- 18-79 år gammel
Har en historie med en af følgende risikofaktorer for svær COVID:
- Astma
- Fysisk inaktivitet (defineret som <150 minutter moderat aktivitet om ugen eller <75 minutter intensiv aktivitet om ugen)
- HIV med CD4-tælling ≥ 500 celler/ul
- Nuværende eller tidligere ryger
- Depression eller anden sindslidelse
- Skizofreni spektrumforstyrrelse
- Cerebrovaskulær sygdom
- Hjertesvigt
- Koronar arterie sygdom
- Kardiomyopati
- Lungeemboli
- Lungehypertension
- Cystisk fibrose
- Bronkiektasi
- Kronisk obstruktiv lungesygdom
- Interstitiel lungesygdom
- Kronisk nyresygdom i stadium I eller II
- Stadium 1 defineret som normal GFR (> 90) men med andre tegn på nyreskade såsom proteinuri eller hematuri
- Stadium 2 defineret som en glomerulær filtrationsrate (GFR) på 60 - 89 ml/min/1,73m2
- Gestationsdiabetes
- Type 1 diabetes med seneste HgbA1C < 7,5%
- Type 2 diabetes med seneste HgbA1C < 7,5%
- Fedme med BMI ≥ 30 og < 40
- Leversygdom uden cirrose og med leverenzymniveauer
- (AST og ALT) ikke højere end tre gange den øvre normale grænse
- Militært sundhedssystem modtager og DEERS berettiget
- Villig til at blive randomiseret til at modtage enten Novavax COVID-19-vaccinen eller mRNA Pfizer-BioNTech COVID-19-vaccinen
- Vil være i stand til at vende tilbage til en klinikbesøg om cirka 30 dage og være i stand til at følge op online i de næste 9 måneder.
Eksklusionskriterier
- Historie med svær allergi eller svær bivirkning såsom myokarditis til enhver komponent af mRNA COVID-19-vaccinerne eller Novavax rekombinante COVID-19-vaccine
- Modtaget en COVID-19-vaccine inden for de sidste 3 måneder.
Testet positiv for COVID-19 inden for de sidste 3 måneder.
- Tilstedeværelse af feber, hoste, kulderystelser, åndenød, løbende næse eller ondt i halsen i dag på screenings/optagelsesbesøget.
Aktiv brug af immunmodulerende medicin.
- Defineret som aktiv brug af kronisk immunmodulerende medicin såsom systemiske kortikosteroider i en dosisækvivalens på 20 mg prednison eller højere dagligt i over en måned, kemoterapi, cytokinhæmmere, eller midler der reducerer T-celle eller B-celle antal eller funktion.
Diagnosticeret med immunkompromitteret tilstand.
- Defineret som: tilstedeværelse af en sygdom der aktivt forårsager svær immunsuppression eller historie med tidligere splenektomi (fjernelse af milten).
- Diabetes med den seneste HgbA1C ≥ 7,5.
Kronisk nyresygdom i stadium III eller højere
- Defineret som estimeret glomerulær filtrationsrate < 60 ml/min/1,73m2)
- Fedme med en BMI ≥ 40

Variant-specific immune responses
Tidsramme: IgG binding antibody levels and neutralizing antibody titers will be assessed on serum samples obtained just prior to vaccination, 30 days (+/- 10 days), and 150 days (+/- 10 days) after vaccination.
|
The primary endpoint is variant-specific immune response (magnitude and breadth) to licensed recombinant and mRNA COVID-19 products administered to adult MHS beneficiaries who are eligible for a fall COVID-19 vaccine and who are not severely immunocompromised.
This will include quantifying the magnitude of binding and neutralizing antibodies to the vaccine variants and to the dominant variant present 30 days and 150 days post-vaccination.
Specifically, we will test neutralizing titers (defined as the inverse serum dilution causing a 50% reduction in relative light units in a pseudovirus neutralization assay) and IgG binding antibody levels (measured in arbitrary units) against the following SARS-CoV-2 variants: XFG (vaccine strain), Wuhan-1 (ancestral strain), plus the dominant circulating variant(s) circulating during the fall of 2026 and winter of 2026-2027.

Antigenkortlægning og antistoflandskaber
Tidsramme: Antigene kartografistudier kan udføres ved hjælp af neutraliserende antistof-titer opnået lige før vaccination og 30 dage (+/- 10 dage) efter vaccination.

Antigen kartografi og antistoflandskaber kan udføres af FDA-samarbejdspartnere for at visualisere antigenisk drift fra vaccinen med sekventielle varianter, idet der udnyttes tidligere analysetilgange brugt af studieteamet.
Disse analyser bruger relative forskelle i neutraliserende antistofrespons til at generere todimensionelle kort (antigen kartografi) eller tredimensionelle landskaber, der gør det muligt at vurdere, hvor antigenisk forskellige virusstammer er fra hinanden.

Salivary and nasopharyngeal IgG and IgA antibody levels to vaccine SARS-CoV-2 strain and dominant SARS-CoV-2 circulating strain at 30 days and 5 months post vaccination will be compared between the study arms using GMT and GSD.
T-tests will compare responses across two SARS-CoV-2 variants (i.e., the vaccine variant and the dominant circulating variant at the time of vaccination), stratified by vaccine type and adjusted for multiple comparisons (e.g., a Tukey's adjustment).
A non-parametric comparison can be used if there is non-normally distributed data despite the use of GMT, these approaches also account for multiplicity (e.g., Kruskall-Wallis with a Dunn's adjustment for multiplicity adjustment).
