Описание
Критерии включения:
- идентифицировать себя как китайский иммигрант;
- быть в возрасте 18-70 лет;
- иметь в медицинской карте диагноз сахарного диабета 2 типа (СД2);
- были на приеме у врача для планового лечения СД2 в течение последних 12 месяцев;
- иметь последний уровень HbA1c не менее 8%;
- будьте готовы получить краткие видеоролики о ведении пациентов с СД2; и
- иметь смартфон или, если у него его нет, иметь желание и возможность использовать учебный смартфон.
Критерий исключения:
- неспособен или не желает предоставить информированное согласие;
- неспособен значимо участвовать в вмешательстве (например, неисправленные нарушения зрения и слуха);
- нежелание принимать рандомизацию;
- беременна, планирует забеременеть в ближайшие 6 месяцев или забеременела во время исследования; или
- кормит грудью (например, у них могут быть потенциальные ограничения в питании).

Экспериментальный: ВИДЕО
Участники, назначенные в подразделение ВИДЕО, будут получать по одному краткому видеоролику по обучению и поддержке самоконтроля диабета (DSMES) в неделю в течение 24 недель.
Видео будут доставлены посредством текстового сообщения.

Видео DSMES дают участникам важное образование в области диабета и дают им возможность стать активными пациентами на индивидуальном уровне.

Экспериментальный: ВИДЕО+CHW
Участники, назначенные в подразделение VIDEO+CHW, будут получать одно короткое видео DSMES в неделю, а также раз в две недели звонки в службу поддержки от местного медицинского работника (CHW) в течение 24 недель.
