- 18-79 jaar oud
Een voorgeschiedenis hebben van een van de volgende risicofactoren voor ernstige COVID:
- Astma
- Lichamelijke inactiviteit (gedefinieerd als <150 minuten matige activiteit per week of <75 minuten intensieve activiteit per week)
- HIV met CD4-aantal ≥ 500 cellen/ul
- Huidige of voormalige roker
- Depressie of andere stemmingsstoornis
- Schizofreniespectrumstoornis
- Cerebrovasculaire aandoening
- Hartfalen
- Coronaire hartziekte
- Cardiomyopathie
- Longembolie
- Pulmonale hypertensie
- Cystic fibrosis
- Bronchiëctasieën
- Chronische obstructieve longziekte
- Interstitiële longziekte
- Stadium I of II chronische nierziekte
- Stadium 1 gedefinieerd als normale GFR (> 90) maar met andere tekenen van nierschade zoals proteïnurie of hematurie
- Stadium 2 gedefinieerd als een glomerulaire filtratiesnelheid (GFR) van 60 - 89 ml/min/1,73m²
- Zwangerschapsdiabetes
- Type 1 diabetes met meest recente HgbA1C < 7,5%
- Type 2 diabetes met meest recente HgbA1C < 7,5%
- Obesitas met BMI ≥ 30 en < 40
- Leverziekte zonder cirrose en met leverenzymwaarden
- (AST en ALT) niet meer dan drie keer de bovengrens van normaal
- Begunstigde van het Military Health System en in aanmerking komend voor DEERS
- Bereid om gerandomiseerd te worden om het Novavax COVID-19-vaccin of het mRNA Pfizer-BioNTech COVID-19-vaccin te ontvangen
- In staat om terug te komen voor een kliniekbezoek binnen ongeveer 30 dagen en in staat om online follow-up te doen gedurende de komende 9 maanden.
Exclusiecriteria
- Voorgeschiedenis van ernstige allergie of ernstige bijwerking zoals myocarditis op enig onderdeel van de mRNA COVID-19-vaccins of het Novavax recombinant COVID-19-vaccin
- Een COVID-19-vaccin ontvangen in de afgelopen 3 maanden.
Positief getest op COVID-19 in de afgelopen 3 maanden.
- Aanwezigheid van koorts, hoest, koude rillingen, kortademigheid, loopneus of keelpijn vandaag op de dag van het screenings-/inschrijvingsbezoek.
Actief gebruik van immuunmodulerende medicatie.
- Gedefinieerd als actief gebruik van chronische immuunmodulerende medicatie zoals systemische corticosteroïden in een dosisequivalent van 20 mg prednison of meer dagelijks gedurende meer dan een maand, chemotherapie, cytokine-remmers, of middelen die T-cel- of B-celaantallen of -functie verminderen.
Gediagnosticeerd met immunogecompromitteerde toestand.
- Gedefinieerd als: aanwezigheid van een ziekte die actief ernstige immuunsuppressie veroorzaakt of voorgeschiedenis van eerdere splenectomie (verwijdering van de milt).
- Diabetes met de meest recente HgbA1C ≥ 7,5.
Stadium III of hoger chronische nierziekte
- Gedefinieerd als geschatte glomerulaire filtratiesnelheid < 60 ml/min/1,73m²)
- Obesitas met een BMI ≥ 40
- HIV met een CD4-celaantal < 500 cellen/ul
- Voorgeschiedenis van vaste orgaan- of beenmergtransplantatie.
Actieve maligniteit
- Gedefinieerd als elke kanker die momenteel wordt behandeld of waarvan in het afgelopen jaar bewijs van progressie is aangetoond.
- Chronische leverziekte met gecompenseerde of gedecompenseerde cirrose, of leverenzymwaarden (AST of ALT) hoger dan drie keer de bovengrens van normaal.

Variant-specific immune responses
Tijdsspanne: IgG binding antibody levels and neutralizing antibody titers will be assessed on serum samples obtained just prior to vaccination, 30 days (+/- 10 days), and 150 days (+/- 10 days) after vaccination.
|
The primary endpoint is variant-specific immune response (magnitude and breadth) to licensed recombinant and mRNA COVID-19 products administered to adult MHS beneficiaries who are eligible for a fall COVID-19 vaccine and who are not severely immunocompromised.
This will include quantifying the magnitude of binding and neutralizing antibodies to the vaccine variants and to the dominant variant present 30 days and 150 days post-vaccination.
Specifically, we will test neutralizing titers (defined as the inverse serum dilution causing a 50% reduction in relative light units in a pseudovirus neutralization assay) and IgG binding antibody levels (measured in arbitrary units) against the following SARS-CoV-2 variants: XFG (vaccine strain), Wuhan-1 (ancestral strain), plus the dominant circulating variant(s) circulating during the fall of 2026 and winter of 2026-2027.

Antigenische cartografie en antilichaamlandschappen
Tijdsspanne: Antigenische cartografiestudies kunnen worden uitgevoerd met neutraliserende antilichaamtiters die vlak vóór vaccinatie en 30 dagen (+/- 10 dagen) na vaccinatie zijn verkregen.

Antigen cartografie en antilichaamlandschappen kunnen door FDA-samenwerkingspartners worden uitgevoerd om antigene drift van het vaccijn met opeenvolgende varianten te visualiseren, gebruikmakend van eerdere analysemethoden die door het onderzoeksteam zijn gebruikt.
Deze analyses gebruiken relatieve verschillen in neutraliserende antilichaamresponsen om tweedimensionale kaarten (antigene cartografie) of driedimensionale landschappen te genereren die het mogelijk maken te beoordelen hoe antigeen verschillende virusstammen van elkaar zijn.
Antilichaamlandschappen zijn nuttige hulpmiddelen die verschillende meervoudige seriële antigene blootstellingen kunnen vastleggen in vergelijkingen van antilichaamtiter en -breedte tussen individuen die zijn toegewezen om de recombinante of mRNA-vaccins te ontvangen (zie voorbeelden van Wang et al., Cell Host Microbe, 2022).

Salivary and nasopharyngeal IgG and IgA antibody levels to vaccine SARS-CoV-2 strain and dominant SARS-CoV-2 circulating strain at 30 days and 5 months post vaccination will be compared between the study arms using GMT and GSD.
T-tests will compare responses across two SARS-CoV-2 variants (i.e., the vaccine variant and the dominant circulating variant at the time of vaccination), stratified by vaccine type and adjusted for multiple comparisons (e.g., a Tukey's adjustment).
A non-parametric comparison can be used if there is non-normally distributed data despite the use of GMT, these approaches also account for multiplicity (e.g., Kruskall-Wallis with a Dunn's adjustment for multiplicity adjustment).
