Beschrijving
Inclusiecriteria:
- zichzelf identificeren als een Chinese immigrant;
- tussen 18 en 70 jaar oud zijn;
- een diagnose van diabetes type 2 (T2D) hebben in het medisch dossier;
- in de afgelopen 12 maanden een afspraak heeft gehad met een arts voor routinematige T2D-zorg;
- een meest recente HbA1c van minimaal 8% hebben;
- bereid zijn korte video's te ontvangen over T2D-management; En
- beschikken over een smartphone of, als ze die niet hebben, een studiesmartphone willen en kunnen gebruiken.
Uitsluitingscriteria:
- niet in staat of bereid om geïnformeerde toestemming te geven;
- niet in staat om betekenisvol deel te nemen aan de interventie (bijvoorbeeld een ongecorrigeerde gezichts- en gehoorstoornis);
- niet bereid de randomisatieopdracht te aanvaarden;
- zwanger is, van plan is zwanger te worden in de komende zes maanden, of zwanger wordt tijdens het onderzoek; of
- borstvoeding geeft (ze kunnen bijvoorbeeld mogelijke dieetbeperkingen hebben).

Experimenteel: VIDEO
Deelnemers die zijn toegewezen aan de VIDEO-arm ontvangen gedurende 24 weken één korte Diabetes Self-Management Education and Support (DSMES)-video per week.
De video's worden via sms verzonden.

De DSMES-video's bieden belangrijke diabetesvoorlichting aan de deelnemers en stellen hen in staat om op individueel niveau een geactiveerde patiënt te worden.

Experimenteel: VIDEO+CHW
Deelnemers die zijn toegewezen aan de VIDEO+CHW-arm ontvangen één korte DSMES-video per week, naast tweewekelijkse ondersteuningsoproepen van een gemeenschapsgezondheidswerker (CHW), gedurende 24 weken.
De DSMES-video's worden via sms geleverd.

De DSMES-video's bieden belangrijke diabetesvoorlichting aan de deelnemers en stellen hen in staat om op individueel niveau een geactiveerde patiënt te worden.
CHW's zullen de sociale determinanten van gezondheidsbarrières (SDOH) van deelnemers voor diabeteszorg type 2 beoordelen en deze koppelen aan beschikbare middelen in de gemeenschap.

Participants assigned to the CONTROL group will continue to receive usual care.
At the end of the study, they will receive DSMES videos.
