Úvod operačního sálu založeného na virtuální realitě pro snížení úzkosti a chirurgického strachu u chirurgických pacientů
Účinek zavedení operačního sálu založeného na virtuální realitě na úzkost, chirurgický strach a spokojenost u pacientů s ortopedickou chirurgií
Vážený účastník,
Tato studie se nazývá „Účinek zavedení operačního sálu pomocí brýlí virtuální reality na úzkost, chirurgický strach a úroveň spokojenosti u pacientů podstupujících ortopedickou chirurgii“. Vaše účast je zcela dobrovolná a rozhodnutí se nezúčastnit neovlivní vaše léčba ani péči.
Cílem této studie je zhodnotit účinky zavedení operačního sálu pomocí brýlí virtuální reality na úzkost, chirurgický strach, pooperační úroveň bolesti, fyziologické parametry a spokojenost pacienta u pacientů podstupujících ortopedickou chirurgii. Pokud jsou výsledky studie pozitivní, budou jako účinný vzdělávací nástroj pro chirurgické pacienty doporučeny úvodní metody podporované virtuální realitou.
V této studii budou údaje shromažďovány prostřednictvím socio-demografických informací a několika dotazníků (Formulář pro zavedení pacienta, inventář úzkosti ve státních znacích, měřítko chirurgického strachu, numerické hodnocení). Navíc budou vaše fyziologické parametry a elektrodermální aktivita měřena pomocí monitorovacích zařízení a budete sledovat video představující prostředí operačního sálu s brýlemi virtuální reality. Toto video nebude pokrývat samotný chirurgický proces, ale poskytne obecné informace o převodu do operačního sálu, prostředí operačního sálu a pooperační služby. Studie začne jeden den před vaší operací a skončí před propuštěním.
Informace získané během této studie budou použity ve vědeckém výzkumu bez odhalení vaší identity a budou udržovány v souladu se zásadami důvěrnosti. Za vaši účast v této studii nebude žádný poplatek, ani vám nebude provedena žádná platba za vaši účast.

Detailní popis
Cílem této studie je vyhodnotit účinky prohlídky operačního sálu prováděného pomocí brýlí virtuální reality (VR) na úzkost, chirurgický strach, pooperační bolest, úroveň spokojenosti a fyziologické parametry u pacientů podstupujících ortopedickou chirurgii.
Intervence bude zahrnovat 3D vzdělávací video zavádějící prostředí operačního sálu, role chirurgických týmů a očekávané zkušenosti s pacientem během perioperačního období. Toto video bude představeno pomocí brýlí Meta Quest 3 VR. Účastníci intervenční skupiny budou sledovat toto video před operací, zatímco kontrolní skupina získá standardní předoperační vzdělávání.
Mezi primární výsledky patří změny v předoperační úzkosti (STAI-I), chirurgický strach (měřítko chirurgického strachu) a fyziologické parametry (např. Srdeční frekvence, krevní tlak a elektrodermální aktivita). Sekundární výsledky zahrnují pooperační bolest a spokojenost pacienta. Data budou shromažďována ve více časových bodech, včetně před a po intervenci, v den chirurgického zákroku a během pooperačního období.
Studie je navržena jako randomizovaná kontrolovaná studie s velikostí vzorku 60 účastníků, stejně distribuovaných mezi intervenční a kontrolní skupiny. Analýza dat bude zahrnovat vhodné statistické testy založené na distribuci proměnných a porovnání skupiny.

- Pacienti naplánovaní na artroskopickou chirurgii
- Ti, kteří nepodstavili žádné předchozí chirurgické zákroky
- Pacienti, kteří podstoupí chirurgický zákrok v celkové anestézii
- Jednotlivci bez komunikačních bariér
- Pacienti s jasným vědomím a orientací na osobu, místo a čas
- Ti, kteří nemají neurologické nebo psychiatrické poruchy
- Jedinci bez závažných vizuálních nebo sluchových postih
Kritéria pro vyloučení:
- Neschopnost komunikovat s pacienty
- Rozvoj života ohrožujícího zdravotního problému po operaci
- Přechod k otevření chirurgického zákroku během procedury
- Pacienti, kteří se chtějí odstoupit ze studie

For the Intervention Group: The intervention consisted of a researcher-developed 360-degree virtual reality operating room education video. The 9-minute 22-second video was recorded from the patient's perspective using an Insta360 X4 camera and presented with Meta Quest 3 VR headsets. It included transfer from the ward, handover to the surgical team, transfer to the operating table, safety and monitoring, anesthesia induction, emergence from anesthesia, and transfer to the ward. At relevant points, perioperative information was provided through researcher-prepared voice-over narration integrated into the video. Participants watched the video once on an examination bed/stretcher in examination rooms, positioned at approximately 30-45 degrees with a pillow under the head. A disposable hygienic headset strap cover was used. The same device, procedure, and video were used. The VR education was provided in addition to the institution's standard preoperative education.
