditiva não corrigida);
- não está disposto a aceitar atribuição de randomização;
- está grávida, planeja engravidar nos próximos 6 meses ou engravida durante o estudo; ou
- está amamentando (por exemplo, eles podem ter possíveis restrições alimentares).

Experimental: VÍDEO
Os participantes designados para o braço VÍDEO receberão um breve vídeo de Educação e Apoio ao Autogerenciamento do Diabetes (DSMES) por semana durante 24 semanas.
Os vídeos serão entregues via mensagem de texto.

Os vídeos DSMES fornecem informações importantes sobre diabetes aos participantes e capacitam-nos a se tornarem pacientes ativados em nível individual.

Experimental: VÍDEO+CHW
Os participantes designados para o braço VÍDEO + CHW receberão um breve vídeo DSMES por semana, além de ligações de apoio quinzenais de um agente comunitário de saúde (CHW), durante 24 semanas.
Os vídeos do DSMES serão entregues via mensagem de texto.

Os vídeos DSMES fornecem informações importantes sobre diabetes aos participantes e capacitam-nos a se tornarem pacientes ativados em nível individual.
Os ACS avaliarão as barreiras dos determinantes sociais da saúde (SDOH) dos participantes para o tratamento do diabetes tipo 2 e vinculá-los aos recursos disponíveis na comunidade.

Participants assigned to the CONTROL group will continue to receive usual care.
At the end of the study, they will receive DSMES videos.
